目的:探讨丙戊酸钠联合利培酮对癫痫性精神障碍患者精神功能及认知功能的影响。方法:纳入2018年12月~2020年12月期间某院收治80例癫痫性精神障碍患者为研究样本,以随机对照原则,经数字表法分为观察组和对照组各40例,对照组采用利培酮治疗,观察组采用利培酮+丙戊酸钠治疗,对比两组患者治疗后各症状缓解时间,治疗前后精神功能评价量表(GAF)、简明精神病评定量表(BPRS)、认知功能障碍评估量表(MoCA)、简易智力状态检查量表(MMSE)的评分结果及用药后不良反应发生情况。结果:观察组治疗后知觉、记忆、思维、情感障碍等症状缓解时间均低于对照组,差异具有统计学意义(P<0.05);治疗前两组患者的以上评分无明显差异(P>0.05),治疗后观察组的GAF高于对照组、BPRS低于对照组,MoCA、MMSE高于对照组,差异均具有统计学意义(P<0.05);观察组治疗后不良反应发生率12.50%(5/40)略高于对照组7.50%(3/40),差异无统计学意义(P>0.05)。结论:丙戊酸钠联合利培酮治疗癫痫性精神病对改善患者的精神、认知功能具有积极影响,二者合用不会增加药物不良反应发生风险,此用药方案有效性及安全性均较高。 相似文献

Discoid lupus erythematosus (DLE) is the most common skin manifestation of lupus; however, the molecular mechanisms underlying DLE remain unknown. Therefore, we aimed to identify key differentially expressed genes (DEGs) in discoid lupus skin and investigate their potential pathways.To identify candidate genes involved in the occurrence and development of the disease, we downloaded the microarray datasets and GSE52471 from the Gene Expression Database (GEO). DEGs between discoid lupus skin and normal controls were selected using the GEO2R tool and Venn diagram software ( GSE72535http://bioinformatics.psb.ugent.be/webtools/Venn/). The Database for Annotation, Visualization, and Integrated Discovery (DAVID), Enrichr, and Cytoscape ClueGo were used to analyze the Kyoto Encyclopedia of Gene and Genome pathways and gene ontology. Protein-protein interactions (PPIs) of these DEGs were further assessed using the Search Tool for the Retrieval Interacting Genes version 10.0.Seventy three DEGs were co-expressed in both datasets. DEGs were predominantly upregulated in receptor signaling pathways of the immune response. In the PPI network, 69 upregulated genes were selected. Furthermore, 4 genes (CXCL10, ISG15, IFIH1, and IRF7) were found to be significantly upregulated in the RIG-I-like receptor signaling pathway, from analysis of Enrichr and Cytoscape ClueGo.The results of this study may provide new insights into the potential molecular mechanisms of DLE. Mammary carcinoma (MC) is one of most common malignancy in women, and ring finger protein 2 (RNF2) possesses various roles in vast human tumors. In MC tissues as well as in cell lines RNF2 exhibited high expression, had significant association with tumor size, lymph node status, TNM stage, patients’ poor survival, and promoted cell proliferation, colony formation, cell migration and invasion of MC cell lines which was mediated by downregulation of E-cadherin protein. 评估玻璃体切割联合内界膜剥除对糖尿病性黄斑水肿(DME)的疗效。方法:回顾性病例对照研究。2014年6月至2017年1月间因糖尿病视网膜病变合并玻璃体积血或增殖病变于温州医科大学附属眼视光医院杭州院区行玻璃体切割手术治疗,且术前或术中经光学相干断层扫描(OCT)检查确诊合并DME的患者31例(33眼)纳入研究。16例(18眼)术中联合内界膜剥除作为剥膜组,15例(15眼)仅接受玻璃体切割手术治疗者作为对照组。所有手术均由同一医师主刀完成。术后1、3个月随访时复查OCT,对比观察黄斑中心厚度(CMT)和视力的术后变化情况。随访中CMT和最佳矫正视力(BCVA)比较采用重复测量方差分析,组间CMT和BCVA比较采用独立样本t检验。结果:手术前,手术后1、3个月2组间比较LogMAR视力总体差异有统计学意义(F=15.93,P<0.001)。术后 1个月时剥膜组BCVA高于对照组(t=2.55,P=0.02),但术后3个月时2组间差异无统计学意义(t=0.82, P=0.42)。手术前,手术后1、3个月CMT总体差异无统计学意义(F=2.85,P=0.065)。术后1、3个月时,剥膜组的CMT均低于对照组,2组间差异均有统计学意义(t=2.24,P=0.03;t=3.79,P=0.001)。术后1个月时,剥膜组有效(与术前比CMT减少20%以上)、无效(变化不超过20%)及恶化(增厚超过 20%)的例数分别为8、6、4例,术后3个月时则分别为11、5、2例,与对照组相比,术后1个月时组间差异无统计学意义(Z=-1.687,P=0.092),术后3个月时剥膜组DME改善有效比例明显高于对照组,组间差异有统计学意义(Z=-2.177,P=0.029)。结论:对于非牵拉性DME,内界膜剥除有助于术后早期DME消退。 相似文献

