Fly ash is widely used in the cement industry to improve the performance and durability of concrete. The future availability of fly ash, however, is a concern, as most countries are inclining towards renewable energy sources as opposed to fossil fuels. Additional concerns have been raised regarding the impact of strict environmental regulations on fly ash quality and variability. This paper, therefore, evaluates if nano calcium carbonate (nano CaCO3) can be used as an alternative to fly ash. This paper presents comprehensive testing results (fresh, hardened, and durability) for OPC (Ordinary Portland Cement) and PLC (Portland Limestone Cement) concretes with 1% nano CaCO3 and compares them to those for concretes with fly ash (both Class F and C). Compared to concretes with fly ash, OPC and PLC with nano CaCO3 presented improved testing results in most cases, including later age strength, permeability, and scaling resistance. As nanotechnology in concrete is a relatively new topic, more research on the efficient use of nanotechnology, such as for proper dispersion of nano CaCO3 in the concrete, has potential to offer increased benefits. Further, nano CaCO3 is environmentally and economically viable, as it has the potential to be produced within the cement plant while utilizing waste CO2 and generating economic revenue to the industry. Thus, nano CaCO3 has the potential to serve as an alternative to fly ash in all beneficial aspects—economic, environmental, and technical.  相似文献

The population of Sikkim is a unique blend of multi-tribal and metropolitan culture. However, till date, no data regarding prevalence of sexually transmitted diseases (henceforth abbreviated as STDs) among this population is available and hence requires attention. Hence the objective is to determine the prevalence of STDs in Sikkim and to describe associated risk factors. A cross-sectional study involving ‘Questionnaire-based anonymous feedback system’ was followed to collect data from 2,000 individuals across the society. The four most common STDs, gonorrhea, syphilis, chlamydia and HIV, were considered for the study. Total 69 (3.6 %) cases of STDs were found in 1,918 individuals was affected by at least one of the STDs, out of which 43 were males and 26 were females. Cases of gonorrhea, syphilis, chlamydiasis and HIV were 25, 22, 4 and 18 respectively. Out of total 69 cases of STDs, 20 individuals were also suffering from some kind of hepatitis. Addictions like alcoholism, smoking and drugs were also found in significant number, with 1,019 (>50 %) individuals with at least one of these addictions. Relative risk analysis indicates that gender-wise females are more vulnerable to STDs than males. The number of partners, addictions, especially alcohol and drug abuse, also contribute to STD cases. STDs act as a significant risk factor in transmitting some of the types of hepatitis. In such cases, females are more vulnerable than males. The results suggest that new community health programs are essential for both, HIV and non-HIV STDs in Sikkim.  相似文献

Taenia solium is a predominant food-borne parasitic zoonosis (FBPZ) in Nepal. Using the PRECEDE framework, as defined by Green and Kreuter, we can identify the factors behind the high incidence of this disease. Armed with this information, we can define the actions necessary to control T. solium. In accordance with the first step of PRECEDE, social assessment, we set the goal of decreasing the potential for T. solium transmission in Nepal by the year 2003. This goal has yet to be endorsed by policy makers. However, an epidemiological assessment based on hospital data alone showed that T. solium is an endemic problem in urban Nepal that must be addressed. Based on behavioral and environmental assessments (Steps 1 and 2 of PRECEDE-PROCEED), we defined the following action objectives to be achieved by 2003: 1) Train meat producers and sellers to detect contaminated pork and avoid selling it, 2) Improve pig husbandry to limit the animals' access to human feces, 3) Construct hygienic model slaughterhouses. These improvements could control the meat-producing environment, thus limiting the potential for cross-carcass contamination and other hygiene deficiencies associated with the spread of T. solium. An educational and ecological assessment shows all predisposing, reinforcing and enabling factors are present in Nepal, consistent with PRECEDE requirements. While T. solium is clearly defined as a health problem according to PRECEDE, there remain significant hurdles to controlling it. These hurdles lie in administration and policy, where standardized law-enforcement and meat inspection practices are needed. Finally, the government of Nepal must assign high priority to T. solium control, as it is a preventable, yet prevalent disease.  相似文献

Cryptosporidium species have been implicated as an important cause of childhood diarrhoea. We determined the prevalence of cryptosporidiosis in HIV seronegative children 15 years of age and below presenting with diarrhoea in the BP Koirala Institute of Health Sciences, Nepal. Faeces were collected over a 12-month period. Coccidian oocysts were detected using modified acid-fast staining. Intestinal parasites were found in 9.15% of diarrhoeal stool. Coccidian parasites were observed in 4.4% (with 4.1% cryptosporidium and two cyclospora). Coccidia were the most recurrent parasite found in this study. The detection was throughout the year with clustering during the rainy season.  相似文献

