Two diabetic patients in whom emphysematous pyelonephritis developed after renal transplantation are described. Clinical recognition of this unusual and serious infection is masked by the effects of immunosuppression. Abdominal radiographic, ultrasound, and computed tomography findings are discussed. The clinical presentation includes urinary tract infection, sepsis, and acute tubular malfunction of the allograft in insulin-dependent diabetics.  相似文献

A case of extraaxial ependymoma of the posterior fossa, arising along the inferolateral aspect of the left cerebellar hemisphere, is reported. The clinical, radiologic, and histologic features of this lesion are presented. The unusual location of the tumor and its possible origin are discussed in the light of previous experience with other extraaxial ependymomas of the neuraxis.  相似文献

Two cases in which gastrointestinal bleeding was localized by serial scintigraphic imaging with red blood cells labeled in vivo with 99mTc-PYP are reported. The importance of both early serial imaging and delayed imaging and of the correlation with other radiographic techniques is demonstrated. Reflux of blood from the sigmoid into the proximal colon was observed in both cases. In one case, contrast material injected into a resected specimen confirmed the scintigraphic localization of the site of bleeding.  相似文献

Summary— Cholesteryl esters in the hydrophobic core of low-density lipoprotein (LDL) particles constitute a major molecular target during copper-mediated oxidation. To facilitate the rapid analysis and quantitation of the oxidative degradation of LDL cholesteryl esters, we describe a new approach based on light scattering detection following separation by HPLC. We have applied this approach to the evaluation of the protective capacity of a new synthetic antioxidant, S20478, during oxidation of LDL in the presence of copper ions. HPLC separation of cholesterol and the four major molecular species of cholesteryl esters (C16:0, C18:1, C18:2 and C20:4) of LDL was achieved in a single run of 20 min with high sensitivity (50 ng) and low background. Time course studies of the oxidative modification of LDL (ratio LDL protein: copper, 100 μg/mL: 1μM) revealed that the content of unsaturated cholesteryl esters (C20:4 and C18:2) decreased (–30% and –15%, respectively) within 90 min of copper-mediated oxidation, while only minor degradation (up to 15%) of monounsaturated (C18:1) and saturated (C16:0) esters occurred. At 24 hours of oxidation, only traces (< 5%) of the C20:4 and C18:2 esters were detectable; whereas 52% of the C18:1 ester remained (P < 0.01). Of the saturated esters, only minor proportions (35% or less) underwent oxidative modification. In addition, some 81% of free cholesterol was conserved as the native sterol. The synthetic antioxidant, S20478 (50 μM) was capable of inhibiting the initiation and the propagation of copper-mediated LDL oxidation as determined by the time- and dose-dependant inhibition of the formation of conjugated dienes and thiobarbituric acid-reactive substances, as well as the conservation of the net electrical charge of LDL; indeed S20478 conserved cholesteryl esters in their native form up to 24 hours. However, after prolonged exposure to copper ions (48 hours), only 47% of the unsaturated esters remained (C18:2, P < 0.05). Nonetheless, S20478 (10 μM) was more efficient in inhibiting copper-mediated LDL oxidation as compared to probucol at the same concentration. These findings suggest that S20478 may be of potential interest in a new antioxidant approach to therapeutic stabilisation and regression of atherosclerotic plaques. Moreover, this method should prove useful in the assessment of the integrity of native LDL, and provides a new chemical marker of the degree of LDL oxidation.  相似文献

Summary   Background. Until recently, neurosurgeons eagerly removed cerebellar lesions without consideration of future cognitive impairment that might be caused by the resection. In children, transient cerebellar mutism after resection has lead to a diminished use of midline approaches and vermis transection, as well as reduced retraction of the cerebellar hemispheres. The role of the cerebellum in higher cognitive functions beyond coordination and motor control has recently attracted significant interest in the scientific community, and might change the neurosurgical approach to these lesions. The aim of this study was to investigate the specific effects of cerebellar lesions on memory, and to assess a possible lateralisation effect. Methods. We studied 16 patients diagnosed with a cerebellar lesion, from January 1997 to April 2005, in the “Centre Hospitalier Universitaire Vaudois (CHUV)”, Lausanne, Switzerland. Different neuropsychological tests assessing short term and anterograde memory, verbal and visuo-spatial modalities were performed pre-operatively. Results. Severe memory deficits in at least one modality were identified in a majority (81%) of patients with cerebellar lesions. Only 1 patient (6%) had no memory deficit. In our series lateralisation of the lesion did not lead to a significant difference in verbal or visuo-spatial memory deficits. Findings. These findings are consistent with findings in the literature concerning memory deficits in isolated cerebellar lesions. These can be explained by anatomical pathways. However, the cross-lateralisation theory cannot be demonstrated in our series. The high percentage of patients with a cerebellar lesion who demonstrate memory deficits should lead us to assess memory in all patients with cerebellar lesions. BackgroundGenetic markers of muscle inflammation (eg, tumour necrosis factor α [TNFα] and interleukin [IL] 6 are downregulated following repeated transient increases after bouts of exercise. Total hip replacement (THR) typically resolves preoperative pain, although strength deficits of 10–21% persist in the affected hip at 1 year postoperatively. This study assessed whether mRNA expression of TNFα and IL6 in the vastus lateralis (VL) of the operated leg was related to changes in the strength of the operated leg quadriceps in patients following THR.MethodsTen patients were recruited prospectively after ethical approval. Distal VL (5 cm proximal to lateral suprapatellar pouch) biopsy samples were obtained intraoperatively and at 6 weeks postoperatively, with maximal voluntary contraction of the operated leg quadriceps (MVCOLQ) in Newtons (N), assessed preoperatively and at 6 weeks postoperatively. RT-PCR was used to assess mRNA expression in the biopsy samples and associations evaluated with Spearman's correlation coefficient.FindingsMean mRNA relative quotient (RQ) for comparison of 6 week intraoperative VL samples was 6·23 [SD 12·85] for TNFα and 17·10 [47·46] for IL6. Preoperatively, mean MVCOLQ was 188·90 N [76·84] and at 6 weeks it was 217·00 N [53·91]. There was no significant relation between TNFα or IL-6 RQ and absolute MVCOLQ at 6 weeks (r=0·115 [p=0·376] and ?0·491 [p=0·075], respectively). No statistically significant relation existed between TNFα mRNA RQ and the improvement in MVCOLQ at 6 weeks (r=–0·498, p=0·071) nor with IL6 and the same measure (r=0·091, p=0·401).InterpretationThere is a trend to correlation that exists for improvement in MVCOLQ with a reduction in TNFα mRNA expression, as well as between absolute MVCOLQ and reduction in IL-6 mRNA expression at 6 weeks postoperatively.
