奥美拉唑静脉给药治疗上消化道大出血的疗效观察

本文阐述了奥美拉唑（Ｏｍｅｐｒａｚｏｌｅ）治疗急性上消化道大出血８０例与法莫替丁（Ｆａｍｏｔｉｄｉｎｅ）治疗上消化道大出血５０例比较，前者有效率明显高于后者。提示奥美拉唑治疗急性上消化道大出血效果好，无副作用，减少了手术率、死亡率，是一种有效的止血药物。     

天然水中三卤甲烷前体活性结构的研究

通过实验证实，可用腐植酸（ＨＡ）代表天然水中三卤甲烷（ＴＨＭｓ）的前体。单一模拟化合物（含一个活性空位碳原子的间苯二酚，三个活性空位碳原子的间苯三酚，及甲基酮类化合物的丙酮酸钠）氯化后氯信形成的规律与氯化ＨＡ的不一致。而间苯二酚与丙酮酸钠的混合物（１：４Ｗ／Ｗ）能很好地反映出ＨＡ中活性结构的特性。为今后研究ＴＨＭｓ的形成提供了一个可资定量比较的ＴＨＭｓ前体。     

Effects of chimeric molecule of recombinant Dsg3EC1-2 with toxin PE40 on T and B lymphocytes in Pemphigus Vulgaris

Objective:To observe the effects of the recombinant chimeric toxin Dsg3EC1-2PE40 on T and B lymphocytes isolated from Pemphigus Vulgaris (PV) patients to further study its biological therapeutic function for PV. Methods :Recombinant chimeric toxin Dsg3EC1-2PE40 was first identified, expressed and purified, and then its effects on T and B lymphocytes of PV patients in vitro were detected and quantified by ELISPOT assay and MTT assay. Results :The purity of the expressed protein Dsg3EC1-2PE40 was up to 80%. In ELISPOT assay, with Dsg3EC1-2PE40, the overall number of B cells that produce anti-Dsg3 antibodies among PV patients was only about 60% of the comparable number with Dsg3EC1-2. The proliferation of T cells of PV patients was inhibited markedly by Dsg3EC1-2PE40. There was significant difference between the different groups with Dsg3EC1-2PE40 and Dsg3EC1-2. Conclusion:The recombinant chimeric toxin Dsg3EC1-2PE40 decrease the number of B cells that produce anti-Dsg3 antibodies in PV patients and can inhibit or kill T cells of PV patients in vitro.     

非惊厥性癫痫持续状态

痫持续状态依据其临床发作形式分为惊厥性和非惊厥性，惊厥性痫持续状态临床表现鲜明，临床医师对其有肯定而详尽的认识，易于诊断。国外报道非惊厥性癫痫持续状态(NCSE)约占儿童癫痫持续状态的20％～25％，但临床中表现多样，且症状、体征轻微，常合并其他综合征、被原发病掩盖、或误诊为其他疾病(如抑郁症、精神病、癔症、脑炎、代谢性脑病、癫痫后状态等)而延误合理的治疗，     

抑郁发作自杀未遂患者脑源性神经营养因子水平及其相关因素研究

Objective To investigate the possible role of brain-derived neurotrophic factor(BDNF)in the pathophysiology of attempted suicide in depressive episode patients.Methods The subjects were 23 depressive episode patients who had recently attempted suicide.24 depressive episode patients without suicide attempt.and 30 sex-age-and education matched healthy controls.The Hamihon Depression Rating Scale-24 items(HAMD24),Beck Helpless Rating Scale(BHS)and Self-rating Idea of Suicide Scale (SIOSS)was used to evaluate the severitv of symptoms.The seruln level of BDNF was detected with sandwich ELISA.Pearson correlation coefficients wag used to measure the correlations between serum BDNF and the related illness variables.Results There were significant difierences in the serum level of BDNF between patients with attempted suicide,pailents without suicide attempt and normal controls(F=21.927,P＜0.05).Post hoc analysis showed the serum BDNF levels in patients with attempted suicide [(57 ±16)ng/L]were lower than that in patients without[(75±28)ns/L;P＜0.05],and were lower in patients without suicide attempt[(75 ±28)ng/L]than that in normal controls[(111 ±39)ng/L;P＜0.01].In patients with attempted suicide,the serum BDNF was negatively correlated to the duration of depressive episode(r=-0.541,P=0.008),the mean BHS score(r=-0.494,P=0.017)and the mean SIOSSscore(r=-0.754.P=0.000).Conclusion The results suggest that the decreased serum brain-derived neurotrophic factor is related to the pathophysiology of attempted suicide in depressive episode patients.     

大剂量适速连续静滴5-Fu／CF配合DDP治疗胰腺癌探讨

目的 评价大剂量适速连续静滴5-Fu／CF配合DDP治疗胰腺癌的疗效。方法 将40例病人随机分为两组，治疗组20例，用大剂量适速连续静滴5-Fu／CF配合DDP静滴方案治疗；对照组20例，小剂量间断多次5-Fu、CF、DDP静滴方案治疗，观察其疗效，毒副作用。结果治疗组总有效率高于对照组，肠道反应小，白细胞变化小，肝肾功能受损轻，免疫功能变化小。结论 大剂量适速连续静脉滴注5-Fu／CF化疗方法是治疗胰腺癌的一种有效的辅助手段之

患者 ,男 ,33岁。因皮肤粘膜发绀 33年 ,阵发性头痛 7年 ,加重伴呕吐半月入院。查体 :神清精神差 ,言语不清 ,皮肤粘膜发绀 ,杵状指、趾 ,SPO2 75 % ,Hb 2 30 g/L。双肺听诊呼吸音稍粗糙 ,未闻及干、湿音。心脏听诊胸骨左缘 2～ 3肋间闻及喷射样杂音。肝肋下未扪及 ,叩击痛可疑阳性。辅助检查 :心电图示右房右室极度长大 ,增厚 ;超声心动图示 :肺动脉狭窄 ,室间隔缺损 ,主动脉骑跨 ,心室心房长大。术前诊断 :1法乐四联症 ,心功 ～ 级。 2右枕叶脓肿。患者经短期营养支持治疗后 ,在静吸复合麻醉下行右枕脓肿切除术。术前给予东莨菪碱 0…
