A 19-year-old female was referred by her dental practitioner for the restoration of missing maxillary lateral incisors and canines. Ridge augmentation was required. This was undertaken using mandibular tori as the sites for harvesting bone. The grafting was successful and the spaces were subsequently restored using resin-bonded bridgework. The case reports that mandibular tori provide a local and convenient source of bone for ridge augmentation procedures.  相似文献

Thirty patients received maxillary infiltration and mandibular inferior alveolar nerve block injections with both a traditional syringe and a computerized system, the Wand. Patients noted their preference for either system and rated their injection pain and postoperative discomfort on a ten-point scale for each type of injection. Mean injection discomfort ratings with the Wand were lower than with the syringe but were not statistically significant. Reduced postoperative discomfort using the Wand for the inferior alveolar nerve block was significant. Both of the dentists in the study and those patients who stated a preference favored the Wand system.  相似文献

INTRODUCTION: Noggin is an antagonist of bone morphogenetic proteins (BMP)-2, -4 and -7. Little data are available regarding its clinical utility. Two hypotheses were put forward: firstly, that spontaneous regeneration of calvarial defects with noggin protein would result in diminished bone volume when compared with calvarial defects not so treated. Secondly, that centrifugal cranial expansion would remain undisturbed whether noggin was applied or not. MATERIAL AND METHODS: A unilateral defect of the frontal and parietal bones (2x4cm) was generated by excising the right coronal suture in 2-month-old minipigs (n=10) and in group 1 (n=5) no further intervention was undertaken. In the second group (n=5), a collagen type I tissue fleece and noggin protein (1.05mg/ml) were applied. After 4 months the coronal suture regions of frontal sides were examined in each animal by computed tomography and non-decalcified histology. RESULTS: Bony gaps of equivalent size remained in animals of both groups. The differences in bone volumes of the experimental sides of group 1 were not statistically significantly different (p=0.117) when compared with those of group 2. A significant difference in the bone volumes of the experimental versus control (unoperated) sides was found in both group 1 (p=0.043) and group 2 (p=0.043). Internal skull diameters increased by 16.4% in both groups but the physiological centrifugal cranial expansion remained undisturbed. Bone densities of the experimental and control sides of groups 1 and 2 were not statistically significantly different (both p>0.05). CONCLUSIONS: The first hypothesis was contradicted: the quantity and quality of spontaneous bone regenerates was not altered by application of noggin protein. The second hypothesis was confirmed: no disruption of subsequent cranial development was seen. It may be that a single application of noggin protein in this study was insufficient. However, it may well be suggested that the continuous supplementation of noggin, for example by adenoviral noggin gene transfer may significantly reduce the quantity of spontaneous bone regeneration in a similar experiment.  相似文献

The principles of orthopaedic distraction osteogenesis (DO) have been successfully applied to the craniofacial skeleton, but the latency time, rate and rhythm of distraction, and length of the consolidation period that are optimal for long-bone distraction may be suboptimal for craniofacial DO. The aim of this study was to provide recommendations for optimal distraction parameters in animal experimental research on craniofacial DO. The data used were from studies, added to the PubMed database between 1 January 1973 and 1 January 2007, on the outcome of DO resulting from variations in a single distraction parameter while standardizing the other distraction parameters. Although experimental animal group sizes were rather small, especially in those studies that used large animals, and both skeletally mature and immature animals were used, the (in most cases quantitative) data provided useful information on the optimal parameters in craniofacial DO. A latency period may not be necessary at all. Distraction should be performed at a rate of 1mm/day (this may be halved when small animals such as rats are used) preferably with a continuous rhythm, followed by a consolidation period of 6-8 weeks. These recommendations can be used as basic guidelines for further animal experimental studies on craniofacial DO.  相似文献

