The structure and contents of most oral health care systems and the contents of dental curricula reflect a deep-rooted tradition for attempting to cure oral diseases by refined technological means. However, better oral health conditions for the world's populations necessitate the application of up-to-date scientific knowledge to control the major oral diseases. This review points out that not only should the structure and contents of oral health care delivery systems be based on state-of-the-art knowledge about the biology of the oral diseases; they must also take into account the trends for change in caries and periodontal diseases within and between populations, and acknowledge the impact of changes in treatment philosophies for these trends. The oral disease profiles for populations in low- and high-income countries are briefly described, and it is concluded that the rapidly changing disease profiles observed in high-income countries necessitate re-thinking of the future role and organization of dentistry in such countries. The priorities for low- and middle-income countries must be to avoid repeating the mistakes made in the high-income countries. Instead, these societies might take advantage of setting priorities based on a population-based common risk factor approach. If such an approach is adopted, the training of personnel with oral health care competence must be rethought. The authors suggest three different cadres of dental care providers to be considered for an approach that allows health care planners in different populations around the world to prioritize appropriate oral health care with due respect for the socio-economic conditions prevailing. 相似文献

A good definition of commensal microflora and an understanding of its relation to health are essential in preventing and combating disease. We hypothesized that the species richness of human oral microflora is underestimated. Saliva and supragingival plaque were sampled from 71 and 98 healthy adults, respectively. Amplicons from the V6 hypervariable region of the small-subunit ribosomal RNA gene were generated by PCR, pooled into saliva and plaque pools, and sequenced by means of the Genome Sequencer 20 system at 454 Life Sciences. Data were evaluated by taxonomic and rarefaction analyses. The 197,600 sequences generated yielded about 29,000 unique sequences, representing 22 taxonomic phyla. Grouping the sequences in operational taxonomic units (6%) yielded 3621 and 6888 species-level phylotypes in saliva and plaque, respectively. This work gives a radically new insight into the diversity of human oral microflora, which, with an estimated number of 19,000 phylotypes, is considerably higher than previously reported. 相似文献

The role of extracellular matrix material (EMM) synthesized from sucrose (S) by Streptococcus mutans IB-1600 in altering the demineralizing potential of artificial plaque was evaluated with an intraoral enamel demineralization test (IEDT). The artificial plaque samples were prepared from cells cultivated in Todd-Hewitt broth (THB) supplemented with various S concentrations and by mixing THB-grown cells with increasing proportions of EMM (heat-killed THB + 2% S-cultivated cells). The samples were also evaluated for cell density (DNA content) and acidogenicity in vitro (pH-stat), as well as for in situ pH changes during a 45-minute intra-oral test following a 10% glucose rinse. An increase in the proportion of EMM relative to cell density was associated with an increase in enamel demineralization. This trend reversed when the ratio of cells to EMM was less than 1:19. Experiments involving strains of S. mitis, S. sanguis, and S. salivarius suggested a similar effect of EMM. The intra-oral pH data suggest that the presence of EMM may enhance demineralization by altering diffusion properties of plaque. 相似文献

