Chronic anterior pallidal stimulation for parkinson's disease   总被引:4,自引:0,他引:4  
Summary We present the case of a 51-year-old female who had a four-year history of Parkinson's disease with severe on-off and disabling progression of symptoms on chronic levodopa therapy.After obtaining FDA approval, we implanted a Medtronic deep brain stimulation lead stereotactically into the right anterior pallidum contralateral to her most Symptomatologic side. Intra-operative stimulation trials at 100 Hz caused reproducible reversal of akinetic symptoms and simultaneous microelectrode recording of the posteroventral pallidum revealed decreased neural activity during anterior pallidal stimulation.The patient was evaluated pre-operatively and postoperatively using the Hoehn and Yahr Staging Scale, the Unified Parkinson's Disease Rating Scale (UPDRS), videotape, and a computerized data glove. Six months after implantation, the total UPDRS score was decreased from 68 to 8 and Hoehn and Yahr Staging improved from 3.0 to 1.5 during periods of chronic high frequency stimulation. Dramatic improvements in tremor, dystonia, bradykinesia, and akinesia were noted within seconds of stimulator activation and were also objectively measured using a computerized data glove. This case reveals the potential for therapeutic pallidal stimulation for Parkinson's akinetic symptomatology.  相似文献

Background

The stool color card has been the primary tool for identifying acholic stools in infants with biliary atresia (BA), in several countries. However, BA stools are not always acholic, as obliteration of the bile duct occurs gradually. This study aims to introduce Baby Poop (Baby unchi in Japanese), a free iPhone application, employing a detection algorithm to capture subtle differences in colors, even with non-acholic BA stools.

Methods

The application is designed for use by caregivers of infants aged approximately 2 weeks–1 month. Baseline analysis to determine optimal color parameters predicting BA stools was performed using logistic regression (n = 50). Pattern recognition and machine learning processes were performed using 30 BA and 34 non-BA images. Additional 5 BA and 35 non-BA pictures were used to test accuracy.

Results

Hue, saturation, and value (HSV) were the preferred parameter for BA stool identification. A sensitivity and specificity were 100% (95% confidence interval 0.48–1.00 and 0.90–1.00, respectively) even among a collection of visually non-acholic, i.e., pigmented BA stools and relatively pale-colored non-BA stools.

Conclusions

Results suggest that an iPhone mobile application integrated with a detection algorithm is an effective and convenient modality for early detection of BA, and potentially for other related diseases.

We studied vascular structure of the rabbit placenta, especially on three-dimensional morphological patterns and developmental process. Basic structure of maternal arterial system was re-constructed during day 13–18 of pregnancy, forming main routes for blood supply through the arterial sinuses and radial arteries. Intra-villous spaces were drastically developed showing as branches from the terminal radial arteries. Fetal arterial system was generated accompanied with maternal vascular development, showing characteristic features such as the perforating linear artery, hairpin flexion, and circular anastomoses in the capillaries. From the correlation of maternal and fetal blood currents, gas-exchange style in the rabbit placenta was considered as counter-current and pool mixed patterns. These data demonstrated an original feature for the placental arterial systems in rabbits, which differed from other animals having a property for discoid placenta.  相似文献

Background contextIn the quest for clinically functional artificial intervertebral discs (AIDs), multidisciplinary technologies have been employed. Existing solid mobile AIDs essentially consist of the superposition of solid plates and core materials; however, it is thought that an ideal surgical AID technology has not yet been developed. To overcome the limitation of these existing AIDs, we developed a unique flexible AID disc system on the basis of our original biomimetic concept. The AID is composed of a cubic three-dimensional fabric (3DF) with a triaxial fiber alignment, which offers biomimetic long-term dynamic mechanical behavior along with durability.PurposeThis article substantiates the potential clinical use of the 3DF disc system that quite differs from existing ones.Study designWe designed the lumbar and cervical 3DF discs that improved the structural weaknesses caused by the collagenous fiber alignment of biological intervertebral disc. Bioresorbable hydroxyapatite particles were deposited on the surface layer of the 3DF disc to promote new bony ingrowth and to ensure secure binding at the interface of the contacting vertebral bodies. A stand-alone system was devised for surgical reliability in terms of both positioning and fixation, allowing tight press fitting with the vertebral bodies. Bioactive and bioresorbable pins were penetrated through the 3DF disc body and projected from the surface to allow ideal insertion and fixation to the disc space, preserving the precise position during dynamical movement. In vitro endurance of the 3DF disc was examined under long-term alternating stresses, and the in vivo animal tests were conducted in the intervertebral lumbar discs at L5–L6 excised from baboons and replaced with the lumbar 3DF disc.MethodsThe static mechanical endurance was assessed through a creep test. In vitro endurance of the 3DF disc under repetitive stresses including axial compressing, flexion-extension, torsional twisting, and lateral bending were applied to the 3DF disc for a long-term for up to 105 million stresses, which is roughly equivalent to exposure of natural biological movement for more than 50 years. In the animal test, eight baboons were euthanized 6 months postoperatively. To their extracted spines, six pure moments (flexion and extension, left and right lateral bending, and left and right torsion) were applied vertically to the superior end of the specimen and then values of range of motions (ROMs) were calculated. Histological analyses were conducted on 12 reticuloendothelial and systemic tissues.ResultsThe 3DF disc retained its biomimetic “J-shaped” stress-strain behavior without generating wear debris for up to 105 million stresses. A 130-N loading for the creep test decreased the height of 0.3 mm during 80 to 1,000 hours. In the biomechanical test, ROM values of axial rotation and flexion-extension showed no significant difference from the intact excluding that of lateral bending because the location of each pin to stand alone certainly controlled the bending behavior only. The histological analysis indicated no significant pathologic changes induced by the 3DF disc.ConclusionsThe 3DF disc system is clinically suitable for human disc replacement arthroplasty based on the findings of long-term durability with dynamic motion in vitro and effective animal tests in vivo. This system surely overcomes the limitations of existing solid AIDs, and the clinical potential of the biomimetic 3DF discs has been verified. This new biomaterial technology delivers most of the functions and characteristics required by a clinically available AID if applied correctly by surgeons.  相似文献

