Intraportal allogeneic transplantation of the islets of Langerhans cultures in the complex treatment of surgical patients with diabetes mellitus

The article presents results of treatment of 21 patients with a surgical pathology and diabetes mellitus who had intraportal allotransplantation of cultures of islet cells of the pancreas and 316 patients with the similar surgical pathology without such transplantation. It was found that the intraportal introduction of islet cell cultures into the liver can result in an early and considerable drop of the injected insulin dose which is very important in the postoperative period. This method allowed to make the period of treatment at the hospital approximately 15 days shorter.     

Value of the quantitative analysis of the number of natural killer cells of peripheral blood in surgical patients with suppurative-inflammatory processes

The amount of natural killer cells (NKC) was found to lessen in 87 patients with pyo-inflammatory processes. The amount of NKC became considerably greater after liquidation of the purulent inflammation focus and improvement of the patient's state. The dynamics of the course of the inflammatory process and effectiveness of treatment can be followed by results of determination of NKC.     

Nutrition and aging: nutritional status of "apparently healthy" elderly (Dutch nutrition surveillance system)

In a nationwide survey the nutritional status was assessed of 539 apparently healthy, independently living elderly aged 65-79 years. Anthropometric data showed no energy deficits. The prevalence of anemia was 4 and 1% among men and women, respectively. Many elderly showed a low level of 25-hydroxyvitamin D in plasma (less than 31 nmol/L: men 35%; women 43%), indicating a marginal status. Although the prevalence of low blood levels of folate, pyridoxal-5'-phosphate, and total carotenoids was higher among the elderly than among younger adults, clear (clinical) signs of nutritional deficiencies were not observed. Prevalence of obesity (13%), hypercholesterolemia (38%), and hypertension (63%) was found to be high, the percentages being higher for women than for men. Several indicators of the nutritional status appeared to differ among age groups. It is concluded that few differences can be considered as being due to physiological aging, which finding should be reflected in reference values for elderly people.     

Synthesis of N-(phosphonoacetyl)-dipeptide derivatives and evaluation of their antihypertensive activity

A series of N-(phosphonoacetyl)-dipeptide derivatives was synthesized for pharmacological testing as antihypertensive compounds. Several of these compounds demonstrated a moderate antihypertensive effect in Wistar spontaneous hypertensive rats (SHR) with p.o. dosing. ACE inhibition by the compounds was studied using ACE from rat plasma and lung. Inhibitors containing esterified C-termini are pro-drugs and showed activity only for plasma ACE.     

闭胸心肺复苏时主动脉内带水囊导管持续阻塞降主动脉的血流动力学效应

为了提高闭胸心肺复苏时心肌冠状循环灌注压(CPP),我们分别观察了用主动脉内带水囊导管周期性阻塞胸主动脉或腹主动脉(OTA/OAA),以及不同剂量肾上腺素(0.02～0.14mg/kg)对CPP的影响。结果:OTA不能提高CPP。然而,在复苏30min内,每3minOAA之后,CPP都比各期阻塞前血压高0.67～1.2kPa(P<0.05或0.01);解除阻塞3 min后,CPP比各期解除前血压低0.8～1.6kPa(P<0.05或0.01)。另外,使用肾卜腺素只在复苏的前9min有效。结论:OAA可以显著提高闭胸心肺复苏时的CPP;在复苏后期,其作用优于大剂量肾上腺素。     

Comparative evaluation of invasive methods of examination of pulmonary coin lesions

The results of examination of 575 solitary spherical structures in the lungs are analysed. Comparative evaluation of various methods of examination showed that transthoracic aspiration biopsy conducted under the control of computered tomography by means of two thin needles has advantages over radiological examination, cytological examination of sputum, and bronchofibroscopic biopsy. The authors developed a diagnostic algorithm used in a patient with a solitary spherical structure in the lung.     

Renal syndrome in amebic lesions of the liver

The author observed 315 patients with the amebic liver abscess. Of them in 134 (42.5%), the renal syndrome was diagnosed. The kidney damage had the course of the secondary pyelonephritis type, or in the form of a toxic kidney. On the basis of clinico-laboratory analysis, the author distinguishes the mild, moderate and severe degrees of kidney damage, recommends an adequate transfusion-infusion, antibacterial and specific therapy.