Development of a novel digital subtraction technique for detecting subtle changes in jawbone density
The objective of this paper is to develop a novel digital subtraction technique for serial intra-oral radiography, which would allow the detection of subtle variations in grey values. Digital images of the maxilla of a dried human skull and of a fresh pig mandible were acquired using intra-oral photostimulable phosphor plates (Digora FMX, Soredex, Helsinki) with an aluminium calibration stepwedge incorporated in the filmholder. Exposures were made with an X-ray tube for intra-oral radiography (Prostyle Intra, Planmeca, Helsinki). During pilot testing, parameter settings were adapted to reach an optimal contrast. Exposures were repeated within a 1-week interval to determine the test-retest reliability of the development. After in vitro and in vivo testing, the exposure technique and software development were used to evaluate its applicability in a pilot clinical case. Although parameter settings remained stable during the in vitro studies, the clinical exposures yielded non-linear digital images, thus, not readily suitable for data acquisition and comparison of the regions of interest. To allow further analysis, image processing was carried out using self-developed software for semi-automated linearisation and optimised contrast normalisation. This processing significantly increased the precise quantisation of jawbone density and the assessment of subtle bone density changes in arbitrarily selected regions of interest of in vivo exposures. The clinical applicability of the technique is demonstrated in a pilot case. It was demonstrated that minute densitometric deviations could be detected. Non-Hodgkin lymphomas (NHLs) are often present outside the lymph nodes. Although primary extranodal NHLs (PE-NHL) form a substantial part of all NHLs, reports on oral PE-NHLs are rare. Forty patients with PE-NHL of the oral cavity have been studied for the distribution of gender, age, oral subsite and presenting complaint, histological subtype according to the WHO classification, clinical stage, treatment, and follow-up. The data are reviewed against the background of the literature. Furthermore, the International Prognostic Index has been taken into consideration. All patients had a lymphoma of B-cell lineage. Two-thirds of patients presented with locoregional disease. Mean survival time was 38 months, with a mean recurrence-free survival time of 31 months. There was no statistically significant difference in survival time between patients with bone versus soft tissue localisation of the PE-NHL. In view of the rarity of PE-NHL involving the oral region multicenter studies are needed for evaluation of the usefulness of the International Prognostic Index for non-Hodgkin lymphoma in this particular part of the body. 相似文献

The effect of pretreatment of dentin with a 5% sodium hypochlorite solution (NaOCl) or Tubulicid on the marginal adaptation of a glass ionomer cement (FUJI), a glass ionomer cement reinforced with silver particles (Ketac Silver) and 3 dentin adhesives (Clearfil, Dentin Bonding Agent and Scotch Bond) was tested in vivo. After a one month experimental period, the formation of gaps was measured from SEM photomicrographs. The sodium hypochlorite pretreated glass ionomer cement fillings showed gapfree margins, while the Tubulicid pretreated ones showed locally small gaps. The treatment of periodontitis/peri-implantitis involves the reduction/eradication of periopathogens. After therapy, beneficial and pathogenic species recolonize the subgingival area. The dynamics of recolonization and especially the role of the supragingival environment in this process are still not well-understood. This prospective, split-mouth study followed the early colonization of 'pristine' pockets created during implant surgery (16 partially edentulous patients), to record the time needed before a complex subgingival flora could be established with the supragingival area as the single source. Four subgingival plaque samples were taken from shallow and medium pockets around implants (test), and neighboring teeth (undisturbed microbiota as reference) 1, 2, and 4 wks after abutment connection. Checkerboard DNA-DNA hybridization and culture data revealed a complex microbiota (including several pathogenic species) in the pristine pockets within a wk, with a minimal increase in counts up to 4 wks. Analysis of these data demonstrated that, even with the supragingival environment as the single source for colonizing bacteria, a complex subgingival microbiota can develop within 1 wk. 相似文献

The purpose of this study was to determine whether application of an intrusive force by an intrusion arch at the distal wings of the lateral incisor brackets causes a change in the axial inclination of the anterior segment. Maxillary incisor intrusion was performed, and records were taken from 40 adolescent patients at the beginning and end of intrusion. Intrusion of the maxillary anterior segment caused a statistically significant mean increase in axial inclination of the central incisor of 8.74 degrees. The following correlations were investigated and found not statistically significant. The correlation between the (1) distance from the point of force application to the center of resistance at the start of intrusion and the change in axial inclination of the incisor, (2) distance from the point of force application to the center of resistance at the start of intrusion and the change in distance from the incisal edge to the distal side of the first molar, (3) distance from the point of intrusive force application to the center of resistance at the start of intrusion and at the end of intrusion, (4) distance from the point of intrusive force application to the center of resistance at the start of intrusion and the change in this distance between start and end of intrusion, and (5) amount of intrusion and the change in axial inclination. 相似文献
