术后快速康复在剖宫产围手术期护理中的应用

Objective?To explore the effect of enhanced recovery after surgery (ERAS) in perioperative nursing of cesarean section. Methods?131 full-term pregnant women who underwent cesarean section in the obstetrics department of the people's Hospital of Peking University from July 2019 to June 2020 were selected. Of which 54 cases received routine nursing measures from July to December 2019 as the control group, 77 pregnant women were treated with ERAS from January to June 2020 as the study group. The perioperative clinical data and outcome indexes of the two groups were compared. Results?The results showed that the first exhaust time of the study group was (24.2±11.9) h, the first ambulation time of (17.9±5.6) h, the catheter removal time of 16.9±4.9 h and the infusion days of 2.4±0.7 d were lower than those of the control group (30.4±12.0) h, (23.4±9.2) h, (34.9±9.6) h and (3.3±1.0) d (P<0.05). There was significant difference between the two groups in micturition patency rate and re catheterization rate (P<0.05). Conclusion?The application of postoperative rapid rehabilitation in perioperative nursing of cesarean section can promote the recovery of intestinal function, shorten the time of getting out of bed,promote the rapid recovery of maternal body.     

置入中心静脉导管的胸部手术患者术后发生导管相关性血栓的影响因素

目的 探讨置入中心静脉导管(CVC)的胸部手术患者术后发生导管相关性血栓(CRT)的影响因素。方法 选取410例行胸部手术治疗且置入CVC的患者作为研究对象，按照术后是否发生CRT分为血栓组和无血栓组。比较两组患者的一般资料和CVC留置时间。采用多因素Logistic回归模型分析置入CVC的胸部手术患者术后发生CRT的危险因素。结果 410例患者中，CRT发生率为38.0%(156/410)。两组患者的性别、手术方式、咳痰难易度及CVC留置时间差异均有统计学意义(均P<0.05)。多因素Logistic回归分析结果显示，CVC留置时间、咳痰难易度是置入CVC的胸部手术患者术后发生CRT的独立影响因素(均P<0.05)。结论 置入CVC的胸部手术患者术后CRT发生率较高，CVC留置时间长及咳痰困难的患者术后发生CRT的风险增加。
