农村医疗设备现状与发展前景

简述了农村医疗设备的现状以及发展的前景，分析了当前存在的问题，综合国家政策，提出了未来农村医疗设备良性发展的几点建议，希望对农村医疗的健康发展有所促进。     

Off-pump versus on-pump coronary artery bypass surgery for the treatment of left main with triple coronary artery disease

Objective:To obtain early results of off-pump coronary artery bypass grafting(OPCAB) in patients with significant left main coro- nary artery(LMCA) and triple vessels stenosis by comparing with those of a similar group undergoing conventional coronary artery bypass surgery(CCAB). Methods:Data for patients with significant LMCA and triple vessels stenosis who underwent CCAB or OPCAB were collected retrospectively between January 1999 and May 2006. Non-randomized, retrospective data analysis included demo- graphic and preoperative risk factors, operative details, clinical outcome and early follow-up. Results: The number of distal anastomo- sis and grafts varied from 3 to 6. The average number per patient was similar in the two groups (OPCAB group:3.76± 0.98, CCAB group:3.81± 1.02). Thirty-day mortality occurred to one patient in the OPCAB group whereas two early deaths were observed in the CCAB group but did not reach statistical significance (P > 0.05). The frequency of atrial fibrillation (AF), IABP usage, mediastinitis, re-operation for bleeding (or tamponade) were similar in the two groups (P > 0.05). Postoperative inotropic requirements, peak CK- MB, ventilation time, blood loss, FFP, RBC transfusion need and the length of ICU-stay were all significantly lower in the OPCAB group compared with CCAB group(P < 0.05).Conclusion: Significant LMCA and triple-vessel stenosis can safely and effectively undergo myocardial revascularization using OPCAB surgery. LMCA should no longer be seen as a contraindication to perform OPCAB grafting.     

自拟英柏汤治疗男性泌尿生殖系感染继发性功能障碍86例

男性泌尿生殖系统感染继发性功能障碍,为本虚标实的兼夹证。肾虚精关不固为发病之本,下焦湿热为致病之标,气滞血瘀为该病缠绵难愈的本质所在,治宜标本同治、扶正祛邪。应宜清热利湿、活血化瘀、补肾为主要治则。宜兼顾标本虚实、消补兼施之法,并注意不妄投苦寒、温热之品,以免损伤肾之阴精阳气。     

川芎嗪注射液治疗慢性肾衰竭

笔者用川芎嗪注射液治疗慢性肾衰竭60例,取得满意疗效,现报道如下. 一般资料 慢性肾功能衰竭患者118例,均符合慢性肾功能衰竭(CRF)诊断标准[1],其中男70例,女48例;年龄17～82岁,平均46.7岁.     

输卵管妊娠后再次妊娠的探讨

对输卵管妊娠后有生育要求的５８例行输卵管切除术，１５例行输卵管开窗术，３０例应用药物保守治疗。结果：１０３例治疗后６７例宫内妊娠，９例再次异位妊娠。     

雌激素对乳腺癌细胞生长及P185蛋白、EGF受体表达的影响

为了研究乳腺癌的发展过程中雌二醇的作用机制及参与因素，我们使用流式细胞仪观察了人乳腺癌细胞株SK-BR-3在雌二醇(E2)作用下的生长特征及P185蛋白、EGF受体的表达与癌细胞生长的关系。研究方法：对人乳腺癌SK-BR-3细胞培养增殖后分别加入10nmol／L雌二醇、7nmol／L Tamoxifen以及不用药对照共三组分别作细胞周期分析，     

RP-HPLC测定葛根芩连片中葛根素的含量

报道RP-HPLC测定葛根芩连片中葛根素含量。采用Shim-Pack CLC-ODS(15cm×6mm)分析柱;北京分析仪器厂ODS保护柱;流动相甲醇-水(23:77);流速1.0ml/min。柱温35℃:检测波长250mm,进样体积10μl;外标法计算含量。理论板数按葛根素峰计算不低于2000;葛根素浓度5～80ug/ml与色谱峰面积呈线性关系,r=0.9999,回收率97.6,RSD 1.8%。     

Human lactoferrin proteolytic activity: analysis of the cleaved region in the IgA protease of Haemophilus influenzae

Human lactoferrin proteolytically cleaves and inactivates two colonization factors of non-typable Haemophilus influenzae, the IgA protease precursor protein (Iga), and Hap, the non-pilus adhesin by which microoganisms adhere to host epithelial cells and form microcolonies. Iga and Hap are homologous proteins that are members of the autotransporter family of secreted proteins expressed by gram-negative bacteria. Studies of Iga cleaved by lactoferrin, reported here, show that proteolysis occurred within the helper region of Iga (Iga(beta)) domain which anchors the autotransporter within the Haemophilus outer membrane. The amino-terminus of the extracted Iga protein was not modified. The location of the proteolytic active site in human lactoferrin is under study. Lactoferrin proteolysis may attenuate pathogenicity of H. influenzae, an important cause of otitis media.     

复方炔雌醚对大鼠肝细胞胞液雌激素受体的影响

口服避孕药所产生的副作用机理—直不甚明了。本研究结果表明,服用复方炔雌醚的大鼠肝胞浆中的雌二醇受体数/mg 蛋白显著高于对照组(P<0.05),而平均解离常数无显著差别(P<0.05),因此可以认为长期给予长效口服避孕药能诱导肝脏中的雌二醇受体的再生和补充,激素一受体复合物改变了肝脏的正常功能,导致某些血浆成份的升高,此即可能为副作用产生的原因之一。     

Study on the effect of compound quinesterol on plasma cortisol concentration in fertile women]

This research reports on the concentration of plasma cortisol and the results of an oral glucose tolerance test in 60 fertile women taking quinestrol for between 4 and 13 years. In these women, the mean plasma cortisol concentration was 27.72 +or- 1.05 mcg/100 ml, which was significantly higher than 19.76 +or- 0.86 mcg/100 mg in 20 cases with a history of taking shortacting oral contraceptives and 15.53 +or- 0.50 mcg/100 ml in the normal volunteers. There was no significant difference in mean blood glucose levels between the group taking quinestrol and the control group. The above results indicated that the effect of quinestrol on adrenal cortisol function may be through the negative feedback of t pituitary-adrenal cortex axis. As a result, the levels of plasma cortisol were increased. Small doses of quinestrol had no apparent effect on carbohydrate metabolism.
