Experimental autoimmune encephalomyelitis (EAE) is a CD4+ T cell mediated inflammatory demyelinating disease that is induced in mice by administration of peptides derived from myelin proteins. We developed EAE in SJL mice by administration of PLP139–151 peptide. The effect of treating these mice with 1α,25-Dihydroxyvitamin D3 (vitamin D3), or with monomethyl fumarate (MMF) was then examined. We observed that both vitamin D3 and MMF inhibited and/or prevented EAE in these mice. These findings were corroborated with isolating natural killer (NK) cells from vitamin D3-treated or MMF-treated EAE mice that lysed immature or mature dendritic cells. The results support and extend other findings indicating that an important mechanism of action for drugs used to treat multiple sclerosis (MS) is to enhance NK cell lysis of dendritic cells.  相似文献

Fatal traumatic basal subarachnoid hemorrhage (TBSAH) is a characteristic forensic pathological entity, the investigation of which requires special techniques. In Liverpool, post-mortem room angiography is undertaken, followed by complete removal of the vertebral arteries and histological examination. It has been observed that the arterial anatomy can be highly variable, particularly the length and course of the loop segments located between the C2 vertebra and the dura. In a number of cases of TBSAH the loop segments of torn vessels have appeared relatively short. Having observed this phenomenon subjectively in our case work, a radiological study was undertaken with the aim of quantifying vertebral artery loop anatomy variation in a ‘normal’ population. Multiphase post-mortem computed tomography angiography (MPMCTA) scans of 98 subjects were reviewed and the lengths of the different portions of the loop segments (foramen magnum to the upper border of C1, between C1 and C2, and the bony foramina themselves) of each vertebral artery were measured using semi-automated vessel analysis software. The measurements obtained provide objective evidence of marked anatomical variation, with some loop segments more than twice the length of others. These results are important because a short vertebral artery loop segment might be a significant factor in predisposing an individual to TBSAH following a blow to the head or neck.  相似文献

Elevation of limbs during burns surgery to access the posterior aspect is routinely required. We describe a method of limb holding during burns surgery using sharp towel clips fixed to the distal phalanges of a patient's hands or feet. The limb is held in elevation using a sterile crepe bandage from the towel clips to a hook hung on a rail fixed to the theatre ceiling. We have used this technique for patients with extensive severe burns for many years with no significant damage to the nail beds or the tips of fingers and toes. This technique is convenient for surgeons as it allows easy access to hands and feet and the posterior aspects of arms and thighs. It is cost effective and safe as it spares an assistant and decreases the risk of potential occupational injury. Level of Evidence: Level V, therapeutic study.  相似文献

Background/Aims:

We aimed to evaluate the efficacy and safety of Spyglass-guided electrohydraulic lithotripsy (EHL) for difficult common bile duct stones (CBD) not amenable to conventional endoscopic therapy.

Design:

A retrospective study evaluating the efficacy of Spyglass-guided EHL in treating difficult CBD stones, in a single tertiary care center.

Patients and Methods:

All patients who underwent Spyglass-guided EHL from 2012 to 2013 were compared with a historical cohort who had ECSWL.

Results:

A total number of 13 patients underwent Spyglass-guided EHL, 8 (61.5%) of them were males. The mean age was 46.5 ± 5.6 years. Bile duct clearance was achieved in 13 (100%) of them. Seventy-six percent required only one Endoscopic Retrograde Cholangiopancreatography (ERCP) to clear the CBD, 7.7% required two ERCPs, and 15.4% required three ERCPs. Adverse effects (cholangitis) occurred in one patient (10%), whereas only 30 patients (64.4%) of the ESWL group had complete CBD stone clearance. Thirty-seven percent required one ERCP to clear the CBD, 35.6% required two ERCPs, and 20% required three ERCPs. Adverse effects happened in seven (15.5%) patients, where five (11%) had cholangitis and two (4.4%) had pancreatitis.

Conclusion:

Although a retrospective design with a small sample size, we concluded that Spyglass-guided EHL is an effective procedure in treating difficult CBD stones.  相似文献
