Thrombocytopenia frequently complicates systemic infection and results from multiple possible mechanisms. We and others have demonstrated that platelet-associated IgG (PAIgG) levels are elevated in the majority of patients with septic thrombocytopenia. Corticosteroids may be undesirable as a treatment for thrombocytopenia for patients with severe infection because of their potential for suppressing the immune response. We hypothesized that septic thrombocytopenia is, in most cases, an immune disorder analogous to idiopathic thrombocytopenic purpura (ITP) which might respond to intravenous gamma-globulin as a treatment for increasing the platelet count in this disorder. Intravenous immune globulin (IVIG), 400 mg/kg daily for 3 days, was administered in a randomized double-blind placebo-controlled trial. Twenty-nine patients who developed thrombocytopenia during a documented, septic episode were studied. Patients with disseminated intravascular coagulation (DIC), hypersplenism, or drugs known to cause thrombocytopenia were excluded. Elevated PAIgG levels were documented in 52% of evaluable patients. Mean platelet counts in the IVIG group rose from 43K at study entry to 178K (411% rise) by Day 9. In the placebo group platelets rose from 51K to 125K (261% rise;P = 0.02). Seventy-seven percent of the IVIG group had a minimum peak rise of 35K, vs 56% of the placebo group. Three patients in the placebo group had a serious bleeding episode, vs one in the IVIG group. The use of IVIG to treat septic thrombocytopenia not associated with DIC leads to a more rapid, more sustained, and greater increase in platelet count than placebo. Summary The purpose of this study was to determine the effect of caffeine ingestion on physical performance after prolonged endurance
exercise. Twenty three trained male volunteers participated in a 40-km march and were divided into two groups, matched for
caffeine clearance rate and aerobic capacity. The experimental group ingested, prior to the march, a caffeinated drink at
a dose of 5 mg·kg−1 body mass and at the 3rd and 5th h of marching an additional drink at a dose of 2.5 mg·kg−1 body mass. The control group ingested a drink of equal volume at the same times. Upon termination of the march each subject
performed a cycle ergometer test at an intensity of 90% maximal oxygen consumption. Time to exhaustion and rate of perceived
exertion (RPE) were recorded. Blood samples were drawn predrink, at the 3rd and 5th h of marching and immediately after the
cycle ergometer test, and were analysed for caffeine, free fatty acids (FFA), lactate and glucose levels. Plasma FFA levels
increased during the march (p<0.05), with no significant difference between groups. Lactate levels increased in the experimental group (p<0.05), with no significant change in the control group. Glucose levels did not change significantly in either group. After
the cycle ergometer test, lactate levels were significantly higher in the experimental, as compared to the control group (3.77±0.33
vs 2.52±0.35 mmol·l−1, respectively). There was no significant difference between treatments in the time to exhaustion on the cycle ergometer,
but RPE was different (p<0.05). Under the conditions of this study, the results do not indicate caffeine ingestion as an ergogenic aid which will
postpone exhaustion following prolonged endurance exercise.

Over the last decade, we have witnessed an explosion of information regarding the function of glycoconjugates, carbohydrate-binding proteins, and the elucidation of the sugar code. This progress has yielded not only important insights into fundamental areas of glycobiology but has also influenced other fields such as immunology and molecular medicine. A family of galactoside-binding proteins, called galectins, has emerged recently as a novel kind of bioactive molecules with powerful, immunoregulatory functions. Different members of this family have been shown to modulate positively or negatively multiple steps of the inflammatory response, such as cell-matrix interactions, cell trafficking, cell survival, cell-growth regulation, chemotaxis, and proinflammatory cytokine secretion. To introduce a comprehensive overview of these new advances, here we will explore the molecular mechanisms and biochemical pathways involved in these functions. We will also examine the role of these proteins in the modulation of different pathological processes, such as chronic inflammation, autoimmunity, infection, allergic reactions, and tumor spreading. Understanding the intimate mechanisms involved in galectin functions will help to delineate selective and novel strategies for disease intervention and diagnosis. 相似文献

Children and adults with Rubinstein-Taybi Syndrome have specific medical conditions that occur with greater frequency than the general population. Based on the available information from the literature and clinical experience, recommendations for specific surveillance and interventions are made to guide those clinicians caring for individuals with Rubinstein-Taybi Syndrome. This is a first attempt at medical guidelines for individuals with RTS in the United States. A comparison of six cypovirus isolates by cross-hybridisation of their dsRNA genome segments 总被引:5,自引:0,他引:5
Summary. Genetic relationships between the genome segments of six cypovirus (CPV) isolates were analysed by RNA cross-hybridisation.
These included three type 1 viruses and single isolates of types 2, 5 and 12, which collectively are identical to those previously
compared by serology and electrophoresis [Mertens et al. (1989), J Gen Virol 70: 173–185]. Since only genome segment 10 of
three cypovirus types and segments 8 and 9 of a single virus strain (of type 1) have currently been sequenced, this initial
study provides some additional information on sequence variation / similarity in each of the ten genome segments. The RNA
of the type 1 viruses showed high levels of cross-hybridisation. Significant but much lower levels of cross-hybridisation
were detected between type 1 and the related type 12 CPV. However, only very low levels of cross-hybridisation were detected
between the other pairs of viruses. Apart from evidence of a slightly higher level of sequence similarity between the largest
segments, the RNA sequence appeared to vary uniformly across the whole genome. There was no evidence for any type specific
RNA sequences restricted to individual genome segment(s). The sequence variation, reflected in the levels of RNA sequence
similarity and cross hybridisation, correlates well with serological data, showing large differences between CPV types and
supports the continued use of electropherotype as one of the ‘species parameters’ for the classification of cypoviruses.

The large use of antibiotics entailed a dizzy fall of the bacteriological positivity in diagnosis of acute anthrax, from 98-100% to 25% in average. Common serological methods are of low sensitivity and not so confluent. The more sophisticated methods (ELISA, EITB) are of a much better sensitivity but their employment in field epidemiological investigations is difficult and does not be suitable for a retrospective diagnosis. These inconveniences can be successfully surmounted by use of the anthraxin skin test. This test allows to diagnose acute anthrax in 98-100% of cases, and retrospectively (from 45 days to 30 years after recovery) in an average of 86.2% of cases. The tetraspans associate with a large number of surface molecules, including a subset of beta1 integrins and, indirectly through CD19, with the complement receptor CD21. To further characterize the tetraspan complexes we have raised and selected monoclonal antibodies (mAb) for their ability to immunoprecipitate a molecule associated with CD9. A unique mAb was identified which recognizes the complement regulator CD46 (membrane cofactor protein). CD46 associated in part with several tetranspans and with all beta1 integrins that were tested (CD29/CD49a, CD29/CD49b, CD29/CD49c, CD29/CD49e, CD29/CD49f) but not with beta4 integrins. These data, together with cross-linking experiments showing the existence in living cells of CD46/integrin complexes, suggest that CD46 associates directly with beta1 integrins and indirectly with tetraspans. CD46 also acts as a receptor for measles virus; however, mAb to various integrins and tetraspans did not modify the virus fusion entry step. 相似文献
