斑点—酶联免疫吸附试验检测卡氏肺孢子虫

应用完整的卡氏肺孢子虫包囊作抗原，以４×１０＾７／ｍｌ包裹点样，作斑点－酶联免疫吸附试验检测肺印片卡氏肺孢子虫包囊阳性的ＢＡＬＢ／ｃ小鼠血清２０份，同时检测包囊阴性的小鼠血清２０份，结果除一份小鼠血清ＩｇＧ阴性反应外，均为阳性反应，阳性率为９５％；而正常对照血清全为阴性反应。另外，检测了３份纤支镜刷检物中包囊阳性的病人血清，均为阳性反应；５０例健康孕妇血清的检查，抗体阳性率为１２％。     

红霉素对犬幽门括约肌压力影响及其与胃动素,生长抑素的关系

为探讨红霉素对犬幽门括约肌的作用及其机理，及其与血浆胃动素，生长抑素的关系。方法采用胃压力测量仪及放射免疫法，同步监测观察了１０条犬静脉滴ＥＭ前后和使用拮抗剂后的幽门压力、血浆ＭＴＬ，ＳＳ的变化，结论：ＥＭ具有增高犬胃幽门压力的作用，其作用机理除与血浆ＭＴＬ有着密切的关系外，血浆ＳＳ也可能共同参与了其调节机制。     

Role of granulocyte colony-stimulating factor as adjunct therapy for septicemia in children with acute leukemia

The granulocyte colony-stimulating factor (G-CSF) has been shown to accelerate recovery from severe neutropenia and to decrease the incidence of documented infections after intensive chemotherapy in cancer patients. However, the routine prophylactic use of G-CSF is expensive. This study was conducted to determine the role of G-CSF as adjunct therapy for septicemia following neutropenia caused by chemotherapy in children with acute leukemia. Fifty consecutive episodes of septicemia were studied involving 34 episodes of Gram-negative, 7 episodes of Gram-positive, 5 episodes of polymicrobial bacterial septicemia, one episode of fungemia, and 3 episodes of disseminated fungal infection. In the first 25 episodes, G-CSF was not used (group A). For the next 16 episodes, G-CSF 200 μg per square meter per day subcutaneously was given immediately after the septicemia was documented until the absolute neutrophil count was maintained at more than 1,500 per cubic millimeter (group B). Thereafter, G-CSF at the same dose as that of group B was prophylactically used in all the children who received high-dose cytosine arablnc-side-containing regimens. Nine episodes of septicemia occurred (group C). The incidences of mortality per episode of septicemia in groups A, B, and C were 12.0% (3/25), 12.5% (2/16) and 0% (0/9), respectively. Statistically, there was no difference between the three groups overall and in pair-wise comparisons (all P > 0.5). The durations of G-CSF administration in group B ranged from 6 to 26 days with a median of 12 days and the durations of G-CSF administration in group C ranged from 10 to 23 days with a median of 19 days. With or without G-CSF, there may be no significant difference in the mortality of septicemia following neutropenia caused by chemotherapy in children with acute leukemia.     

单用体外震波碎石治疗尿路结石的体会

本文报道1988年6月～11月,单独应用上海交通大学电力工程系研制的JT—ESWL—Ⅱ型体外震波碎石机,治疗尿路结石506例,其中肾结石304例(双侧12例)输尿管结石199例(双侧5例),膀胱结石3例。冲击能量15～40焦耳,每次治疗冲击波次数为400～2800次。本组无开放手术、无肾脏丧失、无死亡。效果满意者96.44％。其并发症主要有血尿、结石串。作者认为单用ESWL治疗小于3Cm的尿路结石安全可靠、无严重并发症。文中对单用体外震波碎石(ESWL)治疗尿路结石病例选择,如何减少并发症,提高治疗效果进行了讨论。     

左心室舒张(充盈)功能的影响因素

本文用X线电影摄影方法评价了49例左室舒张(充盈)功能的影响因素。冠心病(CAD)组23例,高血压病组12例,正常对照组14例。左室充盈功能或顺应性主要受冠脉病变严重程度的影响。冠脉狭窄程度越重,并有陈旧性心肌梗塞、室壁运动异常、EF下降,则充盈功能受损愈明显。血压、年龄因素对舒张充盈功能的影响较小。在CAD组中,心率与1/3充盈分数呈负相关,与正常化高峰充盈率(NPFR)不相关,而EF则与NPFR相关良好。     

