BACKGROUND: Insertion of an anterior lumbar interbody fusion cage has been shown to reduce motion in a human spine segment in all loading directions except extension. The "stand-alone" cages depend on compressive preload produced by anular pretensioning and muscle forces for initial stabilization. However, the effect that the in vivo compressive preload generated during activities of daily living has on the construct is not fully understood. This study tested the hypothesis that the ability of the cages to reduce the segmental motions in flexion and extension is significantly affected by the magnitude of the externally applied compressive preload. METHODS: Fourteen specimens from human lumbar spines were tested intact and after insertion of two threaded cylindrical cages at level L5-Sl. They were subjected to flexion and extension moments under progressively increasing magnitudes of externally applied compressive follower preload from 0 to 1200 N. The range of motion at level L5-S1 after cage insertion was compared with the value achieved in the intact specimens at each compressive preload magnitude. RESULTS: The cages significantly reduced the L5-S1 flexion motion at all preloads (p < 0.05). They decreased flexion motion by 29% to 43% of that of the intact specimens for low preloads (0 to 400 N) and by 69% to 79% of that of the intact specimens under preloads of 800 to 1200 N. In extension, in the absence of an externally applied preload, the cages permitted 24% more motion than the intact segment (p < 0.05). In contrast, they reduced the extension motion at preloads from 200 to 1200 N. Under preloads of 800 to 1200 N, the reduction in extension motion after cage placement was 42% to 48% of that of the intact segment (p < 0.05). The reduction of motion in both flexion and extension after cage placement was significantly greater at preloads of 800 to 1200 N compared with the motion reductions at preloads of < or =400 N (p < 0.05). CONCLUSIONS: In contrast to the observed extension instability under anular tension preload only, the two-cage construct exerted a stabilizing effect on the motion segment (a reduction in segmental motion) in flexion as well as extension under externally applied compressive preloads of physiologic magnitudes. The external compressive preload significantly affected the stabilization provided by the cages. The cages provided substantially more stabilization, both in flexion and in extension, at larger preloads than at smaller preloads. Clinical Relevance: The study suggests that the segment treated with an anterior lumbar interbody fusion cage is relatively less stable under conditions of low external compressive preload. The magnitude of preload required to achieve stabilization with stand-alone cages may be only partially achieved by anular pretensioning. Since the magnitude of the preload across the disc space due to muscle activity can vary with activities of daily living, supplemental stabilization of the cage construct may provide a more predictably stable environment for lumbar spine fusion. 相似文献

OBJECTIVE: To determine the efficacy of rosiglitazone compared with placebo in reducing hyperglycemia. RESEARCH DESIGN AND METHODS: After a 4-week placebo run-in period, 959 patients were randomized to placebo or rosiglitazone (total daily dose 4 or 8 mg) for 26 weeks. The primary measure of efficacy was change in the HbA1c concentration. RESULTS: Rosiglitazone produced dosage-dependent reductions in HbA1c of 0.8, 0.9, 1.1, and 1.5% in the 4 mg o.d., 2 mg b.i.d., 8 mg o.d., and 4 mg b.i.d. groups, respectively, compared with placebo. Clinically significant decreases from baseline in HbA1c were observed in drug-naive patients at all rosiglitazone doses and in patients previously treated with oral monotherapy at rosiglitazone 8 mg o.d. and 4 mg b.i.d. Clinically significant decreases from baseline in HbA1c were also observed with rosiglitazone 4 mg b.i.d. in patients previously treated with combination oral therapy. Approximately 33% of drug-naive patients treated with rosiglitazone achieved HbA1c < or =7% at study end. The proportions of patients with at least one adverse event were comparable among the rosiglitazone and placebo groups. There was no evidence of hepatotoxicity in any treatment group. There were statistically significant increases in weight and serum lipids in all rosiglitazone treatment groups compared with placebo. For LDL and HDL cholesterol, the observed increase appeared to be dose related. CONCLUSIONS: Rosiglitazone at total daily doses of 4 and 8 mg significantly improved glycemic control in patients with type 2 diabetes and was well tolerated. 相似文献

Fetal hemoglobin (Hb F) is a major hemoglobin (Hb) component at birth. Hb F levels are markedly elevated in a number of inherited and acquired disorders. Measurement of Hb F levels is usually carried out by alkali denaturation which is not very accurate for low and high values. An accurate estimation of Hb F, and also of F cells, is desired in many hematological disorders like sickle cell disease, in monitoring the efficacy of hydroxyurea (HU) therapy, to assess feto-maternal hemorrhage (FMH) during pregnancy and in the postpartum period. We have raised a murine monoclonal antibody to human Hb F, that accurately measures the number of F cells by flow cytometry. The antibody was found to be potent and specific for F cells. 相似文献

In a previous report the effectiveness of intraperitoneal bupivacaine in reducing pain following laparoscopic cholecystectomy was demonstrated. Other methods of pain relief are commonly used but none has been compared following laparoscopic cholecystectomy. In two further studies we have compared the analgesic effect of intraperitoneal bupivacaine against wound infiltration with bupivacaine, and against intraperitoneal bupivacaine with the addition of a non-steroidal anti-inflammatory drug (NSAID) in patients undergoing laparoscopic cholecystectomy. Two consecutive studies were performed. In the first, patients in group 1 were given 20 ml of 0.25% bupivacaine into the peritoneal cavity; patients in group 2 were given 20 ml of 0.25% bupivacaine injected into the trocar wounds. In the second study, patients in group 1 were given 20 ml of 0.25% bupivacaine into the peritoneal cavity; patients in group 2 were given 20 ml of 0.25% bupivacaine into the peritoneal cavity and a diclofenac suppository (100 mg) one hour before surgery. Postoperative pain was assessed with a visual analogue pain scale. There was no difference in pain scores in the two groups in either study. Intraperitoneal bupivacaine is as effective as wound infiltration. The addition of an NSAID makes no difference in the reduction of postoperative pain following laparoscopic cholecystectomy. 相似文献

L-Tryptophan (L-TP) has been used in migraine and other pain conditions. The mechanism underlying the analgesic effect is still partly undefined. In this study the effects of subchronic administration of L-5-hydroxy-tryptophan (L-5HTP) (with and without carbidopa) on plasma beta-endorphin (beta-EP) levels and subjective pain threshold and tolerance were investigated in seven healthy volunteers. To measure also an objective indicator for pain, the nociceptive flexion reflex threshold was studied. L-5HTP treatment with and without carbidopa administration increased beta-EP levels significantly (p less than 0.05). L-5HTP plus carbidopa induced an increase in beta-EP significantly (p less than 0.05) higher than that after L-5HTP alone. Neither subjective pain threshold and tolerance nor RIII threshold was modified by either treatment. Our data seem to point to the existence of a complex linkage between plasma opioid levels and pain perception. 相似文献
