BackgroudModified tension band wiring is one of the most preferred surgical methods for transverse patellar fractures. However, the optimal depth or sagittal position of a Kirschner wire (K-wire) in modified tension band wiring has yet to be determined. The purpose of this study was to evaluate whether the depth of a K-wire affects the biomechanical characteristics of modified tension band wiring using the finite-element method.MethodsA patella model was designed with a cuboid shape (length, 34.3 mm; width, 44.8 mm; and thickness, 22.4 mm) and divided into the cortical and cancellous bone parts. A transverse fracture line was formed on the midline of the cuboid shape model. The cuboidal model was applied to modified tension band wiring. The depth or sagittal position of the K-wire was divided into superficial, center, and deep. With the Abaqus v2017 program (Dassault System Inc.), the distal part of the model was fixed, and a tensile load of 850 N was applied to the proximal part of the model at an angle of 45°. The maximum pressures of the cortical and cancellous bones at the fracture plane were measured. The largest von Mises values of the K-wire and stainless steel wire were also measured. The fracture gap on the distracted or anterior side was measured.ResultsIn deep K-wire placement, the highest peak von Mises values of the cortical and cancellous bones were observed. The K-wire and stainless steel wire showed the highest von Mises values in deep K-wire placement. The fracture gap was also largest in deep K-wire placement.ConclusionsThe depth of the K-wire affects the biomechanical characteristics of modified tension band wiring. Deep placement of the K-wire will be more favorable for bone union than the empirically known 5-mm anterior or center placement of the K-wire.  相似文献

We assessed the fibrovascular ingrowth into porous orbital implant with MRI. Twelve cases underwent T1WI, T2WI, Gd-enhanced T1WI, and Short Tau Inversion Recovery (STIR) images. Gd-enhanced T1WI showed homogeneous enhancement (n=4), peripheral and posterior part enhancement (n=7), and anterior part enhancement (n=1) of implants. High signal intensity portions on STIR images coincided with the enhancing portions on Gd-enhanced T1WI. Gd-enhanced T1WI is an excellent method for assessment of fibrovascular ingrowth into orbital implant and STIR images may be a comparable method to Gd-enhanced T1WI.  相似文献

Objective

To compare observer performance using liquid-crystal display (LCD) and cathode-ray tube (CRT) monitors in the interpretation of soft-copy chest radiographs for the detection of small solitary pulmonary nodules.

Materials and Methods

By reviewing our Medical Center''s radiologic information system, the eight radiologists participating in this study (three board-certified and five resident) retrospectively collected 40 chest radiographs showing a solitary noncalcified pulmonary nodule approximately 1 cm in diameter, and 40 normal chest radiographs. All were obtained using a storage-phosphor system, and CT scans of the same patients served as the gold standard for the presence of a pulmonary nodule. Digital images were displayed on both high-resolution LCD and CRT monitors. The readers were requested to rank each image using a five-point scale (1 = definitely negative, 3 = equivocal or indeterminate, 5 = definitely positive), and the data were interpreted using receiver operating characteristic (ROC) analysis.

Results

The mean area under the ROC curve was 0.8901±0.0259 for the LCD session, and 0.8716±0.0266 for the CRT session (p > 0.05). The reading time for the LCD session was not significantly different from that for the CRT session (37.12 and 41.46 minutes, respectively; p = 0.889).

Conclusion

For detecting small solitary pulmonary nodules, an LCD monitor and a CRT monitor are comparable.  相似文献

Objective

We wanted to compare the ability of screen-film mammography (SFM) and soft-copy full-field digital mammography (s-FFDM) on two different monitors to detect and characterize microcalcifications.

Materials and Methods

The images of 40 patients with microcalcifications (three patients had malignant lesion and 37 patients had benign lesion), who underwent both SFM and FFDM at an interval of less than six months, were independently evaluated by three readers. Three reading sessions were undertaken for SFM and for FFDM on a mammography-dedicated review workstation (RWS, 2K×2.5K), and for FFDM on a high-resolution PACS monitor (1.7K×2.3K). The image quality, breast composition and the number and conspicuity of the microcalcifications were evaluated using a three-point rating method, and the mammographic assessment was classified into 4 categories (normal, benign, low concern and moderate to great concern).

Results

The image quality, the number and conspicuity of the microcalcifications by s-FFDM (on the RWS, PACS and both) were superior to those by SFM in 85.0%, 80.0% and 52.5% of the cases, respectively (p < 0.01), and those by the s-FFDM on the two different monitors were similar in 15.0%, 12.5% and 35.0% of the cases, respectively (p > 0.01). The mammographic assessment category for the microcalcifications in the three reading sessions was similar.