重视药物利用评价研究,开展药物利用评价活动

药物利用评价是当前医院合理用药的深入发展的重要体现,也是临床药学的重要工作之一。本文论述了药物利用评价的目的、意义、方法和进展,以及药物利用评价在医院药房中的应用,以期引起有关人员的重视。     

Low sodium content formula for infants with congestive heart failure

Twenty-two infants of isolated ventricular septal defect with congestive heart failure were fed with lower-sodium content formula-Lonalac (Mead-Johnson) to study the clinical response of treatment for congestive heart failure. There were no significant changes of intake, urinary output, serum sodium, potassium and osmolality before, 2 days and 6 days after Lonalac feeding. The low sodium content formula may feed the infants with congestive heart failure in addition to the traditional anticongestive therapy.     

实验性尘细胞灶与胸片细网状阴影形成的关系

本文从21只实验性矽肺家兔中筛选出5只胸片有细网状阴影者,用病理与 X 线对照观察的方法,探讨了实验性矽肺尘细胞灶与死后胸片细网状阴影形成的关系。证明,分布在肺细叶内直径约1mm 的尘细胞灶,其分布密度为113个/cm~2以上时,可构成胸片细网状阴影。实验排除了弥漫性肺间质纤维化的结论。并对胸片细网状阴影的形成从肺细叶解剖与 X 线投影进行了讨论。     

Docosahexaenoic acid is superior to eicosapentaenoic acid as the essential fatty acid for growth of grouper, Epinephelus malabaricus.

Juvenile grouper (Epinephelus malabaricus) were fed seven experimental diets, one control diet and one reference diet for 12 wk to determine the dietary requirement of grouper for docosahexaenoic (DHA) and eicosapentaenoic (EPA) acids. Each of the seven diets contained 1 g/100 g DHA and EPA in various combinations and 9 g/100 g tristearin. The control diet contained 1 g/100 g trilinolenin and trilinolein (3:1, wt/wt), and no supplemental EPA or DHA. The reference diet contained only natural oils from a mixture of cod liver oil, linseed oil and safflower oil at a ratio of 2:1:1 (wt/wt/wt). Significant differences (P < 0.05) in growth were observed among the dietary treatments but not in survival rate or relative liver weight. Only the diet with the highest DHA/EPA ratio (3:1) promoted significantly greater growth than the control diet. Purified EPA and DHA did not perform better in promoting growth than did the impure EPA and DHA oils. Enhanced growth was observed when the dietary DHA/EPA ratio was greater than 1, indicating that DHA was superior to EPA in promoting fish growth. Neutral lipid (NL) was the predominant lipid fraction (>70%) in both liver and muscle. Tissue NL/polar lipid did not differ among groups except the reference diet group that had a higher ratio (P < 0.05). DHA and EPA levels in the grouper tissues, especially muscle, were highly reflective of dietary levels of DHA and EPA, indicating that direct incorporation was likely. In addition, the 20:1(n-9), concentration in NL fractions seems to be an appropriate indicator of dietary essential fatty acid deficiency in grouper.     

应用同种动脉治疗主动脉根部病变

报告５例采用同种动脉（ＨＡＶ）治疗５例主动脉根部病变（主动脉瓣狭窄１例，关闭不全４例）。男３例，女２例；年龄１３～４７岁。病因为细菌性心内膜炎２例（包括先天主动脉瓣二瓣化畸型１例），Ｍａｒｆａｎ综合征２例，风湿性主动脉瓣关闭不全１例。术中采用同种主动脉全根置换３例（Ｂｅｎｔａｌｌ手术），其中１例用ＨＡＶ上的二尖瓣前叶加宽主动脉瓣环（Ｍａｎｏｕｇｕｉａｎ手术），余２例用自体肺动脉移置主动脉根部，另取同种肺动脉做原位移置（Ｒｏｓｓ手术）。结果死亡１冽，４例存活，且无并发症发生。术后超声心动图提示主动脉根部良好，无主动脉瓣反流。随访２．５～３．５年疗效满意。
