The role of oxidative stress in postoperative delirium   总被引:2,自引:0,他引:2  
AIM: This study aimed to determine a marker that predicts delirium using preoperative oxidative processes in patients undergoing cardiopulmonary bypass surgery. METHOD: Twelve of the 50 patients included in the study showed signs of delirium during postoperative follow-up. The Delirium Rating Scale was used in patients with delirium according to DSM-IV-TR in the postoperative period. Venous blood samples were obtained from the patients the day before and the day after the surgery to determine plasma antioxidant enzyme levels. RESULTS: While there were no differences in preoperative superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and malondialdehyde (MDA) levels in both groups, catalase (CAT) levels were significantly lower in the delirium group. Postoperative SOD and MDA levels were also higher in the delirium group, while the GSH-Px levels were found to be lower when compared with those during the preoperative period. In the nondelirium group, the postoperative MDA and GSH-Px levels were found to be lower than preoperative levels, and postoperative SOD levels were found to be higher than preoperative levels. CAT levels were lower in the delirium group when the pre- and postoperative levels were compared in both groups. The postoperative levels of SOD, GSH-Px and CAT in the nondelirium group and MDA in the delirium group were significantly higher than preoperative levels. CONCLUSION: Patients with low preoperative CAT levels appeared to be more susceptible to delirium than patients with higher CAT levels.  相似文献

Rheumatoid arthritis (RA) is a chronic, systemic and an autoimmune disease characterized by inflammation of the synovial membrane that affects approximately 1 % of the total world population. Rheumatoid factor (RF) is a widely used auto antibody in diagnosis of the RA and found positive in 50–80 % of the patients but with a lower specificity. On the other hand, anti-cyclic citrullinated peptide (anti-CCP) is the latest serological marker with a specificity around 98 %. This field survey was conducted in different regions to investigate the frequency of RF and anti-CCP and also frequency of RA in a northern province of Turkey. This study was conducted in 70 local areas (12 urban and 58 rural) in the province of Tokat, which is located in northern Turkey. The population of Tokat was reported to be 828,000 at the last census and about 530,000 individuals aged > 18years old. The study population of 941 subjects (462 male and 479 female; urban 501 and rural 440) was selected by random sampling method among 530,000 individuals. Of the 941 healthy controls assigned to the study, 479 of them were female (51 %) and 462 of them were males (49 %), and median age of all participants was 41 ± 17. Twenty-six subjects were RF positive (2.8 %), and 9 patients were anti-CCP positive (1 %). The presence of both RF and anti-CCP antibodies has also been shown in two patients (0.2 %). In conclusion, we demonstrated that the frequency of RA was 0.53 %, RF presence was 2.8 %, and anti-CCP presence was 1 % in total 941 healthy subjects enrolled into study.  相似文献

Objective

To investigate the renin-angiotensin-aldosterone system and angiotensin-converting enzyme (ACE) activity in patients with polycystic ovarian syndrome (PCOS).

Subjects and Methods

In this case-control study, 41 obese (PCOS) women and 29 healthy controls, matched for age and body mass index, were enrolled. Anthropometric, metabolic, and hormonal patterns, including plasma aldosterone, plasma renin, and ACE activity, were measured in each subject.

Results

Plasma renin levels were significantly higher in PCOS patients (19.7 ± 14.5 µg/ml) compared with controls (12.9 ± 9.0 µg/ml, p < 0.05). ACE activity and aldosterone levels did not significantly differ between both groups (p = 0.15 and p = 0.18, respectively). Analysis of PCOS patients showed a significant correlation of fasting insulin levels with levels of renin (r = 0.305, p < 0.01) and free testosterone (r = 0.384, p = 0.001). Similarly, homeostasis model assessment index was positively correlated with total renin concentrations (r = 0.366, p < 0.01) and free testosterone (r = 0.352, p < 0.01).

Conclusion

Obese PCOS women had higher total renin levels, but not ACE activity and aldosterone levels, related to insulin resistance compared with controls.Key Words: Polycystic ovary syndrome, Angiotensin-converting enzyme, Renin  相似文献

MGMT promoter hypermethylation of aggressive pituitary adenomas and pituitary carcinomas and low protein expression are implicated in improved response to treatment with temozolomide (TMZ). The aim of the present study was to investigate MGMT promoter methylation and immunoexpression in an aggressive subset of pituitary adenomas and carcinomas. Our material consisted of 12 silent subtype 3 (SS3) adenomas, 10 primary carcinomas, and 4 disseminated metastases. Two different tissue samples of 7 of the 12 SS3 adenomas and all carcinomas were analyzed for MGMT promoter methylation and immunohistochemical expression of MGMT. Immunoexpression was assessed semi-quantitatively as a percentage of immunoreactive nuclei. Overall 33% of carcinomas exhibited homogenous MGMT methylation in tumor and metastatic specimens. Low immunohistochemical MGMT expression was noted in 50% of carcinomas. Overall, 42% of the SS3 adenomas exhibited MGMT promoter methylation. MGMT immunostaining was predominantly negative (92%), with homogenous immunostaining results across different samples. Whereas all the methylated SS3 adenomas had low MGMT immunoreactivity, five unmethylated adenomas exhibited absent/low MGMT expression. There was no relationship between methylation status and MGMT immunoexpression was not apparent. MGMT methylation and low immunohistochemical expression seen in a subset of carcinomas and SS3 adenomas, suggesting that a subset of tumors may respond to treatment with TMZ. Heterogeneous MGMT methylation status in SS3 adenomas and the lack of concordance between methylation and immunohistochemical expression of MGMT suggest complex regulatory mechanisms, highlighting the need for improved methods in the research on a correlation between MGMT changes and response to TMZ.  相似文献