Conclusion

s-FFDM gives a superior image quality to SFM and it is better at evaluating microcalcifications. In addition, s-FFDM with the PACS monitor is comparable to s-FFDM with the RWS for evaluating microcalcifications.  相似文献

Objective

We wanted to evaluate the feasibility and usefulness of a newly designed balloon sheath for gastrointestinal guidance and access by conducting a phantom study.

Materials and Methods

The newly designed balloon sheath consisted of an introducer sheath and a supporting balloon. A coil catheter was advanced over a guide wire into two gastroduodenal phantoms (one was with stricture and one was without stricture); group I was without a balloon sheath, group ll was with a deflated balloon sheath, and groups III and IV were with an inflated balloon and with the balloon in the fundus and body, respectively. Each test was performed for 2 minutes and it was repeated 10 times in each group by two researchers, and the positions reached by the catheter tip were recorded.

Results

Both researchers had better performances with both phantoms in order of group IV, III, II and I. In group IV, both researchers advanced the catheter tip through the fourth duodenal segment in both the phantoms. In group I, however, the catheter tip never reached the third duodenal segment in both the phantoms by both the researchers. The numeric values for the four study groups were significantly different for both the phantoms (p < 0.001). A significant difference was also found between group III and IV for both phantoms (p < 0.001).

Conclusion

The balloon sheath seems to be feasible for clinical use, and it has good clinical potential for gastrointestinal guidance and access, particularly when the inflated balloon is placed in the gastric body.  相似文献

OBJECTIVE: Our purposes were to determine whether a single application of radiofrequency energy to normal bone can create coagulation necrosis reproducibly and to assess the accuracy of MRI at revealing the extent of radiofrequency-induced thermal bone injury. MATERIALS AND METHODS: Using a 200-W generator and a 17-gauge cooled-tip electrode, a total of 11 radiofrequency ablations were performed under fluoroscopic guidance in the distal femurs of seven dogs. Radiofrequency was applied in standard monopolar mode at 100 W for 10 min. During radiofrequency ablation, the changes in impedance and currents were recorded. MRI, including unenhanced T1- and T2-weighted images and contrast-enhanced fat-suppressed T1-weighted images, was performed to evaluate ablation regions. Six dogs were killed on day 4 after MRI and one dog on day 7. RESULTS: In all animals, radiofrequency ablation created a well-defined coagulation necrosis and no significant complications were noted. The mean long-axis diameter and the mean short-axis diameter of the coagulation zones produced were 45.9 +/- 5.5 mm and 17.7 +/- 2.7 mm, respectively. At gross examination, thermal ablation regions appeared as a central, light-brown area with a dark-brown peripheral hemorrhagic zone, which was surrounded by a pale-yellow rim. On MRI, the ablated areas showed multilayered zones with signal intensities that differed from normal marrow on unenhanced images and a perfusion defect on contrast-enhanced T1-weighted images. The maximum difference between lesion sizes on MR images, established by measuring macroscopic coagulation necrosis, was 3 mm. The correlation between the diameter of coagulation necrosis and lesion size at MRI was strong, with correlation coefficients ranging from 0.89 for unenhanced T1-weighted images and 0.97 for unenhanced T2-weighted images to 0.98 for contrast-enhanced T1-weighted images (p < 0.05). CONCLUSION: Radiofrequency ablation created well-defined coagulation necrosis in a reproducible manner, and MRI accurately determined the extent of the radiofrequency-induced thermal bone injury.  相似文献

OBJECTIVE: The objectives of our study were to determine the incidence of filling defects in pulmonary arterial stumps on CT after pneumonectomy and to evaluate their radiologic and clinical significance. MATERIALS AND METHODS: We retrospectively reviewed 401 contrast-enhanced chest CT scans of 147 consecutive patients (male-female ratio, 123:24; mean age, 60 years) who underwent pneumonectomy (right, 60; left, 87) from 1996 to 2002 in our institution. CT findings were analyzed for the presence or absence of a filling defect in the vascular stump and its size, shape, and interval change on follow-up CT. CT findings were also evaluated for the length of the vascular stump and the presence of embolism in the contralateral pulmonary arteries, pneumonia, bronchopleural fistula, and bronchiolitis obliterans with organizing pneumonia. Intrathoracic or stump recurrence was also assessed in patients with lung cancer. The medical records of lung cancer patients were reviewed for the cause of pneumonectomy and stage and cell type of cancer at surgery. Statistical tests were performed to determine the relationship between the filling defect and other radiologic and clinical findings. RESULTS: A filling defect in the vascular stump was seen on CT scans of 18 patients after pneumonectomy (12%), and all had undergone the surgery for lung cancer. It was more frequently found in the right-sided stump (23.3%) than in the left-sided stump (4.6%) (p = 0.001). The vascular stump was longer in patients with a filling defect (37.2 +/- 6.8 [1 SD] mm) than those without this finding (25.0 +/- 12.5 mm) (p < 0.001). Other radiologic and clinical findings were not significantly related to the presence of the filling defect in the vascular stump. CONCLUSION: A filling defect in the pulmonary arterial stump seen on CT after pneumonectomy is thought to be an in situ thrombus caused by stasis of blood flow and is not related to pulmonary embolism, tumor recurrence, or other complications after pneumonectomy.  相似文献

Objective

To determine the feasibility of transcaval transjugular intrahepatic portosystemic shunt (TIPS) creation in patients with unusual anatomy between the hepatic veins and portal bifurcation, and inaccessible or inadequate hepatic veins.

