The new magnetically suspended axial pump is free of seals, bearings, mechanical friction and wear. In the absence of a drive shaft or flow meter, pump flow assessment is made with an algorithm based on currents required for impeller rotation and stabilization. The aim of this study is to validate pump performance, algorithm-based flow and effective flow. A series of bovine experiments was realized after equipment with pressure transducers, continuous-cardiac-output-catheter, intracardiac ultrasound (AcuNav) over 6 h. Pump implantation was through a median sternotomy (LV-->VAD-->calibrated transonic-flow-probe-->aorta). A transonic-HT311-flow-probe was fixed onto the outflow cannula for flow comparison. Animals were electively sacrificed and at necropsy systematic pump inspection and renal embolus score was realized. Observation period was 340+/-62.4 min. The axial pump generated a mean arterial pressure of 58.8+/-14.3 mmHg (max 117 mmHg) running at a speed of 6591.3+/-1395.4 rev./min (min 5000/max 8500 rev./min) and generating 2.5+/-1.0 l/min (min 1.4/max 6.0 l/min) of flow. Correlation between the results of the pump flow algorithm and measured pump flow was linear (y=1.0339x, R2=0.9357). VAD explants were free of macroscopic thrombi. Renal embolus score was 0+/-0. The magnetically suspended axial flow pump provides excellent left ventricular support. The pump flow algorithm used is accurate and reliable. Therefore, there is no need for direct flow measurement.  相似文献

Consumption of amfetamine-type stimulants, including classical amfetamines and 'designer drugs', has been recognised as one of the most significant trends in drug abuse at the end of the past century and at the beginning of the current one. The first cause is the increasing consumption amongst youth of methylenedioxy- and methoxy-substituted amfetamines, of which the pharmacology in humans is currently under investigation. Secondly, the abuse of more classical amfetamines, such as amfetamine itself and metamfetamine, continues to be highly prevalent in some geographical regions. Amfetamines are powerful psychostimulants, producing increased alertness, wakefulness, insomnia, energy and self-confidence in association with decreased fatigue and appetite as well as enhanced mood, well-being and euphoria. From a clinical pharmacokinetic perspective, amfetamine-type stimulants are rather homogeneous. Their oral bioavailability is good, with a high distribution volume (4 L/kg) and low binding to plasma proteins (less than 20%). The elimination half-life is 6-12 hours. Both hepatic and renal clearance contribute to their elimination from the body. Hepatic metabolism is extensive in most cases, but a significant percentage of the drug always remains unaltered. Amfetamine and related compounds are weak bases, with a pKa around 9.9, and a relatively low molecular weight. These characteristics allow amfetamine-type stimulants to diffuse easily across cell membranes and lipid layers and to those tissues or biological substrates with a more acidic pH than blood, facilitating their detection in alternative matrices at relatively high concentrations. In most cases, the concentrations found are higher than expected from the Henderson-Hasselbach equation. Drug monitoring in non-conventional biological matrices (e.g. saliva, hair, nails, sweat) has recently gained much attention because of its possible applications in clinical and forensic toxicology. An individual's past history of medication, compliance or drug abuse can be obtained from testing of hair and nails, whereas data on current status of drug use can be provided by analysis of sweat and saliva. Because of the physicochemical properties of amfetamine-type stimulants, this group of drugs is one of the most suitable for drug testing in non-conventional matrices.  相似文献

The use of ultrasound in Crohns disease has a recent history. This method is useful in various situations like: the diagnosis of the disease, the diagnosis of intra-abdominal complications and the follow-up of the operated patient. Moreover, thanks to its practicality of use, ripetibility and accuracy, ultrasounds can represent a first line diagnostic instrument for Crohns disease both in elective and emergency conditions. Liquid chromatography/atmospheric pressure chemical ionization ion trap mass spectrometry (LC/APCI-ITMS) was applied to evaluate the bioavailibility of two different forms (free and complexed with soy phospholipids) of pure bilobalide and ginkgolide B in rats after acute administration. The same technique was used to measure the levels of ginkgolide A, B and bilobalide in plasma of guinea pigs fed Ginkgo biloba extract enriched in terpene lactones after chronic administration. The ratio R(P)/R(A) increased two to four times after the administration in the phytosomic form, where R(P) and R(A) represent the percentage ratio between the concentration of each terpene lactone in plasma and in the administrated form, respectively.  相似文献