Adult rhabdomyoma is a rare primary benign tumour of striated muscle origin that almost exclusively presents in the head and neck region with predilection for male. We herein report a case of an adult rhabdomyoma in a 67-year-old male with cytologic features. The lesions was located in the tongue extended to the left parapharyngeal region. Panic patients have many functional deficiencies in the hypothalamic-pituitary-adrenal (HPA) axis. Previous studies have shown changed pituitary gland volume in some psychiatric disorders that have functional deficiencies in the HPA axis. However, to date no study has evaluated the pituitary gland volume in patients with panic disorder (PD). We investigated the pituitary gland volume in patients with PD (n = 27) and age- and sex-matched healthy controls (n = 27), using 1.5-T magnetic resonance imaging in this study. Analysis showed that patients with PD had significantly smaller pituitary volume compared to healthy subjects. Patients with agoraphobia especially had a significantly smaller pituitary volume than patients without agoraphobia. There was a significant relationship between the pituitary volume and both the severity of symptoms and the illness duration in the patient group. The results show that patients with PD have reduced pituitary volume, which may reflect the functional abnormalities seen in this disorder. These findings may help us better understand the pathology of PD.  相似文献

Objective: To compare refraction measurements of young patients obtained using Plusoptix S04 with those obtained using cycloplegic retinoscopy.Design: Cohort study.Participants: Data were collected from 64 patients (128 eyes), aged 2–19 years.Methods: All eyes underwent photorefraction using Plusoptix S04 with or without cycloplegia and cycloplegic retinoscopy. Eyes with a maximum spherical and cylinder range of −7.00 to +5.00 were excluded from the study.Results: The difference in spherical equivalents between Plusoptix S04 without cycloplegia and cycloplegic retinoscopy was more than ± 0.50 D in 70.2% of the eyes. In 100 eyes (78.1%), photorefraction underestimated the hyperopic refractive error. The cylinder powers determined by Plusoptix S04 and cycloplegic retinoscopy were similar. The difference in the spherical equivalents between Plusoptix S04 with cycloplegia and cycloplegic retinoscopy was + 0.50 D in 89.8% of the eyes. In 96 eyes (75%), photorefraction with cycloplegia overestimated the hyperopic refractive error. The difference in cylindrical power determined using Plusoptix S04 with cycloplegia and cycloplegic retinoscopy was statistically significant (paired ttest, p = 0.009).Conclusions: The statistically significant difference in spherical equivalent values obtained using Plusoptix S04 with or without cycloplegia and cycloplegic retinoscopy suggests that Plusoptix S04 is more in evaluating the cylinder power and axis in young patients. Our results also suggest that Plusoptix S04 is not an accurate tool to estimate the refraction in children.  相似文献

Purpose: To investigate the effects of cyclopentolate hydrochloride 1% on the main numerical parameters of anterior segment with Pentacam rotating Scheimpflug camera in children. Setting: Ege University Faculty of Medicine, Department of Ophthalmology. Methods: Pentacam measurements of 50 (23 boys and 27 girls) children before and after 45 min of cyclopentolate hydrochloride 1% instillation were performed and compared with 31 (17 boys and 14 girls) no medication instilled control group. Measurements of the control group (cycloplegia‐free) were taken at two separate sessions with 45‐min intervals. For comparison of the test parameters, paired t‐test in the groups and unpaired t‐test in between the groups were used. Results: Mean ages in the study and control groups were 10.06 ± 2.87 (range 6 and 16 years) and 9.87 ± 2.72 (range 6 and 16 years), respectively, and the difference was statistically insignificant (p > 0.05). In the study group, measurements between two sessions were significantly different for the parameters of anterior chamber depth (ACD), anterior chamber volume (ACV) and central corneal thickness (CCT) (p < 0.05), whereas none of the parameters was different between two sessions for control group. Conclusions: Cyclopentolate hydrochloride 1% instillation leads to significant decrement in CCT. ACD and ACV are the other important refractive parameters that increase significantly after cycloplegia. Effects of cycloplegia on these parameters for Pentacam measurements should be regarded for proper clinical interpretation in children.  相似文献