Stellate ganglion (SG) modification has been investigated for arrhythmia treatment. In this study, transesophageal SG imaging and intervention were explored using a homemade 30F integrated focused ultrasonic catheter in healthy mongrel canines in vivo. Anatomic details of SGs were ultrasonically imaged and evaluated. SG had a heterogeneous echoic structure and characteristic profiles sketched by hyper-echoic outlines in an ultrasonogram. Left SGs in the experimental group were successfully ablated through the esophagus under ultrasonic guidance provided by the catheter itself. Two weeks after the ablation, the QT and QTc of the experimental group decreased compared with those of the sham group and at baseline (both p values < 0.001). Histologic examination revealed that left SGs were destroyed. No major complications were observed. This approach may be further explored as a method for ganglia remodeling evaluation and as a strategy of ganglia modification for arrhythmia and for other diseases. 相似文献

目的 探讨分析基于护理专业能力进阶制度的优化层级管理模式在临床的应用效果。方法 选择重庆市大足区二级及以上5家医院共500名护士作为研究对象,随机分为干预组和对照组,每组护士各250名,对照组护士采用传统管理模式,干预组护士采用基于专业能力进阶制度护理人员优化层级管理模式,比较分析两组护士护理能力、临床工作质量和不良事件发生率。采用SPSS 23.0进行卡方检验和t检验。结果 两组护士年龄、性别、婚姻、职称、工作年限等差异没有统计学意义(P>0.05),基线资料保持一致,具有可比性。进行优化层级管理后,干预组护士护理能力得分(82.30±3.83)明显高于对照组(72.78±5.33);干预组护士护理工作质量得分(97.53±9.71)明显高于对照组(90.17±8.18)。对两组医院不良事件发生率进行组间比较,干预组不良事件发生率(0.9%)明显低于对照组(3.9%),差异均有统计学意义(P<0.05)。结论 基于专业能力进阶制度的护理人员优化层级管理模式能够提升护理人员的专业能力和临床护理质量,降低不良事件发生率,为提升护理管理水平提供借鉴和参考。 相似文献

Microglia, the innate immune cells of the CNS, perform critical inflammatory and noninflammatory functions that maintain normal neural function. For example, microglia clear misfolded proteins, elaborate trophic factors, and regulate and terminate toxic inflammation. In Alzheimer’s disease (AD), however, beneficial microglial functions become impaired, accelerating synaptic and neuronal loss. Better understanding of the molecular mechanisms that contribute to microglial dysfunction is an important objective for identifying potential strategies to delay progression to AD. The inflammatory cyclooxygenase/prostaglandin E2 (COX/PGE2) pathway has been implicated in preclinical AD development, both in human epidemiology studies and in transgenic rodent models of AD. Here, we evaluated murine models that recapitulate microglial responses to Aβ peptides and determined that microglia-specific deletion of the gene encoding the PGE2 receptor EP2 restores microglial chemotaxis and Aβ clearance, suppresses toxic inflammation, increases cytoprotective insulin-like growth factor 1 (IGF1) signaling, and prevents synaptic injury and memory deficits. Our findings indicate that EP2 signaling suppresses beneficial microglia functions that falter during AD development and suggest that inhibition of the COX/PGE2/EP2 immune pathway has potential as a strategy to restore healthy microglial function and prevent progression to AD. 相似文献
