目的探讨电影MR测定心功能的价值。材料和方法；通过对21例健康成人电影MR成像，采用双盲法，分析和测量左心功能指标，并与超声心动图（UCG）测值进行相关性t检验。结果：电影MR测定左室短径、左室后壁／室间隔厚度、左室舒末／缩末容积等指标，与UCG比较，具有良好的相关性（r＝0．87－0．99）。结论：电影MR是又一检测心功能准确、无创的影像新技术，具有较大发展潜力。     

Subconjunctival Injection of Regulatory T Cells Potentiates Corneal Healing Via Orchestrating Inflammation and Tissue Repair After Acute Alkali Burn

PurposeThis study aimed to investigate the therapeutic effects and underlying mechanisms of locally delivered regulatory T cells (Tregs) on acute corneal wound healing after alkali burn.MethodsAfter corneal alkali burn, the mice were injected subconjunctivally with regulatory T cells (Tregs) isolated from syngeneic mice. The wound healing process was monitored by clinical manifestation, flow cytometry, and enzyme-linked immunosorbent assay (ELISA). As amphiregulin (Areg) was significantly upregulated, its reparative function in injured corneas was suggested. The hypothesis was further verified via loss- and gain-of-function experiments by administrating the antibody of Areg (anti-Areg) and recombinant Areg (rmAreg).ResultsSubconjunctivally injected Tregs rapidly migrated to injured corneas. The mice treated with Tregs showed prominently reduced corneal opacity, alleviated edema, and faster re-epithelialization compared with the control group. Mechanistically, Treg treatment led to suppressed infiltration of inflammatory cells, along with improved proliferation and inhibited apoptosis of corneal epithelial cells. Tregs expressed upregulated functional markers, including Areg. Expectantly, the levels of Areg in corneas were dramatically higher in the Treg injection group, in line with better corneal restoration. Additional experiments showed that the administration of anti-Areg blunted the reparative effect of Tregs, while exogenous Areg enhanced it. Treg-treated corneas also exhibited less neovascularization and fibrosis at a later reconstruction stage of corneal repair.ConclusionsThe findings showed that the subconjunctival injection of Tregs effectively promoted corneal wound healing by inhibiting excessive inflammation and enhancing epithelial regeneration, with an indispensable reparative role of Areg. Subsequent complications of corneal vascularization and fibrosis were therefore reduced.     

Long-Term toxicity studies in Canine of E10A, an adenoviral vector for human endostatin gene

E10A, a recombinant adenovirus type 5 vector carrying the human endostatin gene, may be a promising gene therapy drug in the treatment of solid tumors by antiangiogenesis, but a preclinical safety evaluation of E10A has not yet been performed. With high and low doses equivalent to 30 and 7.5 times the human curative dose, respectively, intramuscular injections of E10A were given once daily, 6 days/week, for 3 months, followed by a 1-month recovery period. As of 4 months, all experimental animals appeared generally healthy: normal behavior and eating habits, no nausea, vomiting, or salivation, no abnormal changes in urination or defecation, and increased body weight with the time of experiment. Urinalysis, hemogram, blood biochemistry, electrocardiogram, macroscopic and microscopic studies of organs and tissues were done before treatment, at month 3 of treatment, and 1 month posttreatment. At all time points, no significant abnormal toxic effects were noted. Preliminary investigation of E10A immunotoxicity in dogs indicated that anti-adenoviral antibodies were generated, in a dose- and time-independent manner, after E10A injection. Our data demonstrated that, long term, high-dose intramuscular administration of recombinant human endostatin-carrying adenovirus (E10A) was not notably toxic and might be safe for clinical therapeutic use, although additional long-term toxicity studies by other administration routes are still necessary.

SICU病人院内感染的现况调查

本文自1994年12月至1996年4月间连续收集手术后转入SICU监护 10 d以上,有明确院内感染证据的危重病人共14例,结果提示SICU病人院内感染的特点之一是多部位感染,具体感染部位数与手术方式及采用侵人性诊疗操作密切相关。细菌学研究发现当今主要感染致病菌 G~ 球菌已上升至 35.7％,多重耐药菌株的出现和二重感染的发现预示合理而有效联合应用抗生素的重要性。     

早期康复对脑卒中患者功能活动能力的影响

采用FIM量表对68例急性脑卒中患者功能活动能力进行临床对照研究。两组患者在病后3月时的功能活动能力均有不同程度的改善，但康复组优于对照组（P＜0．001），在恢复独立功能活动者中，康复组16例（47．06％），对照组4例（11．76％）。研究结果表明，早期康复干预有助于脑卒中患者独立功能活动能力的恢复；患者在病后1月内，FIM评定为72．25±20．96分考，经早期康复治疗有可能达到功能活动独立。     

中医药诊治新冠肺炎的思考

新冠肺炎属于疫病,疠气从口鼻而入,与六淫邪气相合(以湿邪为主),可化寒、化热、化毒、化燥,主要伤及肺脾功能。病机认识上不同学者有寒湿与湿热的差异,与受邪轻重、气候差异、患者体质等有关。分期辨证可有效整合不同辨证方法,初期可从辨口气、辨鼻气、辨鼻涕、辨面色、辨小便来区分寒热。中期热盛于湿易从阳化燥、化毒,或湿胜于热从阴化寒而出现多种病变,临床根据寒、湿、热(毒)病邪的多少而随证治疗。中医诊治新冠肺炎既要突出中医理论特色,也要积极彰显中西医结合的优势。
