Pulmonary embolism after hip or knee replacement: postoperative changes on pulmonary scintigrams in asymptomatic patients

Serial pulmonary imaging has proved to be effective in the evaluation of patients undergoing total joint arthroplasty. A clinical dilemma arises in asymptomatic patients whose postoperative pulmonary images differ from the preoperative images. The authors prospectively evaluated 403 patients with serial imaging to determine the significance of changed postoperative images in asymptomatic patients undergoing total hip or knee arthroplasty. Twenty-two (5.5%) patients had significant changes on postoperative images. Seventeen were asymptomatic; all but one underwent pulmonary angiography. Documented pulmonary emboli were demonstrated in 100% of patients whose postoperative images changed to indicate a high probability of pulmonary embolism, 71% whose images changed to a moderate probability, and 0% whose images changed to indeterminate probability. Overall, pulmonary emboli occurred in 76% of all asymptomatic patients with significantly change postoperative images. Asymptomatic pulmonary embolism is a significant occurrence after total hip or knee repair, and a changed lung scan with appropriate clinical evaluation is an accurate indicator of pulmonary emboli in asymptomatic postarthroplasty patients.     

US-guided percutaneous biopsy: use of a screw biopsy stylet to aid needle detection

Insertion of a screw biopsy stylet into a thin-walled biopsy needle greatly enhances detection of the needle during ultrasound-guided percutaneous biopsy. This technique is helpful when precise needle-tip localization is needed for biopsies of small lesions.     

Anti-Sc1 in pregnancy

Anti-Sc1 was detected in a gravida-2 patient at 12 weeks' gestation. At 29 weeks, the antibody was found to be of the IgG3 subclass with a titer of 16, score 36, by the indirect antiglobulin test, and it produced 7 percent lysis by antibody-dependent cellular cytotoxicity (ADCC) assay, a finding that suggested an unaffected fetus. The titer remained constant throughout the pregnancy, as did the IgG subclass and activity in the ADCC assay. At delivery of the full-term infant, the cord hemoglobin was 13.5 g per dL and the direct antiglobulin test was positive (3+) with anti-IgG. The infant did not require transfusion. A sample taken 9 weeks after delivery showed 44 percent lysis in the ADCC assay. The anti-Sc1 titer was 32, score 65.     

Age characteristics of the normal human skeletal musculature--mathematical description of postnatal skeletal muscle growth

The number of muscle fibres per mm2 is age dependent, and therefore briefly denoted as age specific number (Alterskennzahl AK). We determined the AKs for the age groups up to 25 years, separated after the sexes and for M. vastus lateralis et deltoideus. According to the trend of the original data the approximations have been carried out applying 4 functions of organismic growth in modified form, i.e. the Pütter (1920), Bertalanffy (1934), Richards (1959), Janoschek extended by Sager (1984) and Richards extended after Sager (1980). Curve fitting is reached by regression analysis and nonlinear regressions using the sums of the linear deviations as a criterion for the goodness of fit. Results including initial as well as final values are summarized in tables with the parameters of the calculation methods added. The AKs allow judging the degree of maturity in the skeletal musculature during childhood. Transposing of the equations for AK = W yields t as a function of the normally adjunctive age specific number. The difference against real age reveals to which degree the musculature is eventually underdeveloped. Concerning the interpretation the fact should be kept in mind that biological processes are involved. The relation of the fibre types too implies a characteristic age dependency. This statement and the fibre diameters gained from the same material allow a mutual control and an exact fixation in each case.     

A review of tazarotene in the treatment of photodamaged skin

Chronic sun exposure leads to photodamage, which is characterized clinically by fine and coarse wrinkles, dyspigmentation, telangiectasia, laxity, roughness and a sallow appearance. Many treatments claim to reduce the signs of photodamage, however evidence from randomized controlled trials (RCT) to support these claims is limited. The use of topical retinoids, particularly tretinoin, isotretinoin and tazarotene, has been shown to significantly reduce signs of photodamage both clinically and histologically. Over recent years a number of RCTs, have affirmed that topical tazarotene is an effective and safe treatment for photodamaged skin.
