ObjectiveTo determine the appropriate amount of indocyanine green for bronchial insufflation.MethodsWe enrolled 20 consecutive patients scheduled for anatomical segmentectomy in the Kochi Medical School Hospital. After inducing general anesthesia, 6 to 60 mL of 200-fold-diluted indocyanine green (0.0125 mg/mL) was insufflated into the subsegmental bronchi in the targeted pulmonary segmental bronchus. The volume of the targeted pulmonary segments was calculated using preoperative computed tomography. Fluorescence spread in the segmental alveoli was visualized using a dedicated near-infrared thoracoscope.ResultsThe targeted segment was uniformly visualized by indocyanine green fluorescence in 16/20 (80.0%) cases after insufflating indocyanine green. A receiver operating characteristic curve indicated that the area under the curve was 0.984; the optimal cut-off volume of diluted indocyanine green for insufflation was 8.91% of the calculated targeted pulmonary segment volume.ConclusionsThe setting for indocyanine green insufflation was optimized for near-infrared fluorescence image-guided anatomical segmentectomy. By injecting the correct amount of indocyanine green, fluorescence-guided anatomical segmentation may be performed more appropriately.  相似文献

Background The cellular immune system is the most dominant factor in curing acute dermatophytosis. However, the exact immune mechanisms involved in generating this defense are complex and still obscure. The aim of this study was to investigate the fungicidal mechanism of T cells in the normal population versus patients with chronic fungal infections. Methods Thirty patients were included in the study: 15 patients with chronic dermatophytosis and 15 normal healthy patients with a history of acute dermatophytosis. The procedures were performed as follows. 1) Proliferation and cytotoxic activity of lymphocytes cultured with various dermatophytes homogenate such as, Trichophyton rubrum, Trichophyton mentagrophytes and Microsporum gypseum. 2) CD4+ and CD8+ T cells were separated by magnetic beads before culture with fresh spores of either T. mentagrophytes or T. rubrum. 3) Routine histology and ultrastructural study were performed to illustrate the mode of activity of the T cells against the dermatophytes. Results The study showed that both CD4 and CD8 possess cytotoxic activity against dermatophytes. However, the results demonstrated a suppression of lymphocyte proliferation response and a significant lower cytotoxic effect in chronic patients. Ultra structure and histological evaluation of the culture of hyphae with CD4+ or CD8+ T cells showed more prominently destructive effects in the culture of cells that had been obtained from normal population than those of patients with long‐lasting fungal infections. Conclusion The study suggests a selective impairment of lymphocyte function against dermatophytes, in patients with chronic dermatophytoses.  相似文献

BACKGROUND/AIMS: Serum concentrations of the 7S fragment of type IV collagen (7S collagen), amino-terminal propeptide of type III procollagen (PIIIP), and hyaluronic acid (HA) have been reported to serve as serologic markers of liver fibrosis in hepatitis and cirrhosis. We investigated whether these fibrosis markers reliably reflect histologic changes in the livers of patients with hepatocellular carcinoma. METHODOLOGY: Subjects included 165 patients undergoing liver resection for hepatocellular carcinoma. Most were seropositive for chronic hepatitis B or C. Histopathologic changes in liver tissue resected with the tumor were scored according to Knodell's histologic activity index. Serum was sampled for assays shortly before surgery. RESULTS: Significant correlations were found between hepatitis activity score and 7S collagen, PIIIP, and HA. Concentrations of 7S collagen differed significantly between activity grades, but differences were not significant for PIIIP or HA. Significant correlations were found between fibrosis staging score and all these three markers. When patients were divided according to activity grade, 7S collagen showed stronger correlation with fibrosis staging score than did PIIIP or HA. CONCLUSIONS: The 7S collagen fragment correlated more strongly than PIIIP or HA with stage and activity grade in patients with hepatocellular carcinoma. However, overlapping of results between histologically defined groups appeared to limit clinical diagnostic usefulness of all markers in individual patients.  相似文献

BACKGROUND/AIMS: Hepatocytes express bile duct-type cytokeratin in various conditions, which indicates the presence of hepatic progenitor cells. We investigated the clinical significance of cytokeratin 7 expression in the noncancerous hepatitis tissue in patients with hepatitis B virus-related hepatocellular carcinoma. METHODOLOGY: Specimens were obtained from 24 patients who underwent liver resection for hepatitis B virus-related hepatocellular carcinoma. The hepatocytes in the noncancerous hepatic tissue were examined immunohistochemically using monoclonal antibodies against cytokeratin 7. RESULTS: Expression of cytokeratin 7 was observed in 9 patients (group 1). Group 2 consisted of the other 15 patients. Serum transaminases were significantly higher in group 1 than in group 2. The percentage of patients with moderate or severe active hepatitis, the percentage of patients with cirrhosis, and the percentage of patients with portal invasion were significantly higher in group 1 than in group 2. The tumor-free survival rate was significantly lower in group 1 than in group 2. CONCLUSIONS: The expression of cytokeratin 7 in noncancerous hepatic tissue is closely related to the severity of and duration of chronic hepatitis B. The expression of cytokeratin 7 in the noncancerous hepatic tissue is a possible risk factor for recurrence after resection of hepatitis B virus-related hepatocellular carcinoma.  相似文献