We tested the hypothesis that acute hypoxia may alter the circadian pattern of body temperature in adult humans. Six healthy subjects were studied in normoxia, hypoxia (approximately 13% inspired O(2)), and again normoxia, each session lasting >24 h and spaced a few days apart, with a constant routine protocol of sustained wakefulness and minimal activity. Some parameters (e.g., tympanic and abdominal temperatures, heart rate) were recorded continuously; others (e.g., oxygen consumption and pulmonary ventilation) were monitored for approximately 10 min every 2 h. The amplitudes of the circadian oscillation of tympanic, abdominal, and calf skin temperatures were reduced in hypoxia, averaging, respectively, 61%, 80% and 50% of the normoxic amplitude. Oxygen consumption and pulmonary ventilation, which presented a circadian pattern in normoxia, had no longer significant oscillations during hypoxia, whereas the opposite was the case for heart rate and diastolic pressure. Therefore, acute hypoxia can disturb the normal circadian patterns and, specifically, depress those of body temperature. These effects, qualitatively similar to those observed in chronically hypoxic animals and humans, could contribute to sleep disturbances at high altitude.  相似文献

A glutaraldehyde preserved valved bovine jugular xenograft mounted in a nitinol 'Z' stent, expandable from 7 to 28 mm of internal diameter, was evaluated in vitro (column of water developing a pressure of 45 mmHg and a mock loop including a pulsatile pump) and in vivo in five adult pigs with intra-vascular ultrasound to measure the inferior vena cava diameter via a retroperitoneal access. Through a stent-graft delivery system (24 French) the self expandable valved stent was implanted off-bypass in the inferior vena cava, between hepatic veins and cavo-atrial junction, with flow and pressure gradient recording. The mean length of the valved stent was 22.80+/-1.06 mm, the mean internal diameter 20.97+/-0.5 mm and the mean external diameter 26.67+/-0.9 mm. The valve leaking under pressure was 32.5+/-12.3 ml/min. The mean pressure gradient recorded across the self expandable valved stent implanted in the inferior vena cava was 1.0+/-0.5 mmHg (range 0-2 mmHg). Intra-vascular ultrasound showed partial opening and closing of the valve (mean area reduction from 148.5 to 81.5 mm2), with almost complete occlusion only during deep breaths. The in vitro and in vivo experiments confirmed the feasibility of potential application of the self-expandable valved stent implanted off-bypass in the inferior vena cava for late conversion of failing total cavo-pulmonary connection; intra-vascular ultrasound allows for adequate implantation and evaluation.  相似文献

Cardiopulmonary bypass (CPB) remains the key technology for more complex cardiac operations. The perfusion equipment used nowadays has seen tremendous progress since its introduction into clinical practice 50 years ago. However, overall, CPB is still far from perfect. Major haemodilution is not only a problem for red cell-dependent gas transport, but also for the platelet and humeral factor-dependent coagulation, the protein-dependent intravascular oncotic pressure and so forth. Reduction of the priming volume through further miniaturization of CPB equipment is, therefore, the most obvious next step. A systematic approach needs to optimize all CPB components, including pumps, oxygenator/heat exchanger structures, filters, reservoirs, cardiotomy suction, tubings and cannulas. This report provides an update of already commercially available low prime perfusion devices (e.g., the CORx integrated pump-oxygenator) as well as promising prototypes like the smart suction system and the smartcanula.  相似文献

The aim of this review was to identify the imaging methods at our disposal to optimally manage posterior uveitis at the present time. The focus was put on methods that have become available since the 1990s, some 30 years after fluorescein angiography had revolutionised imaging of posterior uveitis in particular imaging of the retinal vascular structures in the 1960s. We have focussed our review on precise imaging methods that have been standardised and validated and can be used universally thanks to commercially produced and available instruments for the diagnosis and follow-up of posterior uveitis.
