The detection of main portal vein thrombosis (PVT) on preoperative imaging of liver transplant candidates has important technical implications for the transplantation procedure. Data are scarce regarding the accuracy of magnetic resonance imaging (MRI) at detecting PVT. The aim of our study was to compare preoperative findings of the portal vein on MRI to operative findings at liver transplantation. Abdominal MRI and clinical records of 172 consecutive patients who received liver transplants between January 1999 and September 2004 were reviewed. Two radiologists independently evaluated the last abdominal magnetic resonance examinations obtained before liver transplantation, blinded to the original reading, operative findings, and clinical data. Findings on MRI were compared with intraoperative findings at transplantation. Main PVT was detected in 12 patients, in whom 8 were found to have thrombus at surgery, with 6 requiring a jump graft or thrombectomy. Sensitivity and specificity of MRI for detecting main PVT were 100% and 98%, respectively. The cause of discordance between findings on MRI and at transplantation in 2 cases was a diminutive caliber of the main portal vein that was interpreted as recanalized chronic thrombosis on MRI. In conclusion, in our study group MRI detected PVT in all liver transplant recipients requiring jump grafts at transplantation. The major reason for a false-positive MRI was a diminutive but patent portal vein.  相似文献

The objective of this study was to demonstrate the appearance of ampullary carcinoma using current MR techniques, including fat suppression, gadolinium enhancement, and MR cholangiography. Nine patients with ampullary carcinoma were examined by MRI at 1.5 T. MR examinations included T1-weighted spoiled gradient echo, T1-weighted fat-suppressed, and immediate postgadolinium spoiled gradient echo images for all patients and MR cholangiography for three patients. The imaging features of ampullary carcinomas, including tumor size and morphology, signal intensity, and enhancement characteristics, were determined. Ampullary carcinomas shown on MR images ranged in size from 1.5 to 5.5 cm. Tumors were low in signal intensity on precontrast T1-weighted spoiled gradient echo and T1-weighted fat-suppressed images relative to normal pancreatic tissue and enhanced less than normal pancreas on immediate postgadolinium spoiled gradient echo images. Tumor conspicuity was greatest on immediate postgadolinium spoiled gradient echo images. MR cholangiography demonstrated high grade obstruction of the common bile duct and mild dilatation of the pancreatic duct at the level of the ampulla with abrupt termination of the ducts in two untreated patients and moderate dilatation of the common bile duct in one patient who had a biliary stent. Ampullary carcinomas can be demonstrated on MR images as small masses arising at the ampulla. Tumors are well defined on immediate postgadolinium spoiled gradient echo images.  相似文献

To assess the reproducibility and image quality of immediate postgadolinium chelate spoiled gradient-echo MRI in demonstrating disease of the abdominal aorta. All patients (27 patients: 21 men, 6 women) with substantial disease of the abdominal aorta, who underwent abdominal MR examinations at 1.5 T between 1991 and 1995, were entered in the study. Patients were referred for evaluation of suspected aortic disease (14 patients) or other abdominal diseases (13 patients). Three experienced investigators manually measured luminal and external aortic wall diameters and rated image quality, definition of inner and outer walls, extent of disease, and presence of other abdominal abnormalities, in an independent fashion. A cardiovascular surgeon then rated all studies to determine whether clinical management could be based on the MR findings alone. There was 98 to 99% agreement in measurements of luminal and external wall diameter between the three investigators. Overall image quality was rated as good in 77.8 to 88.9% of patients. A total of 31 additional nonaortic abdominal abnormalities were detected by all observers. The cardiovascular surgeon rated 25 of 27 studies as adequate to determine clinical management based on MR findings alone. Immediate postgadolinium spoiled gradient-echo MRI is a reproducible technique for the demonstration of abdominal aortic disease and possesses good image quality. Advantages of this technique include simultaneous evaluation of other nonvascular diseases of the abdomen, short examination time, and easy implementation as part of routine abdominal MRI scanning protocol.  相似文献

Purpose

To compare the quantitative and qualitative image quality intra‐individually, at 1.5 and 3.0 Tesla (T) in patients with chronic liver diseases.

Materials and Methods

The study group included 24 consecutive patients (17 males, 7 females; mean age ± standard deviation 56.5 ± 11.5) who had chronic liver diseases and underwent abdominal MRI for the liver evaluation at both 1.5 and 3.0T within a 4‐month period. All MRI studies were retrospectively evaluated quantitatively and qualitatively. Quantitative analysis was performed by measuring signal to noise ratio (SNR) on various abdominal organs. Qualitative analysis was performed by two reviewers to assess image quality, artifacts, and imaging findings of chronic liver diseases. Quantitative and qualitative analyses findings were compared between 1.5 and 3.0T using the paired Student t‐test and Wilcoxon signed rank test, respectively.

Results

The statistically significant increase in SNRs in various abdominal tissues ranged from 1.3‐ to 3.5‐fold at 3.0T compared to 1.5T. Three‐dimensional gradient echo (3D‐GE) sequences demonstrated significantly higher image quality at 3.0T (P < 0.01), whereas precontrast spoiled gradient echo (SGE) sequences demonstrated significantly higher image quality at 1.5T (P < 0.01). T2‐weighted sequences did not show any significant difference in image quality between 1.5 and 3.0T (P > 0.05).

