We report a sporadic case of Lenz-Majewski syndrome (LMS) with newly recognized manifestations including facial palsy, cleft palate and hydrocephalus developing later in infancy. The clinical course of the patient and neuroimaging studies are described. Increased intracranial pressure was recognized and treated early with the aim of preventing neurological morbidity.  相似文献

Cross-sectional surveys of parasitic infection were performed using the agar plate culture technique (APCT) and modified formalin-ethyl acetate concentration technique (MFECT) to assess the true prevalence of Strongyloides stercoralis relative to other parasites in north-east Thailand. The enzyme-linked immunosorbent assay (ELISA) for diagnosis of S. stercoralis infection was used to estimate the seroprevalence for comparison with coproprevalence. Faecal and serum samples were collected from study participants during October-November 2000. Within the sample population of 332 rural northeast Thais from 3 communities, S. stercoralis was the most common parasitic infection (average 28.9%, range 27.7-30.3%) as determined by APCT; by MFECT the average was 5.4% (range 1.8-8.6%). Other intestinal parasites by order of prevalence were Opisthorchis viverrini (average 14.2%, range 8.6-19.4%), hookworm (average 12.3%, range 4-20.2%), Echinostoma sp. (7.5%), Giardia intestinalis (0.9%), Trichuris trichiura (0.6%), and Taenia sp., Hymenolepis nana and Entamoeba coli (all 0.3%). In an analysis of a subset of the sample population for which serum samples were available (n = 120), coproprevalence by APCT was 33.3% (range 27-53.8%) and seroprevalence was 47.5% (range 29.7-57.9%) by modified unit-based ELISA and 34.2% (range 21.6-42.1%) by conventional optical density (OD)-based ELISA. Taking APCT as the reference method for diagnosis of strongyloidiasis, the sensitivity and specificity of the OD-based ELISA were 65% and 81.3%, respectively, and of the unit-based ELISA were 77.5% and 71.3%, respectively. Our results indicate that S. stercoralis is the predominant parasite in rural north-east Thailand, and that APCT and ELISA should be used as complementary diagnostic methods for community-based parasite surveys, at least among those in high-risk groups.  相似文献

Rabies is inevitably fatal and presents a horrifying clinical picture. Human rabies can manifest in either encephalitic (furious) or paralytic (dumb) forms. The brainstem is preferentially involved in both clinical forms, though there are no clinical signs of brainstem dysfunction. Differences in tropism at the inoculation site or the CNS, in the route of spread, or in the triggering of immune cascades in the brainstem may account for clinical variation. Rabies still poses diagnostic problems, particularly the paralytic form, which closely resembles Guillain-Barré syndrome, or when a patient is comatose and cardinal signs may be lacking. Molecular methods allow reliable detection of rabies-virus RNA in biological fluids or tissue before death. Deviations from the recommendations on prophylaxis of the World Health Organization lead to unnecessary loss of life. To date, attempts to treat human rabies have been unsuccessful.  相似文献

We investigated the effect of supplementing post-wash asthenozoospermic spermatozoa with coenzyme Q10 (CoQ10) in vitro, which may reduce oxidative stress and improve sperm motility. Semen samples were collected from 39 men with asthenozoospermia, and their spermatozoa were isolated by two-layer Percoll density-gradient centrifugation. Kinetic parameters of the isolated spermatozoa (baseline before intervention) were determined immediately by computer-aided semen analysis. Total anti-oxidant capacity and protein carbonyl levels, as markers of oxidative stress, were also measured in the baseline spermatozoa. The baseline spermatozoa suspension was divided equally into two portions, one for CoQ10 supplementation (50 µg/ml for 1 hr) and the other as an un-supplemented vehicle control. The total motility of the CoQ10-supplemented spermatozoa was significantly higher than in the control (p = .009) and progressive motility tended to be higher (p = .053). Immotile sperm concentration in the CoQ10-supplemented spermatozoa was significantly lower than in both the baseline (p = .026) and control (p = .009). Total anti-oxidant capacity and protein carbonyl levels between the baseline, CoQ10-supplemented and control spermatozoa were not significantly different. Our data suggest that CoQ10 treatment reactivated sperm motility. We propose short-term supplementation of post-wash asthenozoospermic spermatozoa with CoQ10 before intrauterine insemination.  相似文献

