COVID-19 in pediatric kidney transplantation: The Improving Renal Outcomes Collaborative

There are limited data on the impact of COVID-19 in children with a kidney transplant (KT). We conducted a prospective cohort study through the Improving Renal Outcomes Collaborative (IROC) to collect clinical outcome data about COVID-19 in pediatric KT patients. Twenty-two IROC centers that care for 2732 patients submitted testing and outcomes data for 281 patients tested for SARS-CoV-2 by PCR. Testing indications included symptoms and/or potential exposures to COVID-19 (N = 134, 47.7%) and/or testing per hospital policy (N = 154, 54.8%). Overall, 24 (8.5%) patients tested positive, of which 15 (63%) were symptomatic. Of the COVID-19-positive patients, 16 were managed as outpatients, six received non-ICU inpatient care and two were admitted to the ICU. There were no episodes of respiratory failure, allograft loss, or death associated with COVID-19. To estimate incidence, subanalysis was performed for 13 centers that care for 1686 patients that submitted all negative and positive COVID-19 results. Of the 229 tested patients at these 13 centers, 10 (5 asymptomatic) patients tested positive, yielding an overall incidence of 0.6% and an incidence among tested patients of 4.4%. Pediatric KT patients in the United States had a low estimated incidence of COVID-19 disease and excellent short-term outcomes.

Online health communities,contributions to caregivers and resilience of older adults

The aim of this paper is twofold. Firstly, to investigate the potential benefits of online health communities (OHCs) for informal caregivers by conducting a systematic literature review. Secondly, to identify the relationship between the potential benefits of OHCs and resilience factors of older adults. Performing a thematic analysis, we identified the potential benefits of OHCs for informal caregivers of older adults, including two salient themes: (a) caregivers sharing and receiving social support and (b) self and moral empowerment of caregivers. Then, we uncovered how these potential benefits can support resilience of older adults. Our findings show that sharing and receiving of social support by informal caregivers, and self and moral empowerment of informal caregivers in OHCs, can support four resilience factors among older adults, including self-care, independence, altruism and external connections. This review enables a better understanding of OHCs and Gerontology, and our outcomes also challenge the way healthcare and aged-care service providers view caregivers and older adults. Furthermore, the identified gap and opportunities would provide avenues for further research in OHCs.     

New syndrome: Mother and son with hypertelorism,downslanting palpebral fissures,malar hypoplasia,and apparently low-set ears associated with joint and scrotal anomalies

We report on a mother and son with a similar syndrome of hypertelorism and telecanthus, epicanthal folds, downslanting palpebral fissures, ptosis, broad nasal bridge, malar hypoplasia, thin upper lip, smooth philtrum, and apparently low-set prominent ears. The son also has a hypoplastic shawl scrotum, cryptorchidism, and genu valgum. His language development was delayed at 18 months, but subsequently improved and was normal at age 3. The mother has the additional findings of marked cubitus valgus, hyperextensible joints, dull normal intelligence and a bleeding diathesis. This pattern of multiple congenital anomalies may represent a new syndrome.

This article addresses this unusual employee-manager problem by first describing the characteristics and dynamics of the behavior and then providing guidelines for responding to this problematic behavior.     

Instructional control of female sexual responding

To examine whether women can suppress genital arousal under instructional control, 25 women were presented with three erotic films and instructions to become as aroused as possible (Arouse) or to suppress arousal (Suppress). Genital responding was assessed using the heated oxygen electrode which yields two measures of vaginal responding: oxygen perfusion and thermistor assessment of vasocongestion. 36 to 40% of the sample was able to demonstrate a substantial reduction in sexual responding under Suppress instructions (depending upon the measure used), based on externally derived criteria to determine suppression. Differences were noted in the patterns of responding between suppressors and nonsuppressors, on both the pO₂ and heat measures. Examination of thought-listing data indicated that subjects were able to identify cognitive strategies they used to suppress responding. In particular, successful suppressors were more likely to report the use of 2+ cognitive strategies, whereas the nonsuppressors reported reliance on a single strategy. Results are discussed in light of current theories of emotional factors in sexual responding, with emphasis on clinical implications. Taking the dread out of annual performance evaluations

Many employees and managers alike dread annual performance evaluations. However, there is a way to get past the "dread" factor. Annual performance evaluations can be a positive experience and even motivational. They can be a time for affirming good work of the past year and planning together what will be accomplished in the year to follow. By keeping employees informed of the larger picture, helping them to see how they fit in, involving them in setting performance objectives, dealing with performance problems as they arise, and noticing when they have achieved their objectives can make evaluation a positive experience.