Conclusion

The SNRs of various abdominal tissues demonstrated significant increases at 3.0T. The image quality of 3D‐GE sequences was higher at 3.0T, whereas the image quality of precontrast SGE sequences was higher at 1.5T. J. Magn. Reson. Imaging 2009;29:869–879. © 2009 Wiley‐Liss, Inc.  相似文献

The purpose of the study was to define the MRI appearance of multilocular cystic nephroma (MLCN), using current MR techniques, including gadolinium (Gd)-enhanced sequences. Seven patients with MLCN underwent MR imaging with the following sequences: T1-weighted spin echo with fat suppression (TIFS, five patients), T1-weighted spoiled gradient echo (SGE, seven patients), T2-weighted fast spin echo (two patients), and Gd-enhanced TIFS (seven patients) and SGE (seven patients). MLCN was histologically proven by resection of the mass in six patients and by observation of typical imaging features with stability in appearance over a 6-month period in one patient. Lesion morphology and signal intensity (SI) features were retrospectively evaluated. MRI features of MLCN included a solitary cystic lesion with thin internal septations in six patients and a cluster of closely grouped cysts similar in size in one patient. Individual cystic spaces demonstrated SI, varying from low to high on T1-weighted images in three patients and demonstrated low-to-intermediate SI in four patients. Herniation of the lesions into the renal collecting system and thin enhancing septa were demonstrated in all patients. A complex cystic renal lesion with enhancing septa and herniation into the renal collecting system are the characteristic MR findings of MLCN. The direct multiplanar capability of MR may optimally show the relationship of MLCN to the renal pelvis and, thus, facilitate correct diagnosis.  相似文献

HASTE (HAlf fourier Single-shot Turbo spin-Echo) is a single-section T2-weighted sequence that acquires images in less than 1 second. Images are breathing independent and possess a variety of other features useful for imaging the abdomen. The design of this technique is described. Clinical studies of 38 consecutive patients were performed using this technique. HASTE images were considered good in 28 and fair in 10 patients, including five patients who could not suspend respiration. Definition of liver and bowel was particularly clear.  相似文献

The efficacy of contrast-enhanced magnetic resonance imaging (MRI) for detecting and characterizing, or excluding, hepatic masses was assessed in 404 patients, following the intravenous administration of mangafodipir trisodium (MnDPDP) injection, a hepatic MRI contrast agent. An initial contrast-enhanced computed tomography (CT) examination was followed by unenhanced MRI, injection of MnDPDP (5 micromol/kg IV), and enhanced MRI at 15 minutes post injection. Agreement of the radiologic diagnoses with the patients' final diagnoses was higher for enhanced MRI and for the combined unenhanced and enhanced MRI evaluations than for unenhanced MRI alone or enhanced CT using the clinical diagnosis as the gold standard. Mangafodipir-enhanced MRI uniquely provided additional diagnostic information in 48% of the patients, and patient management was consequently altered in 6% of the patients. MnDPDP-enhanced MRI was comparable or superior to unenhanced MRI and enhanced CT for the detection, classification, and diagnosis of focal liver lesions in patients with known or suspected focal liver disease.  相似文献

Magnetic resonance imaging (MRI) and computed tomography (CT) have become essential to diagnostic evaluation of many, or most, important medically and surgically treated diseases. It is important to consider comprehensively the implications in making decisions when choosing one or the other cross‐sectional imaging modality. Factors to consider include the relative risks of the contrast agent. Other factors include the relative procedural risks, including radiation risks and the relative expected diagnostic yield of the examination technique ( 1 , 2 ). In this review we describe both nephrogenic systemic fibrosis and contrast‐induced nephropathy to compare the implications with regard to relative risks and benefits of contrast‐enhanced MRI or CT in patients with impaired renal function. The purpose of this study was to compare dual-phase spiral computed tomography (CT) and magnetic resonance imaging (MRI) using dynamic gadolinium enhancement for liver lesion detection and characterization. Twenty-two consecutive patients underwent dual-phase spiral CT and MRI for the evaluation of focal liver disease within a 1-month period. Spiral CT and MR images were interpreted prospectively, in a blinded fashion by separate, individual, experienced investigators, to determine lesion detection and characterization. Liver lesions were confirmed by surgery and pathology in 6 patients, and by clinical and imaging follow-up in the other 16 patients. Pathological correlation of a primary extrahepatic malignancy was available in 5 of the 16 patients who had metastatic liver disease. Spiral CT and MRI detected 53 and 63 lesions, and characterized 39 and 62 true positive lesions, respectively. A kappa statistic test was applied to assess agreement between MR and CT results. MR versus CT for lesion detection resulted in a kappa statistic of 0.54 (95% confidence interval), indicating moderate agreement, and 0.32 (95% confidence interval) for lesion characterization, indicating only slight agreement. More lesions were detected on MR images than CT images in 6 (27%) patients, with lesions detected only on MR images in 4 (18%) patients. More lesions were characterized on MR images in 9 (41%) patients. In 9 patients with a discrepancy between MR and CT findings, the MR images added information considered significant to patient management in all 9 cases. MRI was moderately superior to dual-phase spiral CT for lesion detection, and was markedly superior for lesion characterization, with these differences having clinical significance.  相似文献
