Complex dislocation of the first metatarsophalangeal joint

Dislocation of the first metatarsophalangeal(MTP) joint is uncommon.1-5 Jahss1has classified these dislocations based on the dislocation of hallux with or without disruption of the sesamoid mass. In this study, we described a case of type Ⅰ complex dislocation of the MTP joint. It was irre-ducible by closed manipulation and required open re-duction achieved through a dorsal approach.     

The prospects for using extracorporeal detoxication methods in disaster medicine]

A lot of specific injuries caused by catastrophes provoke endogenic intoxication. In this situation the application of extracorporal detoxication methods will be conditioned by pathogenetical mechanisms of their action. There are indications for ED management in mechanical injuries, prolonged squeezing effects, thermal injuries, acute poisoning, irradiation injuries, acute infectious diseases, acute reactivity state. The experience of 163 operations on 111 wounded in catastrophes proves the worth-while character of work in this sphere. 10 variants of extracorporal operations including combined methods were applied.     

Constipation and reversible urinary tract abnormalities

Urinary tract anomalies were prospectively investigated with ultrasound in 29 children with functional constipation. These children were compared before and after treatment with 451 age matched healthy controls without constipation. The bladder residue and upper renal tract dilatation after micturition were significantly increased in the group with constipation and improved after treatment.     

Major trends in the treatment of patients with septic shock

The authors formulate main tasks to be solved which will result in successful treatment of septic shock in the surgical clinic. Choice of the methods affecting the septic focus is determined including measures from the rational systemic antibacterial therapy to interventions on the focus with its ablation or draining. The infusive and medicamentous programs are formulated, debatable questions in the treatment of such patients are discussed on the basis of the personal experience: the application of vasoactive drugs, corticosteroids, heparin.
