Antiviral properties of ent-labdene diterpenes of Andrographis paniculata nees, inhibitors of herpes simplex virus type 1

Andrographolide, neoandrographolide and 14-deoxy-11,12-didehydroandrographolide, ent-labdene diterpenes isolated from Andrographis paniculata showed viricidal activity against herpes simplex virus 1 (HSV-1). None of these compounds exhibited significant cytotoxicity at viricidal concentrations.     

心理一致感在脑卒中患者主要照顾者照顾负担与抑郁间的中介效应研究

目的 探讨心理一致感在主要照顾者照顾负担与抑郁间的中介作用,为提高主要照顾者身心健康提供理论参考。方法 用便利抽样方法抽取某三甲医院神经内科的脑卒中患者主要照顾者,使用一般情况调查表、照顾负担量表、心理一致感量表和流调中心抑郁量表进行调查。采用SPSS 21.0与AMOS 22.0对数据进行统计分析。结果 回收有效问卷436份,有效回收率为96.88%。主要照顾者照顾负担得分为36.50±7.49分、心理一致感得分为63.15±8.46分、抑郁得分为14.03±6.77分。心理一致感与照顾负担、抑郁得分均呈负相关(r=-0.642、-0.592,P<0.01);照顾负担与抑郁得分呈正相关(r=0.625,P<0.01)。Bootstrap 法进一步验证了心理一致感的中介效应,中介效应占总效应的16.6%。结论 心理一致感是脑卒中患者主要照顾者照顾负担与抑郁间的中介变量。     

贵州省2014-2018年人感染H7N9禽流感病例流行病学调查分析

目的 了解贵州省Ｈ７Ｎ９禽流感病例的特征,为进一步防控提供参考。方法 收集贵州省2014-2018年人感染Ｈ７Ｎ９禽流感临床及流行病学疫情资料,用描述性流行病学方法分析其流行特征。结果 2014-2018年贵州省共确诊21例人感染H7N9人禽流感病例,死亡11例,病死率52.3%。以男性、中老年、农民为主,61.9%的病例发生在黔东南和黔南地区,2017年病例数占总病例数的80.9%,未发现聚集性疫情和二代病例。病例发现以不明原因肺炎监测发现为主,从暴露到发病、发病到首诊、发病到确诊、发病到抗病毒药物使用及发病到死亡的时间中位数分别为5天、3天、7天、7天和14天;有无基础性疾病的病死率差异无统计学意义（〖XC小五号.EPS;P〗=0.38,P=0.54）,后期病例病死率低于前期（〖XC小五号.EPS;P〗=4.24,P=0.04）。19例本地病例均有明确的活禽或活禽市场相关环境接触史,其中94.7%的活禽市场或活禽相关场所均检出H7核酸阳性,平均阳性率为32.3%(1.34%～82%),全面长期关闭活禽市场后疫情得到有效控制。 结论 接触活禽或活禽市场环境是感染的主要因素;加强活禽市场外环境监测,规范化管理及长期关闭活禽市场,可有效预警和控制疫情;加强基层医疗机构救治水平可有效降低病死率。     

Complete paraparesis following resection of parasagittal meningioma: recovering function with an early intensive neurorehabilitation program

Paraparesis can occur as a primary presentation of brain pathology at the motor strip along the parasagittal region. It could also occur as a neurological complication especially following resection of parasagittal meningioma with infiltration of the superior sagittal sinus (SSS). We report a case of a complete paraparesis immediately following resection of bilateral parasagittal meningioma with infiltration of the middle third of the SSS. A gradual improvement in neurological recovery and functional outcome was observed over a period of one year after undergoing an intensive neurorehabilitation program beginning from the acute inpatient phase post surgery.     

Double-bundle ACL reconstruction versus anatomic single-bundle ACL reconstruction combined with anterolateral ligament reconstruction: a comparative study of clinical outcome

Introduction

Recent studies showed increasing evidences of anterolateral ligament (ALL) reconstruction in conjunction with anterior cruciate ligament (ACL) reconstruction that proves to be more superior to the previous double-bundle (DB) ACLR technique especially in improving knee stability in biomechanic studies. However to date, there have been no studies evaluating the functional outcome of DB-ACLR vs single-bundle (SB) with ALL in ACLR.

Methods

A total of 138 patients with ACL deficient knee were enrolled to the study. Double-bundle ACL reconstruction (DB-ACLR) was performed in 75 patients and single-bundle ACL reconstruction with anterolateral ligament reconstruction (SB + ALL) ACLR was performed in 63 patients. Surgical outcomes were compared with The International Knee Documentation Committee Subjective Knee Form (IKDC) and the Kujala score at the 3rd, 6th and 9th month post-operative.

Results

The IKDC score in the 3rd and 6th month was superior in the SB + ALL group compared to the DB group. However, this difference was not statistically significant. In the meantime, there were no statistically significant difference between the Kujala Score at the 3rd, 6th, and 9th months consecutively. There were no statistically significant differences in the functional outcomes of DB-ACLR and SB + ALL groups at the 3rd, 6th, and 9th months post-operative period.

Conclusion

The functional outcome comparison showed non-superiority of SB + ALL group compared to the DB-ACLR group. The patellar pain was less found in DB-ACLR group at 6- and 9-months post-operative period.

建立了高效薄层荧光光密度法测定生物样品中小檗碱的含量,并比较了口服小檗碱(100mg/kg)及含等量小檗碱的黄连粉(2 g/kg)后的血药浓度、降血糖效应和组织分布。     

Overexpression of DNA methyltransferase 1 (DNMT1) protein in astrocytic tumour and its correlation with O6-methylguanine-DNA methyltransferase (MGMT) expression

Background: The relationship between DNA methyltransferase (DNMT) and O6-methylguanine-DNA methyltransferase (MGMT) in mediating tumorigenesis is still poorly understood. This study was carried out to investigate a correlation between DNMT1 and MGMT immunoexpression in astrocytic tumour samples. Methods: Formalin-fixed paraffin embedded tissues of astrocytic tumour patients was obtained from an observational study conducted in Hospital Universiti Sains Malaysia (USM), which was performed from January 1997 until May 2012. Patient’s histological information was retrieved from the accessible Pathology Registry. Immunohistochemistry (IHC) staining was performed to assess DNMT1 and MGMT expressions in patients’ tumours. Results: Our data showed that DNMT1 was highly expressed in high grade astrocytic tumours. A multiple regression analysis demonstrated a significant association of DNMT1 overexpression with tumour grade III and IV (GIII: OR=5.802; 95% CI: 1.059, 31.785; p value=0.043; GIV: OR=40.663; 95% CI=4.069, 406.347; p value=0.002). The MGMT protein was downregulated in tumours with higher grade as evident by a reduction mean H-score for MGMT expression from GI to GIV [28.36 ± 43.88, 28.08 ± 33.67, 26.00 ± 48.70 and 16.20 ± 35.61]. However, a good negative correlation was observed between DNMT1 and MGMT in high grade tumour [Spearman correlation test: r=-0.561, p value≤0.001 in percentage expression and r=-0.576, p value≤0.001 in H score]. Conclusion: DNMT1 overexpression was seen correlated with a reduction of MGMT protein expression in high grade astrocytic tumour. Understanding the role of these markers could be important to overcome astrocytic tumour aggresiveness.
