Cryoinjury with compartment syndrome of bilateral hands secondary to Freon gas: a case report and review of current literature

Freon is an industrial refrigerant that can result in serious cryoinjury of the skin. Although there had been a few reported cases, the optimal management is not clear. We present the first case report of a patient with freon-induced cryoinjury of bilateral hands, complicated with compartment syndrome and treated with a combination of surgical decompression, negative-pressure wound therapy (NPWT), and hyperbaric oxygen therapy (HBOT). On follow-up, there was a complete healing without tissue loss or significant atrophy of the intrinsic muscles of the hand. Therefore, we opined that HBOT and NPWT may work synergistically to prevent progressive tissue damage and improve the long-term outcome, especially when vascularity is compromised in severe cryoinjuries of the hand.Level of Evidence: Level V, therapeutic study.

Splenic dermoids are rare, with few published case reports and no ultrasound images in the English literature. We report the case of a 57‐year‐old woman with that diagnosis and illustrate it with ultrasound, CT, and pathology images. Can you have dementia with an MMSE score of 30?

OBJECTIVE: To investigate the possibility that a patient with a diagnosis oF probable Alzheimer's disease (AD) can still obtain a score oF 30/30 on the Mini-Mental State Exam (MMSE). DESIGN: Chart review. Setting. The McGill University/Jewish General Hospital Memory Clinic. PARTICIPANTS: Participants were selected From the Memory Clinic's patient database. All underwent comprehensive evaluations, including relevant blood work and a computed tomographic scan or a magnetic resonance imaging scan oF the brain to rule out other causes oF dementia. MEASUREMENTS: All patients had one or more neuropsychological evaluation. Data oF all psychometric testing, including the MMSE, were gathered From these visits. Results. Eight patients were Found to meet the criteria oF AD although achieving a score oF 30/30 on the MMSE. Four oF 8 patients achieved this score although they were taking cholinesterase inhibitors. CONCLUSION: Although rare, it is possible to achieve a score oF 30/30 on the MMSE even iF a subject is suFFering From a dementing illness.     

Hypothermic responses to 8-OH-DPAT in the Ts65Dn mouse model of Down syndrome

Recently, we have demonstrated that potassium channels containing G-protein-activated potassium channel 2 (GIRK2) subunits play a significant role in hypothermia induced by several neurotransmitter receptor agonists, including the serotonin (5-HT)1A/5-HT7 receptor agonist 8-OH-DPAT [R-(+)-8-hydroxy-2-(di-n-propylamino) tetralin]. The GIRK2 gene is located in human chromosome 21 (its mouse ortholog, Girk2, is in mouse chromosome 16). Down syndrome is produced by the trisomy of chromosome 21. Here, we used quantitative radiotelemetry to investigatehypothermic responses to 8-OH-DPAT in the Down syndrome mouse model Ts65Dn (which carries an extra chromosomal 16 segment containing Girk2). Our results indicate that, in relation to euploid controls, Ts65Dn mice display significantly increased hypothermic responses to 8-OH-DPAT. This finding may be relevant to the understanding of previously reported differences in serotoninergic neurotransmission in persons with Down syndrome.     

Does Presentation Order Impact Choice After Delay?

Options are often presented incidentally in a sequence, but does serial position impact choice after delay, and if so, how? We address this question in a consequential real‐world choice domain. Using 25 years of citation data, and a unique identification strategy, we examine the relationship between article order (i.e., position in a journal issue) and citation count. Results indicate that mere serial position affects the prominence that research achieves: Earlier‐listed articles receive more citations. Furthermore, our identification strategy allows us to cast doubt on alternative explanations (i.e., editorial placement) and instead indicate that the effect is driven by psychological processes of attention and memory. These findings deepen the understanding of how presentation order impacts choice, suggest that subtle presentation factors can bias an important scientific metric, and shed light on how psychological processes shape collective outcomes.
