Hypothalamic hamartoma associated with Laurence-Moon-Biedl syndrome. Case report and review of the literature.

A rare case of a patient with Laurence-Moon-Biedl syndrome associated with hypothalamic hamartoma is described. The English-language literature contains no cases of patients with this association. The clinical manifestations of this syndrome, those of hypothalamic hamartomas, and the appearance of the tumors on magnetic resonance images are discussed.     

Cesarean section and maternal age in low-risk nulliparas.

The relationship between maternal age and cesarean section in low-risk nulliparas was analysed in 7454 perinatal clinical records selected from eleven databases of Latin American institutions obtained with a comprehensive perinatal information system. The risk of cesarean section in nulliparas between 29 and 49 years of age was found to be three times higher than that corresponding to pregnant women of lesser age. This clearly more aggressive obstetrical conduct could not be explained by the main indications for cesarean section.     

Sequential occurrence of IgA nephropathy and Henoch-Schönlein purpura: support for common pathogenesis

We report a patient who developed Henoch-Schönlein purpura (HSP) 13 years after he presented with IgA nephropathy (IgAN). In both HSP and IgAN renal biopsy most commonly reveals focal proliferative glomerulonephritis on light microscopy and immunofluorescence displays mesangial IgA deposits. In addition, patients with HSP or IgAN have elevated serum IgA levels, circulating IgA immune complexes, IgA-bearing lymphocytes, immunoglobulin-producing cells, and binding of IgG to glomerular components of similar molecular weight. The occurrence of both diseases in the same patient or the same families and the presence of immune abnormalities compatible with HSP or IgAN in relatives of patients with these diseases suggest a common pathogenesis.     

Cholecystokinin inhibits DNA alkylation induced by N-nitrosobis (2-oxopropyl)amine (BOP) in hamster pancreas.

Cholecystokinin (CCK) inhibits pancreatic cancer but not hepatic tumor induction by N-nitrosobis (2-oxopropyl) amine (BOP) in hamsters when administered with or shortly before BOP. In this study, we evaluated the capability of sulfated CCK-8 to inhibit DNA alkylation in the hamster pancreas. We examined the pattern of O6-methylguanine (G6-Me) and N7-methylguanine (G7-Me) in pancreatic ductal, acinar and liver tissues from Syrian hamsters treated with a single dose of BOP (20 mg/kg s.c.) and with five s.c. injections of CCK-8 (200 pM/kg, 30 min apart). The first CCK injection was given either 90 min before, or together, or 3 h after POP administration. The amount of G6-Me in liver DNA did not differ significantly. We observed a decrease of G7-Me in the liver of the group treated with CCK together with POP as compared to POP alone (P less than 0.005). Lower amounts of G6-Me were found in ductal preparations (P less than 0.01) of the animals treated with CCK before POP as compared to POP alone. CCK also modified the pattern of alkylation in the acinar tissue, but without a clear relationship with the timing of administration. The results suggest that the inhibitory effect of CCK-8 on pancreatic carcinogenicity of BOP could be related to its capability to modify DNA alkylation by yet unknown mechanisms.     

Effect of Bendazac Lysine on lens and retina in diabetics

The possible beneficial effects on the lens and retina which Bendazac Lysine may have in the treatment of adult diabetic patients were investigated. Twenty patients, ranging in age from 54.80 ± 5.86 years old, were studied. The average duration of the diabetes was 11.32 ± 4.10 years. Thirteen patients had background retinopathy. The metabolic controls carried out during the study were satisfactory (HbA1<11%). Bendazac Lysine (500 mg three times a day) was administered for 6 months. Blood-retinal barrier permeability (VPR and VPRt) and lens transmittance (t) were evaluated prior to and 6 months after treatment by fluorophotometry. No statistically significant differences between the pre- and post-treatment values of the retina permeability were observed, however, there was a statistically significant improvement (p<0.05) (initial value: t= 0.813 ± 0.040 and final value: t=0.823 ± 0.037) in the lens transmittance. The authors conclude that Bendazac Lysine has a beneficial effect on the lens in the diabetic adult although no improvement in the permeability of the blood-retinal barrier has been observed.     

Positive Thinking Among Slot Machine Gamblers: A Case of Maladaptive Coping?

Background  Research has demonstrated that optimism and “positive illusions” can be used a coping mechanism among those facing adversity. Gamblers are a little studied group who also experience adversity and uncertainty. They often feel considerable levels of frustration, guilt, anger and a sense of feeling cheated after making significant losses. In order to deal with such feelings it is hypothesized that these individuals will search for positive consequences from their behaviour in order to offset this negative affect. Objectives  To (1) determine whether after gambling, gamblers compensate and reduce negative affect by identifying positive consequences from experiencing a loss, and (2) identify types of strategies which gamblers employ and consider how these should be classified. Materials and Methods  Eighty-seven regular slot machine gamblers were interviewed in a variety of environments housing slot machines. Each participant was asked a series of questions in a semi-structured format, to explore possible styles of positive thinking. Results  Nine types of ‘positive thinking’ experienced by gamblers were identified. These included Comparative thinking, Prophylactic thinking, Biased frequency thinking, Responsibility avoidance, Chasing Validation, Prioritization, Resourcefulness, Thoughtfulness, and Fear Reduction. Gamblers who were positive thinkers experienced significantly less guilt than non-positive thinkers. Conclusions  While reduction of negative affect may be perceived as positive in many other contexts, it is argued that it may counteract efforts to promote responsible gambling. Clinical implications and directions for future research are discussed.     

Structural analysis of poly(3-alkylthiophene)s

Poly(3-hexylthiophene) (PT6), poly(3-octylthiophene) (PT8), poly(3-decylthiophene) (PT10) and poly(3-dodecylthiophene) (PT12) were synthesized by electrochemical and chemical polymerization. Investigation of these polymers by means of gel permeation chromatography indicates that the polymers prepared from the different monomers under similar conditions do not differ substantially considering molecular weight and molecular weight distribution. Bimodal distributions were found for electropolymerized polymers, and with increasing polymerization time, the high-molecular-weight and probably highly branched fraction increased relatively to the low-molecular-weight fraction. The microstructure of the polymers was investigated by nuclear magnetic resonance, and it has been found that the polymers exhibit more types of structural units than expected from a simple coupling of adjacent thiophene moieties in 2,5′-positions. Chemical oxidation with iron trichloride in chloroform gave soluble, high-molecular-weight poly(3-alkyl-thiophene)s with a rather low amount of irregular couplings, and these more regular polymers exhibited a higher degree of crytallinity. Room temperature conductivities of the oxidized polymers were between 0,1 and 30 S/cm depending on the polymerization conditions, but these values were rather independent of the length of the alkyl substituent.
