Human T‐cell leukemia virus type I (HTLV‐1) infection is endemic in Japan, particularly clustered in the southwestern district, Kyushu‐Okinawa, which consists of eight prefectures that further consist of 274 municipalities. However, no information is available about the fine‐scale distribution of HTLV‐1 infection within Kyushu‐Okinawa. To assess the municipal‐level distribution of people with HTLV‐1 infection in Kyushu‐Okinawa, we performed a cross‐sectional study using a fine‐scale geographic information system map based on HTLV‐1 screening test results from the Japanese Red Cross database from September 2012 to February 2014. Of the 881 871 (646 914 male, 234 957 female) screened blood donors, 981 were seropositive for HTLV‐1 by confirmatory test. The seroprevalence was 0.11% (95% confidence interval [CI] 0.10%‐0.12%) for all, 0.094% (95% CI, 0.09%‐0.10%) for male, and 0.16% (95% CI, 0.14%‐0.18%) for female individuals. The sex‐ and age‐specific HTLV‐1 seroprevalence varied significantly across municipalities; particularly, the seroprevalence among women aged 50 years was significantly higher than that of men in both the mainland of Kyushu‐Okinawa and the satellite island, in all of which the seroprevalence of HTLV‐1 was more than 1.2%. These results show that, even in the Kyushu‐Okinawa district, there are endemic clusters of HTLV‐1 in small areas. This suggests that public health education programs are needed to eliminate new HTLV‐1 infection in these areas.  相似文献

Potential effectiveness of stored cord blood (non-frozen) for emergency use   总被引:1,自引:0,他引:1  
Bone marrow has been used for a number of years to assist patients who have accidentally received potentially lethal levels of irradiation. The intent of the transplant is to replace the victim's own bone marrow that has been injured from the irradiation or to act as temporary support to allow the patient's own marrow to recover. Following the Chernobyl disaster, some victims received bone marrow that was HLA matched or partially matched. However, donor marrows were difficult to obtain in adequate numbers; as a substitute for bone marrow, frozen fetal liver cells were used as a source of hematopoietic stem cells. The use of fetal livers, however, was unsuccessful. Human umbilical cord blood, currently considered an excellent source of hematopoietic stem cells, was not used at Chernobyl. For several years, we have been able experimentally to keep SJL/J mice alive with the use of human umbilical cord blood after the animals received lethal levels of irradiation. This finding suggests that under certain conditions human cord blood does not have to be HLA matched to facilitate rescue from irradiation. In addition, there are reports of unmatched HLA cord blood being used successfully for marrow transplantation. If human cord blood does not have to be matched for HLA, there may be emergency cataclysmic circumstances where the availability of umbilical cord blood may be of considerable value. To simulate a clinical situation such as a nuclear accident, in which human cord blood might serve as a source of stem cells for marrow transplantation, we attempted to rescue immunocompetent mice after 900 cGY of irradiation with the use of (nonfrozen) human cord blood stored in a blood bank. The blood was stored under routine conditions (3–6 °C) for 5 and 7 days in special bags that allow transmission of oxygen. Following lethal levels of irradiation, the cord blood was administered to the animals and a significant survival rate was obtained.  相似文献

Nalbuphine analgesia in the prehospital setting   总被引:3,自引:0,他引:3  
Forty-six patients with moderate to severe pain caused by orthopedic injuries, burns, multiple trauma, or intraabdominal conditions were treated with intravenous (IV) nalbuphine hydrochloride (Nubain; DuPont Pharmaceuticals, Wilmington, DE) by paramedics before arrival at the hospital. Patients who weighed less than 60 kg received 15 mg nalbuphine, and patients weighing greater than 60 kg received 20 mg nalbuphine. Forty-one of 46 patients (89%) experienced pain relief from nalbuphine, with maximum relief occurring within 15 minutes after the administration of the drug. Two addicted patients received no pain relief. There were no untoward side effects following nalbuphine administration, and the patients' heart rates, mean arterial pressures, and respiratory rates remained constant and stable throughout the study period. Repeated assessment of the patient by paramedics in the field was not impaired by nalbuphine treatment. In summary, nalbuphine hydrochloride is a useful and safe analgesic drug for IV use by paramedics in the prehospital setting.  相似文献

Current echocardiographic devices provide only 2-dimensional views of the heart. To appreciate 3-dimensional structural relations, therefore, requires mental reconstruction of 2-dimensional views by an experienced observer. Our ability to answer new questions about the heart could be increased if 2-dimensional images could be combined to display 3-dimensional relations. Such 3-dimensional reconstruction would permit analysis of structures of unknown or complex shape and the noninvasive quantification of cardiac chamber size and function without making geometric assumptions. To overcome previous limitations, mechanisms have been developed for automated integration of images and positional data during routine echocardiographic scanning, thereby greatly enhancing the efficiency and application of image reconstruction. Refining the diagnosis of mitral valve prolapse has presented a uniquely 3-dimensional problem requiring information previously unavailable from the 2-dimensional technique. To date, 3-dimensional studies have demonstrated that the mitral valve is saddle-shaped in systole, so that apparent superior leaflet displacement in the mediolateral 4-chamber view, often seen in otherwise normal individuals, lies entirely within the bounds defined by the mitral annulus and occurs without leaflet distortion or actual displacement above the entire mitral valve. Other applications of 3-dimensional image reconstruction include calculation of ventricular volume and ejection fraction by transthoracic or transesophageal scanning without geometric assumptions; improving the standardization and accuracy of 2-dimensional measurements by improving spatial appreciation; and 3-dimensional reconstruction of vascular walls to guide interventions. In the future, systems for acquiring multiple views more rapidly by parallel processing and improving endocardial border extraction should allow more routine application of 3-dimensional methods as the next stage in the evolution of cardiac ultrasound, thereby expanding the range of questions that can be answered. Achieving these goals will depend, in large measure, on persistence in developing the necessary technology.  相似文献