AIM: To study the influence of two educational programmes on the adoption of nickel-titanium rotary instrumentation (NTRI) amongst general dental practitioners in a short-term as well as a long-term perspective. METHODOLOGY: All dentists employed in the Gothenburg Dental service (n = 148) were enrolled in the study. The clinics in the organization were randomly assigned to one of two educational programmes. In the first programme a 4-h lecture on root canal instrumentation was given. In the second programme the lecture course was supplemented by a 6-h hands-on training session. The short-term effect was measured by a questionnaire distributed 6 months after completed education. The long-term effect was evaluated 4 years later. RESULTS: The overall utilization rate of NTRI increased from 4% to 73%. However, lectures in combination with hands-on training resulted in a better short-term acceptance rate (94%) than if teaching was given only in lecture-format (53%) (P = 0.000). As a consequence, all staff were offered hands-on training. The long-term adoption rate was 88%. Reasons for accepting the new technology usually were found within the 'relative advantage' category. Common reasons for dentists not to adopt NTRI were that they could not get started or that they found no advantage over the old technology. CONCLUSIONS: The short-term adoption of a new technology might be influenced by the design of an introductory educational programme. For clinical procedures, such as root canal instrumentation, the inclusion of hands-on training sessions seems to be important to reach a high acceptance rate.  相似文献

The purpose of this study was to compare the cytotoxicity, induced apoptosis and/or necrosis, and apoptotic mechanisms in human periodontal ligament (PDL) fibroblasts treated with four different endodontic materials: White ProRoot mineral trioxide aggregate (MTA) (MTA/Dentsply; Tulsa Dental, Memphis, TN), Diaket (ESPE, Seefeld, Germany), Endion (VOCO, Cuxhaven, Germany), and CYMED 8410 (NANO, Kaohsiung, Taiwan). The effects of these four materials on the viability of PDL fibroblasts were determined by MTT (3-(4,5-dimethyl-thiazoyl)-2,5-diphenyl-SH-tetrazolium bromide) assay. Apoptotic pathways were evaluated via several mechanisms. Exposure to MTA for 24, 48, and 72 hours resulted in no significant differences in MTT reduction and viable cell number compared with controls. However, treatment of PDL fibroblasts with Diaket, Endion, and CYMED 8410 for 24, 48, and 72 hours resulted in cytotoxicity with MTT and a reduction of viable cell number with trypan blue dye exclusion test compared with controls (from p < 0.05 to p < 0.001). Annexin V-FITC/PI staining showed that Diaket, Endion, and CYMED 8410 induced higher percentages of apoptosis and/or necrosis than in controls (45.6%, 25.5%, and 6.3%, respectively). Results of cell-cycle analyses were concordant with annexin V-FITC/PI staining findings. These results suggest that MTA is a very biocompatible filling material. However, Diaket, Endion, and CYMED 8410 are toxic to PDL fibroblasts in vitro. The main form of cell death induced by these filling materials was determined to be apoptosis and/or necrosis.  相似文献

Summary Occlusal indicators are widely used in dental treatment to measure tooth contacts that occur during occlusion. However, the presence of an indicator may affect the mechanics of occlusion and lead to invalid tooth contact data. The objective of this study was to determine the effect of four indicators (Parkell, silk, T‐Scan® sensor and paper) on surface electromyography (SEMG) activity during occlusion. Twenty‐three subjects performed strong bites and maximum clenches onto the four indicators and natural dentition. Surface electromyography recordings of anterior temporalis and superficial masseter activity and the subjects’ perception of each indicator were measured. SEMG activity with the T‐Scan® sensor and paper was significantly different (higher masseter activity; P < 0·05) compared to that for natural dentition. The Parkell and silk gave no significant differences to natural dentition. Similarly, subjects perceived that T‐Scan® sensor and paper had the greatest effect on occlusion and were the least comfortable (P < 0·05). Thus, the very plastic T‐Scan® sensor and very thick articulating paper both affected SEMG activity during occlusion and, therefore, may not provide valid tooth contact information for dental treatment. In conclusion, occlusal indicators can change SEMG activity during occlusion which may affect the validity of the measurements they provide.  相似文献

Objectives

To evaluate food habits and oral care level of eleven and twelve years old students with the purpose of preventing obesity and oral diseases. The role of Dental Hygienists in the prevention of such problems was examined as well.

Materials and methods

A questionnaire was distributed to a sample of 90 secondary school students (41 males and 49 females).

Results

The answers point out that some improper food habits are particularly common, such as the excessive consumption of sweets and fizzy drinks, and the low consumption of fruit and vegetables.

Conclusions

Since not all children have a good oral care and follow a correct diet, the role of Dental Hygienists is paramount.  相似文献