Opisthorchis viverrini is considered as a carcinogenic parasite which is responsible for cholangiocarcinoma in Southeast Asia. Effective treatment and control of the parasite to reduce the risk of cancer requires efficient diagnostic methods. Because of the limitations involved in human studies, the present work is aimed at comparing diagnostic performance of copro-DNA detection by PCR and fecal examination by formalin-ethyl acetate concentration technique (FECT) during the course of O. viverrini infection and postcurative chemotherapy in experimentally infected hamsters. A manual method of DNA preparation from fecal specimens previously established in human studies was used in PCR analysis. Following infection with varying doses of metacercariae (5, 10, 25, and 50 cysts/animal), PCR analyses were positive as early as 3 weeks post-infection while FECT were negative. PCR tests were comparable to FECT regardless of intensity of infection beginning from 4 to 12 weeks post infection. In chemotherapeutic experiments, with reference to the presence of worm in liver, treatment failures were detected by PCR but not FECT in a group of hamsters infected with 10 cysts/animal. PCR and FECT both detected residual infection at 1 and 2 weeks post-treatment in the group of animals infected with five cysts per animal. High concordant results between diagnoses by PCR, FECT, and worm burden indicated that PCR is suitable for an early diagnosis, evaluation of drug efficacy, and also re-infection post-treatment.  相似文献

The carcinogenic liver fluke, Opisthorchis viverrini, requires Bithynia snail intermediate hosts in its life cycle. However, the prevalence of O. viverrini in snail intermediate hosts is typically low (< 1%). Here, we examined B. siamensis goniomphalos from 48 localities in Thailand and The Lao People's Democratic Republic (Lao PDR) and reported high-prevalence levels of O. viverrini. The highest-prevalence levels per locality were 6.93% (mean = 3.04%) in Thailand and 8.37% (mean = 2.01%) in Lao PDR; 4 of 13 localities examined showed prevalence higher than any prevalence previously recorded. The number of cercariae infecting snails and their prevalence were positively correlated with the size of the snails. High prevalence occurred in the Songkram River wetland (Thailand) and the Nam Ngum River wetland (Lao PDR). Our results show that transmission of O. viverrini from humans as well as animal reservoir hosts to snail intermediate hosts is ongoing and potentially increasing in endemic areas across Thailand and Lao PDR.  相似文献

This study was designed to examine the vasorelaxant effects of hexahydrocurcumin (HHC), one of the major natural metabolites of curcumin from Curcuma longa, on rat isolated aortic rings, and the underlying mechanisms. Isometric tension of the aortic rings was recorded using organ bath system. HHC (1 nM to 1 mM) relaxed the endothelium‐intact aortic rings pre‐contracted with PE and KCl in a concentration‐dependent manner. Removal of the endothelium did not alter the effect of HHC‐induced relaxation. In Ca2+‐free Krebs solution, HHC significantly inhibited the CaCl2‐induced contraction in high K+ depolarized rings and suppressed the transient contraction induced by PE and caffeine in a concentration‐dependent manner. HHC was also observed to relax phobal‐12‐myristate‐13‐acetate (PMA), an activator of protein kinase C (PKC), precontracted aortic rings in a concentration‐dependent manner with EC50 values equivalent to 93.36 ± 1.03 μM. In addition, pre‐incubation with propranolol (a β‐adrenergic receptor blocker) significantly attenuated the HHC‐induced vasorelaxation. These results suggest that the vasorelaxant effect of HHC is mediated by the endothelium‐independent pathway, probably because of the inhibition of extracellular Ca2+ influx through voltage‐operated Ca2+ channels and receptor‐operated Ca2+ channels, the inhibition of Ca2+mobilization from intracellular stores, as well as inhibition of PKC‐mediated Ca2+‐independent contraction.
