Chronic Myeloid Leukemia Presenting with Visual and Auditory Impairment in an Adolescent: An Insight to Management Strategies

A 15-year-old girl presented with progressive deterioration in vision and hearing over 1 week. A huge spleen was palpated below the left costal margin laying down to inguinal region. Blood count showed hyperleukocytosis with a white blood cell count of 455 × 10⁹/l. Peripheral smear yielded myeloid precursor cells with basophilia. Bone marrow aspiration revealed a blast count of 5% morphologically and 4% by flow cytometry. Fundoscopic examination revealed bilateral retinal exudates, edema and hemorhages. Partial sensorioneural hearing loss was also detected on the right ear. The diagnosis of chronic myeloid leukemia was confirmed by positive t(9;22) by RT-PCR. After commencing on hydroxyurea and intrathecal methotrexate-prednisolone, progressive improvement in hearing and vision was obtained. In our brief report, we aimed to emphasize rare presentation with visual and hearing impairment of chronic myeloid leukemia during childhood, especially in “chronic phase”.

Impact of body image on quality of life and mood in mastectomized patients and amputees in Turkey

Background: The aim was to investigate the impact of loss of the breast on body image, as well as the relationship of the body image with quality of life and severity of the depression, comparing mastectomized patients with those with an apparent limb loss (extremity amputation). Methods: Demographic data, and the side of the tissue/limb loss for mastectomy and transtibial amputation patients were recorded. Postoperative shoulder range of motion limitations of the operated-side in mastectomized patients, and postoperative knee flexion contracture in patients with amputation were evaluated. All patients were asked to complete Body Image Scale, Beck Depression Inventory, and Short Form-36. Results: All of the 40 patients in the mastectomized group were female. In the amputation group, 4 (10.8%) patients were female and 33 (89.2%) were male. Body-image scores in mastectomized and amputation groups were 98.7±26.5, and 77.2±19.7, respectively (p<0.05). Physical-function scores of quality-of-life were significantly lower in the amputation group. For both groups, a significant positive relationship was evident between body-image and depression (r=0.327 and r=0.574, respectively). There were also significant negative relationships between body-image and physical role limitations (r=-0.395) and mental health (r=-0.335) in the mastectomized group, and between body-image, and emotional role limitations in the amputation group (p<0.05). Conclusions: Although mastectomy does not result in loss of physical function, distortion of body image perception is worse than that caused by extremity amputation, and distortion of the body image affects the quality of life and mood negatively in mastectomized patients.

Serum caspase-9 levels are increased in patients with amyotrophic lateral sclerosis

It is known that apoptosis may play a role in the pathophysiology of amyotrophic lateral sclerosis (ALS). Moreover, caspase-9 is implicated in the apoptosis pathway. The aim of the study was to investigate caspase-9 levels in serum of patients with ALS. The study involved 30 patients with ALS and 30 patients from the control group. The serum caspase-9 levels were measured using the enzyme-linked immunosorbent method. The study showed that caspase-9 levels are significantly increased in serum of the patients with ALS comparing to the control group (p < 0.05). There was a significant correlation of serum caspase-9 levels with severity of clinical state of ALS patients and duration of the disease (p < 0.05). The results indicate that caspase-9 may be implicated in pathomechanism of neurodegeneration in ALS.