Five hundred fifty patients hospitalized to our hospital during 1990 to 1999 were studied. Subjects consisted of 413 males and 102 females and mean age was 62.1 years. Association with HBV infection, HCV infection and both infection was 11.1%, 78.4% and 2.5% respectively. According to the criteria based in Liver Cancer Study of Japan, 5 year survival rate in clinical stage I, II, III was 42.3%, 38.8% and 17.5%. Tumor morphology was nodular type in 78.9%, massive type in 9.1% and diffuse type in 10.5% of cases. Portal tumor thrombus and distal metastasis were observed in 19.9% and 6.3% of all cases. The 1-, 3- and 5-year survival rate in patients received any therapy was 80.8%, 44.6% and 28.7%, respectively. Analysis by the proportional hazard model showed that HBV infection, advanced clinical stage, multiple tumors, a tumor diameter in excess of 5 cm and AFP positivity were shown as significant factors on poor prognosis.  相似文献

We report a 20-year-old man with intractable myasthenia gravis successfully treated with total lymphoid irradiation (TLI). An encapsulated thymoma in the anterior mediastinum was resected as extended thymectomy by video-assisted thoracoscopic surgery at 2 months after the onset of ptosis and muscle weakness. Following treatments, such as ambenonium hydrochloride, an immunosuppression therapy (prednisolone and azatioprine), 5 courses of immunoadsorption therapy, and a high dose of cyclophosphamide and methylprednisolone, did not make persistent improvement of myasthenic symptoms. Ageusia occurred twice prior to myasthenic crises and subsided with other myasthenic symptoms after treatments. Steroid psychosis and secondary Cushing's syndrome made us to reduce the dose of prednisolone. Post-operative residual, recurrent, or metastatic thymus was not unveiled, then we added the low dose fractionated irradiation (1.5 Gy x 12 = 18 Gy) to the mediastinum. Three months after the irradiation, however, a crisis occurred and the titer of anti-acetylcholine receptor antibody increased up to 100 nmol/l. Therefore, we performed TLI (Mantle; 1.5 Gy x 9 = 13.5 Gy, paraaortic and inverted-Y; 1.5 Gy x 14 = 21 Gy), which brought about persistent improvement of myasthenic symptoms with decrease in the titer of anti-acetylcholine receptor antibody.  相似文献

We report a 4-year-old girl who underwent replacement of a bioprosthetic valve in the tricuspid position due to rare bioprosthetic dysfunction. The bioprosthetic valve showed marked tricuspid insufficiency as well as stenosis caused by the adhesion of the preserved native valve leaflets to the undersurface of the implanted bioprosthesis.  相似文献

Purpose

Surgery for lumbar spinal degeneration disease is widely performed. While posterior decompression and fusion are popular, anterior lumbar interbody fusion (ALIF) is also used for treatment. Extreme lateral interbody fusion (XLIF) is commonly used for noninvasive ALIF; however, several complications, such as spinal nerve and psoas muscle injury, have been reported. In the current study, we examined the clinical efficacy and complications of oblique lateral interbody fusion (OLIF) for lumbar spinal degeneration disease.

Materials and Methods

Thirty-five patients with degenerated spondylolisthesis, discogenic pain, and kyphoscoliosis were examined. All patients underwent OLIF surgery (using a cage and bone graft from the iliac crest) with or without posterior decompression, without real-time electromyography monitoring. Posterior screws were used in all patients. Visual analog scale (VAS) score and Oswestry Disability Index (ODI) were evaluated before and 6 months after surgery. Surgical complications were also evaluated.

Results

Pain scores significantly improved after surgery, compared to those before surgery (p<0.05). There was no patient who underwent revision surgery. There was no spinal nerve, major vessel, peritoneal, or urinary injury. Few patients showed symptoms from psoas invasion.

Conclusion

OLIF surgery produced good surgical results without any major complication.  相似文献
