Synergistic Effect of Febantel and Pyrantel Embonate in Elimination of Giardia in a Gerbil Model

The objectives of the study were to determine the optimal dose of febantel, pyrantel embonate and a combination of febantel/pyrantel embonate required to effectively treat Giardia in a gerbil model and to determine if there is a synergistic effect with the two drugs. SPF gerbils were infected by oral inoculation with 105 Giardia duodenalis trophozoites (day 0). On days 5 to 7, animals (n = 6) were treated once daily via oral gavage with febantel, pyrantel embonate, febantel and pyrantel embonate, metronidazole or placebo. Gerbils were euthanised 24 hours after last treatment and duodenal trophozoites were enumerated on a haemocytometer to obtain a concentration of trophozoites/ cm of gut. Febantel alone, effectively eliminated Giardia trophozoites at 160 and 80 mg/kg. Pyrantel embonate did not eliminate Giardia from the animals but significantly reduced parasite counts at all dosages. Febantel combined with pyrantel embonate effectively eliminated Giardia trophozoites at 160, 80 and 40 mg/kg. Metronidazole did not eliminate Giardia trophozoites from the gut. All placebo-treated animals were heavily infected with Giardia trophozoites. It can be concluded that febantel is more effective in elimination of Giardia infections when combined with pyrantel embonate compared to the agents used alone.     

Influence of isostatic compression on the stability of vancomycin loaded with a calcium phosphate-implantable drug delivery device

It is essential to prevent microbial infections after osteoarticular trauma or prosthesis implantation. As an alternative to antibiotic parenteral administration, antibiotic-loaded biomaterials allow high concentrations to be obtained in situ without systemic toxicity. Although the formulation of biphasic calcium-phosphate (BCP)-vancomycin granules by isostatic compression has recently been used to produce drug-delivery devices, the stability of vancomycin needs to be proven. In this study, vancomycin was associated with BCP powders by isostatic compression at 100, 140, or 200 MPa and then extracted or released by a rotating paddle system for 24 h. Vancomycin assays were performed by spectrophotometric and microbiological methods. The results show that all vancomycin associated with the material was recovered after extraction without degradation. Thus, vancomycin was not denaturated after application of 100, 140, or 200 MPa of isostatic compression. The results for vancomycin released from granules compressed at the three pressures were not significantly different (p =.01) whether assays were performed microbiologically or spectrophotometrically, indicating a good correlation between the two methods. This process involving high pressure appears to be a good means of developing drug delivery devices loaded with therapeutic agents without denaturating the components.     

Management of early endometrial carcinoma

Management of early endometrial carcinoma is controversial in regard to timing and indication of adjunctive radiation therapy. Two hundred eighty patients with stage I carcinoma of the endometrium are analyzed: 135 patients were treated with surgery only and 61 patients underwent preoperative and 83 patients postoperative radiation therapy. The overall survival was 94%. Recurrence rates in all three treatment arms were equal. Tumor grade was found to change from the diagnostic D&C specimen to the definite surgical specimen in 31% of all cases and in 50% of all grade 3 lesions. As only 39% of all patients required postoperative radiation therapy with equal survival, a primary surgical approach spares the majority of patient unnecessary treatment and preserves prognostically important histology.     

Tympanometric and acoustic-reflex studies in neonates

Tympanograms and acoustic reflexes for a broadband noise and for a 1000-Hz tone were measured in normal neonates. Notched tympanograms were typical of neonatal ears for a 220-Hz probe tone. A single-peaked tympanogram was most characteristic for a probe frequency of 660 Hz. Ipsilateral and contralateral acoustic reflexes were present more frequently for a 660-Hz probe tone compared to a 220-Hz probe tone, but acoustic-reflex thresholds were not significantly different between probe tones. As with adults, acoustic-reflex thresholds for the noise were significantly lower than for the tone, and ipsilateral reflex thresholds were lower than contralateral reflex thresholds. Reliability of acoustic-reflex and tympanometric measures was high. Age change from 2 to 4 days had no significant effect on tympanometric or acoustic-reflex characteristics. There was no apparent relation between tympanometric pattern and acoustic-reflex characteristics.     

Unilateral mid-femoral periosteal newbone of varying aetiology in children

Thigh pain is a serious condition in childhood and should be promptly and thoroughly investigated. Twenty-five cases of thigh pain lasting between 1 day and 7 months with mid-femoral periosteal thickening were analysed. There were 10 patients with Ewing's sarcoma, 7 with eosinophilic granuloma, 1 with osteoid osteoma, 1 with leukaemia, 3 with osteomyelitis, 1 with myositis and 2 with post-traumatic periosteal reaction. It was shown that patients with Ewing's sarcoma and eosinophilic granuloma have certain patterns of X-ray changes of high diagnostic value, which allow a proper diagnosis in most cases.     

Dose reduction in paediatric radiology using rare earth filtration

Rare earth filters are known to reduce radiation dose in radiological investigations. The present investigation used Er and Sm filters that were selected because of their chemical stability and ability to withstand prolonged exposure to the atmosphere. They were used in a comparison with a conventional aluminium filter in a range of paediatric radiological procedures. Introduction of the filters instead of the conventional filter, resulted in a dose reduction of up to twofold with no discernible deterioration in image quality.