Developing a strategy for monitoring iodine deficiency disorders (IDD) remains a big challenge in rural Nepal where great variations could exist in IDD status. To explore the possibility of variation in urinary iodine excretion (UIE) level in rural settings, we carried out a detailed study of UIE among 586 school children of 20 schools in five villages. Our data revealed statisitically significant differences in UIE values among rural villages and schools in the same villages. The policy-makers should keep such variations in mind for a successful monitoring of IDD in Nepal and other countries where such variations may exist.  相似文献

The prevalence of metabolic syndrome including central obesity, insulin resistance, impaired glucose tolerance, hypertension, and dyslipidemia is increasing. Development of adequate therapy for metabolic syndrome requires an animal model that mimics the human disease state. Therefore, we have characterized the metabolic, cardiovascular, hepatic, renal, and pancreatic changes in male Wistar rats (8-9 weeks old) fed on a high-carbohydrate, high-fat diet including condensed milk (39.5%), beef tallow (20%), and fructose (17.5%) together with 25% fructose in drinking water; control rats were fed a cornstarch diet. During 16 weeks on this diet, rats showed progressive increases in body weight, energy intake, abdominal fat deposition, and abdominal circumference along with impaired glucose tolerance, dyslipidemia, hyperinsulinemia, and increased plasma leptin and malondialdehyde concentrations. Cardiovascular signs included increased systolic blood pressure and endothelial dysfunction together with inflammation, fibrosis, hypertrophy, increased stiffness, and delayed repolarization in the left ventricle of the heart. The liver showed increased wet weight, fat deposition, inflammation, and fibrosis with increased plasma activity of liver enzymes. The kidneys showed inflammation and fibrosis, whereas the pancreas showed increased islet size. In comparison with other models of diabetes and obesity, this diet-induced model more closely mimics the changes observed in human metabolic syndrome.  相似文献

War and conflict have consequences on the mental health of individuals and entire communities and the communities in Aceh, Indonesia, having experienced more than 30 years of armed conflict, are no exception. This study presents results from an evaluation of a non-specific mental health group counseling program among adults affected by conflict. Interventions such as these need to be evaluated to further the limited empirical evidence base for efficacious community-based treatments for improving the mental health and psychosocial problems in humanitarian settings.  相似文献

Summary: This study was aimed to evaluate the effectiveness of solution form of 17% ethylenediaminetetraacetic acid (EDTA) on removing smear layer of root canals at different exposure time periods and to provide scientific basis for EDTA as a choice of root canal irrigation in clinical practice. Twenty-five single-rooted teeth were randomly divided into 5 groups: control group (group A) was given 2.5% NaOC1, and 4 experimental groups were given 2.5% NaOC1 and 17% EDTA, including groups B, C, D and E with exposure time of 1, 3, 5 and 7 min, respectively. After preparation of the root canals, the teeth were split along their longitudinal axis, and the root sections were examined under scanning elec- tron microscope for evaluation of smear layer removal and erosion on the surface of the root canal walls. The specimens in group B showed presence of smear layer on the walls of the root canal with no statistical difference from that in group A (P〉0.05). In groups C and D, partial removal of smear layer was obtained, and there was no significant difference between the two groups (P〉0.05), but there was significant difference in removal of smear layer between group C and group B (P〈0.05). Root canal walls in group E specimens showed almost complete removal of smear layer, and the removal of smear layer was significantly different from that in group D (P〈0.01). There was no significant change in the structure of the surface of root canal for each sample. It was concluded that combined irrigation with 17% EDTA and 2.5% NaOC1 could remove the smear layer with no significant alteration in dentinal structure when the chelating agent was applied for 7 min. At 3 and 5 min of application, partial removal of smear layer was observed and at 1 min negligible removal of smear layer was achieved.  相似文献

Cerebral venous thrombosis (CVT) is caused by partial or complete occlusion of the major cerebral venous sinuses or the smaller feeding cortical veins which predispose to the risk of venous infarction and hemorrhage. Current guidelines recommend treating CVT with either low-molecular-weight heparin (LMWH) or unfractionated heparin (UFH) followed by an oral vitamin K antagonist (VKA) for 3–12 months. Direct oral anticoagulants (DOACs) have already established benefit over warfarin as a long-term treatment of symptomatic venous thromboembolic disorder like deep vein thrombosis (DVT), and pulmonary embolism (PE) given its equal efficacy and better safety profile. The benefit of DOACs over warfarin as a long-term anticoagulation for CVT has likewise been extensively studied, yet it has not been approved as first-line therapy in the current practice. We therefore performed a systematic review and meta-analysis of relevant studies to generate robust evidence regarding the safety and efficacy of DOACs in CVT. This meta-analysis demonstrates that the use of DOACs in CVT has similar efficacy and safety compared to VKAs with better recanalization rate.  相似文献
