This study used grounded theory to explore the process of transfer of responsibility for asthma management from parents to their school-age children. Interviews were conducted with 11 mothers, 2 fathers, 1 grandmother, and their 14 children who were between 8 and 13 years old. Responses revealed that transfer of responsibility within families is a complex, uneven process that occurs over years and involves identifiable stages and transitions. The Theoretical Model for Parent-Child Transfer of Asthma Responsibility emerged from the data. The central concept underlying the process was controlling the situation. Specific cues stimulated changes in parent-child responsibility.  相似文献

Androgen secretion by the fetal testis is essential for male phenotypic differentiation. In the human fetus testosterone formation is initiated soon after the differentiation of the testis (approximately 8 weeks of gestation), and the maximal testosterone content in fetal testes is achieved between 10 and 15 weeks of fetal life. The testosterone content of the fetal testis declines at the beginning of the third trimester and remains low until after birth. In an effort to understand the regulation of the onset of testosterone formation in the human fetal testis we measured adenylate cyclase activity in response to hCG stimulation in homogenates of fetal testes obtained from first and second trimester human abortuses. Basal adenylate cyclase activity was 50 pmol/mg protein.min at 10 weeks gestation, the peak activity was 137 pmol/mg protein.min at 12 weeks gestation, and activity declined thereafter to 8 pmol/mg protein.min by 16 weeks gestation. NaF-stimulated (0.6 mmol/L) and forskolin-stimulated (50 mumol/L) activities were 4- to 8-fold greater than basal adenylate cyclase activities. The maximal forskolin-stimulated activity occurred at 11 weeks (803 pmol/mg protein.min), and it fell to 35 pmol/mg protein.min by 17 weeks gestation. In contrast, hCG-stimulated (1 mumol/L) adenylate cyclase activity was only slightly greater than basal rates at all ages examined. In addition, hCG did not stimulate baseline testosterone formation in minces of testes obtained between 12 and 18 weeks of gestation. These findings suggest that the onset of testosterone formation in human fetal testes may be independent of gonadotropin control.  相似文献

Our work was motivated by discoveries of prokaryotic communities that survive with little nutrient in ice and permafrost, with implications for past or present microbial life in Martian permafrost and Europan ice. We compared the temperature dependence of metabolic rates of microbial communities in permafrost, ice, snow, clouds, oceans, lakes, marine and freshwater sediments, and subsurface aquifer sediments. Metabolic rates per cell fall into three groupings: (i) a rate, microg(T), for growth, measured in the laboratory at in situ temperatures with minimal disturbance of the medium; (ii) a rate, microm(T), sufficient for maintenance of functions but for a nutrient level too low for growth; and (iii) a rate, micros(T), for survival of communities imprisoned in deep glacial ice, subsurface sediment, or ocean sediment, in which they can repair macromolecular damage but are probably largely dormant. The three groups have metabolic rates consistent with a single activation energy of approximately 110 kJ and that scale as microg(T):microm(T):micros(T) approximately 10(6):10(3):1. There is no evidence of a minimum temperature for metabolism. The rate at -40 degrees C in ice corresponds to approximately 10 turnovers of cellular carbon per billion years. Microbes in ice and permafrost have metabolic rates similar to those in water, soil, and sediment at the same temperature. This finding supports the view that, far below the freezing point, liquid water inside ice and permafrost is available for metabolism. The rate micros(T) for repairing molecular damage by means of DNA-repair enzymes and protein-repair enzymes such as methyltransferase is found to be comparable to the rate of spontaneous molecular damage.  相似文献

Analyzing data from our optical dust logger, we find that volcanic ash layers from the Siple Dome (Antarctica) borehole are simultaneous (with >99% rejection of the null hypothesis) with the onset of millennium-timescale cooling recorded at Greenland Ice Sheet Project 2 (GISP2; Greenland). These data are the best evidence yet for a causal connection between volcanism and millennial climate change and lead to possibilities of a direct causal relationship. Evidence has been accumulating for decades that volcanic eruptions can perturb climate and possibly affect it on long timescales and that volcanism may respond to climate change. If rapid climate change can induce volcanism, this result could be further evidence of a southern-lead North-South climate asynchrony. Alternatively, a volcanic-forcing viewpoint is of particular interest because of the high correlation and relative timing of the events, and it may involve a scenario in which volcanic ash and sulfate abruptly increase the soluble iron in large surface areas of the nutrient-limited Southern Ocean, stimulate growth of phytoplankton, which enhance volcanic effects on planetary albedo and the global carbon cycle, and trigger northern millennial cooling. Large global temperature swings could be limited by feedback within the volcano-climate system.  相似文献

Sugars normally are absorbed in the small intestine. When carbohydrates are malabsorbed, the osmotic load produced by the high amount of low molecular weight sugars and partially digested starches in the small intestine can cause symptoms of intestinal distention, rapid peristalsis, and diarrhea. Colonic bacteria normally metabolize proximally malabsorbed dietary carbohydrate through fermentation to small fatty acids and gases (ie, hydrogen, methane, and carbon dioxide). When present in large amounts, the malabsorbed sugars and starches can be excreted in the stool. Sugar intolerance is the presence of abdominal symptoms related to the proximal or distal malabsorption of dietary carbohydrates. The symptoms consist of meal-related abdominal cramps and distention, increased flatulence, borborygmus, and diarrhea. Infants and young children with carbohydrate malabsorption show more intense symptoms than adults; the passage of undigested carbohydrates through the colon is more rapid and is associated with detectable carbohydrates in copious watery acid stools. Dehydration often follows feeding of the offending sugar. In this review we present the clinical and current molecular aspects of disaccharidase digestion.  相似文献

Knowledge of the three-dimensional balance of loads at the knee joint is required to adequately assess the treatment and rehabilitation of the malfunctioning knee. This report focuses upon the moment arms for the knee in internal/external (IE) rotation motion. It augments prior work that defined flexion/extension moment arms. Muscle excursions and angular motion of the lower leg during IE rotation were measured in 17 fresh-frozen hemi-pelvis specimens. Moment arms were calculated as the derivatives of excursion with respect to the angle. Rotational motion was performed for the normal and anterior cruciate ligament (ACL)-deficient knee. Of the 13 muscles measured at the knee, seven were significant contributors to IE rotation: the biceps femoris long and short head externally rotate opposite the gracilis, sartorious, semimembranosis, semitendonosus and popliteus, functioning as internal rotators. Moment arm magnitudes were greatest with the knee in a flexed position (internal [external] rotators peaked at 70° [90°] flexion). At 30° flexion, the IE rotation moment arm minima and maxima were 10.1–11.6, 6.8–9.0, 6.0–15.7, 8.2–14.1 and 0.0–10.4 mm for the semimembranosis, semitendonosus, gracilis, sartorius and popliteus, and 14.7–27.9 and 18.5–31.5 mm for the biceps femoris short and long, respectively. Moment arms for the ACL-deficient condition were significantly changed only at extremes of flexion–extension.  相似文献