Materials and Methods

Transcaval TIPS, performed in six patients, was indicated by active variceal bleeding (n=2), recurrent variceal bleeding (n=2), intractable ascites (n=1), and as a bridge to liver transplantation (n=1). The main reasons for transcaval rather than classic TIPS were the presence of an unusually acute angle between the hepatic veins and the level of the portal bifurcation (n=3), hepatic venous occlusion (n=2), and inadequate small hepatic veins (n=1).

Results

Technical and functional success was achieved in all patients. The entry site into liver parenchyma from the inferior vena cava was within 2 cm of the atriocaval junction. Procedure-related complications included the death of one patient due to hemoperitoneum despite the absence of contrast media spillage at tractography, and another suffered reversible hepatic encephalopathy.

Conclusion

In patients with unusual anatomy between the hepatic veins and portal bifurcation, and inaccessible or inadequate hepatic veins, transcaval TIPS creation is feasible.  相似文献

Among the MRI signs of bucket-handle tears of medial meniscus, double posterior cruciate ligament (PCL) sign denotes a low signal band anterior and parallel to the PCL, which looks like another PCL in MR images. If the bucket-handle fragment subsequently tears at the anterior horn, the torn meniscal substance can be displaced to the posterosuperior region of the PCL, and looks like another PCL behind the original PCL. We propose the lesion be called the “posterior double PCL sign” in contrast to the ordinary double PCL sign. We present a case showing the posterior double PCL sign.  相似文献

OBJECTIVE: To ascertain the incidence of acute and chronic complications of aortic intramural hematoma (IMH) and to analyze the predictors of the development of each complication. MATERIALS AND METHODS: This retrospective study includes 107 consecutive patients diagnosed with aortic IMH by means of computed tomography (CT) during the period from January 1998 to December 2003 and followed up with serial CT examinations (median follow-up period, 320 days). There were 36 patients with type A and 71 with type B IMH. Initial and follow-up CT scans were reviewed, with special attention given to the development of complications, such as increase in the thickness of IMH, clinical and hemodynamic evolution requiring urgent surgery, and development of aortic dissection and/or aneurysm. If each complication developed within 30 days after the initial episode, we classified it as an acute complication; the others were classified as chronic complications. The time interval between the initial and the subsequent CT examination showing each complication was recorded. To identify the predictors of each complication, we analyzed the demographic and CT findings with regard to the following factors: age, sex, maximum thickness of the hematoma, maximum aortic diameter on initial CT examination, ulcerlike projection (ULP) on initial and follow-up CT examinations, and the degree of atherosclerosis. The Cox proportional hazards regression model with stepwise multivariate analyses was used to determine the significant predictors of each complication. RESULTS: Sixteen patients had acute complications consisting of aortic dissection (n = 7), aortic aneurysm (n = 6), and acute clinical and hemodynamic evolution requiring operation (n = 3). Three additional patients with aortic dissection (n = 1) and aneurysm (n = 2) underwent emergency surgery. Twenty-three patients with chronic complications had aortic dissection (n = 3), and aortic aneurysm (n = 20). Cox proportional hazards regression model revealed that the maximal diameter of involved aorta is the only significant predictor of the development of acute complications (P = 0.006), whereas the age (P = 0.040), type A IMH (P = 0.015), presence of ULP (P = 0.015), and newly developed ULP as revealed on follow-up CT examination (P = 0.032) were significant predictors of the development of chronic complications. With regard to the aortic dissection in 10 patients (9.3%; type A/B ratio, 5:5; median time interval, 34 days), Cox proportional hazards regression model revealed that the maximal thickness of the hematoma is the only significant predictor (P = 0.018). Twenty-one saccular and 5 fusiform aneurysms (24.3%) developed, as revealed on follow-up CT examinations (median time interval, 180 days). The presence of ULP (P = 0.030), type A (P = 0.038) and the maximal thickness of the hematoma (P = 0.017) were significant predictors for the development of an aneurysm. CONCLUSIONS: The maximum thickness of a hematoma on the initial CT is the significant factor predicting the development of aortic dissection and aortic aneurysm. Patients with type A IMH and ULP, as revealed by initial and short-term follow-up CT examinations, should be carefully followed up with subsequent CT examination to monitor the development of an aortic aneurysm, which is a relatively common chronic complication of IMH.  相似文献
