PURPOSE: Positionally induced cyclotorsion could be an important factor in the correction of astigmatism during refractive surgery. We analyzed the change in cyclotorsional rotation during excimer laser ablation in LASIK surgery using the NIDEK Torsion Error Detector (TED). METHODS: One hundred ten patients (192 eyes) who underwent LASIK for myopic astigmatism, using the NIDEK Advanced Vision Excimer Laser (NAVEX) were measured for cyclotorsion during surgery using the TED system. The manifest refraction of these patients was -6.80+/-2.74 diopters (D) (range: -1.00 to -13.75 D). The iris pattern of the patient's eyes in the supine position was recorded via a CCD camera in the EC5000CXII excimer laser system, and it was compared to the iris pattern acquired during OPD-Scan measurement in the sitting position. RESULTS: During laser ablation, the degree of cyclotorsional rotation detected by TED was 1.33+/-1.88 degrees (range: -6.33 to 2.99 degrees) clockwise and 1.00+/-1.79 degrees (range: -3.70 to 7.34 degrees) counterclockwise. The absolute degree of torsion error detected by the TED system was 2.33+/-1.16 degrees (range: 0 to 6.21 degrees). CONCLUSIONS: The effectiveness of the cylinder treatment can be reduced due to torsion errors. The degree of cyclotorsion constantly changes during laser ablation. Therefore, a monitoring system should be developed for the measurement of torsion error, and this will enable the maximum possible correction of the error during laser ablation.  相似文献

We describe the case of a patient with anticentriole antibody-positive scleroderma spectrum disorder (SSD) who developed pulmonary hypertension. A 54-year-old woman had noticed Raynauds phenomenon and digital ulcers during the winter for the past 10 years. Although sclerodactyly was not present, digital ulcers, swelling of her hands, and phalangeal contracture were observed. An indirect immunofluorescence test revealed anticentriole antibody. Other SSc-specific antoantibodies were negative. An echocardiogram demonstrated that the estimated right ventricular systolic pressure was increased to 51 mmHg. She was diagnosed as SSD with pulmonary hypertension. A 54-year-old woman had an episode of sudden oral bleeding and generalized petechiae 1 week after a sore throat and diarrhea. On admission, the platelet count was 0.1 x 10(4)/microl, and the platelet-associated IgG level was elevated. Hyperplasia of megakaryocytes in a bone marrow specimen and aberrant Epstein-Barr virus (EBV) antibody patterns led to a diagnosis of EBV-associated idiopathic thrombocytopenic purpura (ITP). Prednisolone (PSL) promptly restored her platelet count; however, she developed disorientation and affective lability soon after PSL was tapered. Subsequently, she ran a high fever and developed convulsive seizures. T2-weighted MRI demonstrated a high signal area in the subcortical white matter, and no abnormal findings were found on examination of the cerebrospinal fluid. The diagnosis of acute disseminated encephalomyelitis (ADEM) was made and steroid pulse therapy was started, which resulted in remission of the symptoms without recurrence in the following months. This is the first reported case of ADEM following EBV infection during treatment for ITP. Administration of PSL for ITP might mask the presenting clinical picture of ADEM. The possibility of ADEM should be investigated in patients of ITP following viral infection who develop acute encephalopathy.  相似文献

Background: Chronic colitis-harboring TCRα? / ? × AIM? / ? mice showed PBC-like bile duct damage in the liver. Bacterial infection is one of the candidates for the pathogenesis of PBC. We demonstrated that the bacterial cell wall component lipotheicoic acid (LTA) was detected at sites of inflammation around damaged bile ducts in PBC patients. The aim of this study was to investigate the pathophysiology of the liver and other organs in TCRα? / ? × AIM? / ? mice.

Methods: Thirteen female TCRα? / ? × AIM? / ? mice were sacrificed at 24 weeks of age. The liver, stomach, small intestine, colon, pancreas, kidney and spleen were studied for pathological examination. Using anti-LTA antibody as the primary antibody, an immunohistochemical study was carried out.

Results: In the liver, LTA was mainly detected in the portal area with inflammation, and some of the cytoplasm of hepatocytes. Inflammations were also observed in the stomach, intestine, pancreas and kidney. Throughout the gastrointestinal tract, from the stomach to the colon, LTA was detected in the epithelium at sites of inflammation. Furthermore, LTA was detected around both pancreatic ducts with inflammation and distal renal tubules with inflammation.

Conclusions: The development of inflammations in the liver as well as extensive organs, strongly suggests a close relationship between bile duct damage and systemic multifocal epithelial inflammations, perhaps involving bacterial LTA, in TCRα? / ? × AIM? / ? mice.  相似文献
