动脉路去白细胞滤器在体外循环中的临床应用   总被引:4,自引:1,他引:3  
目的 观察常规瓣膜手术患者体外循环 (CPB)中应用动脉路去白细胞滤器的临床使用效果及对炎症介质的影响。方法  30例风心病患者随机分为试验组和对照组 ,各 15例 ,试验组使用动脉路去白细胞滤器LG6 ,对照组使用常规的国产动脉过滤器。测定血细胞计数及分类、血浆丙二醛 (MDA)、髓过氧化物酶 (MPO)、白介素 - 1β(IL - 1β)和白介素 - 8(IL - 8) ,动脉血气分析。观察两组术前、注射鱼精蛋白后和术后 4h的氧合指数及气道压力。用Swan -ganz导管监测肺血管压力及阻力。结果 白细胞计数 :实验组 -右房 (8.4 7± 2 .0 1)× 10 9/L、左房(7.96± 2 .2 7)× 10 9/L(P >0 .0 5 ) ,对照组右房 (10 .6 9± 2 .4 7)× 10 9/L、左房 (9.6 7± 1.98)× 10 9/L(P <.0 0 1)。血小板和红细胞压积两组无差异。术后肺氧合指数 (PaO2 /FiO2 × 10 0 )两组间无统计学差异。术后其它肺参数如肺血管阻力 (PVR)、平均肺动脉压 (MPAP)、肺毛细血管楔压 (PCWP)和气道压力等两组间无统计学差异。血浆MDA ,MPO ,IL - 1β和IL - 8两组间无统计学差异。结论 CPB中应用动脉路去白细胞滤器可暂时减少循环血液中的白细胞和中性粒细胞数量 ,减少再灌注过程中白细胞的肺内滞留 ,对术后肺功能没有明显的保护作用。  相似文献

目的 评价控制性降压是否增加脊髓对牵拉损伤的易感性。材料与方法健康成年杂种犬6只,随机分为常压和控制性降压脊髓牵拉损伤组。观察常压及控制性降压水平下相同程度牵拉损伤后脊髓血流(SCBF)、体感诱发电位(SEP)、神经源性运动诱发电位(NMEP)改变的差异。结果 外周血有创动脉压(MABP)平均下降幅度为40.5%。经SSPS统计软件独立样本t检验,不同牵拉水平下,常压组及低压组的SCBF(%)、SEP波幅(Asep)(%)及NMEP波幅(%)无显著差异。结论 尼卡地平控制性降压不增加脊髓对牵拉损伤的易感性。  相似文献

Objective: Severe scoliosis refers to scoliosis with serious and stiff curve. It always combins with trunk imbalance in coronal and sagittal contour. Besides complex pathological changes, cardiopulmonary deficits and other concomitant diseases increase treatmental difficulties. So the treatment of severe scoliosis is always a great challenge to spine surgeon. Methods :Thirty-six patients with severe scoliosis received one stage posterior correction followed by anterior release during July 1997 to January 2003, including 9 males and 27 females. Mean age was 17.2 years. Of them, 33 was idiopathic scoliosis and 3 was neurofibromatosis scoliosis( Cobb angle: 85-116 degree); 20 cases were abnormal in sagital plane. Three-dimensional devised instrumentation were applied such as CD, CD-Horizon, TSRH or Isola in posterior procedure followed by anterior release during the same anesthesia. 31 cases of this group received thorac icplasty. Results: The correction in the frontal plane achieved an average of 48.5%. In the sagittal plane, the pathological shape of the spine was reduced and distinctly ameliorated. 80. 6% of the patients maintained or achieved balance of sagittal plane. There were no complications of severe neurological deficit, hook displacement, rod broken, and deep infection at follow-up. One case occurred traumatic pleurisy after operation and another appeared pseudarthrosis 2 years later. One case demonstrated imbalance 11 months after operation. One patient was presented loss of correction more than 10 degree at one year follow-up and 5.2 degree in average. Conclusion:The study indicates that the one stage posterior correction combined with anterior release in treatment of severe scoliosis can achieve satisfactory correction. Appropriate choice of cases, preoperational detailed assessment and application of SEP and wake-up test during operation can possibly reduce severe complication. The long-term outcomes still need further observation.  相似文献

Various methods of cryopreservation of human endothelial cells (EC) were studied to determine their viability and behavior when seeded onto vascular prostheses made of polytetrafluoroethylene (PTFE). Three different protocols were used: (1) cyropreservation of whole umbilical vein, (2) cyropreservation of freshly extracted umbilical EC in suspension, and (3) cryopreservation of EC derived from a first subculture. Fresh EC and EC from a first subculture were used as controls. The viability and growth of these cells in culture media were studied, and basal prostacyclin levels were determined. The cells were assessed morphologically after they were seeded onto PTFE discs. Our results showed that the cryopreservation method that maintained the greatest viability was that in which previously cultured EC were used. Basal prostacyclin levels were significantly different following cyropreservation. However, when these cells were seeded onto PTFE discs their behavior was similar to that of fresh EC.We thank W. L. Two alternatives for the treatment of lead intoxication, administration of zinc or a thiol donor, S-adenosyl-L-methionine (SAM), were analysed. Rats were exposed to lead (Pb)-acetate (60 mg/1) in drinking water during 90 days; one group also received SO4Zn in water (40 mg/l), while another received both Pb and SAM (5 mg/24 hr intraperitoneally. Erythrocytic δ-aminolaevulinic dehydratase (ALA-D) activity was significantly reduced (P<0.001) both in rats receiving Pb alone and in rats receiving Pb and each of the other two treatments. The high erythrocytic uroporphyrinogen synthetase (URO-S) activity noticed in Pb administered rats, was significantly (P< 0.001) reduced in animals treated either with zinc or with SAM. Hepatic ALA-D activity tended to decrease while renal enzyme activity was not modified by the low level Pb exposure used in this work. Interestingly, SAM treated rats in both tissues exhibited significantly (P<0.01) higher activities of the enzyme. It is argued that SAM treatment causes a surplus of thiols that allows the full expression of ALA-D catalytic activity.  相似文献

本研究经杀菌试验和免疫后保护力试验探讨小鼠巨噬细胞在抗白色念珠菌感染中的作用,结果表明白色念珠菌酵母形体对巨噬细胞杀伤作用的抵抗力及对小鼠的毒力均较菌丝形体强。小鼠体内不同部位巨噬细胞杀伤白色念珠菌的作用不同,脾脏巨噬细胞作用大于腹腔残存巨噬细胞。经白色念珠菌活菌静脉免疫后脾巨噬细胞杀白色念珠菌作用增强,但其对再感染的抵抗力不增强。  相似文献

The neuropeptides neurotensin and neuromedin N (from 10−12 M to 10−9 M) have been showed in this study to stimulate significantly in vitro several steps of the phagocytic process: adherence to substrate, chemotaxis, ingestion of inert particles (latex beads) and production of superoxide anion measured by nitroblue tetrazolium reduction in resting peritoneal macrophages from BALB/c mice. A dose-response relationship was observed, with a maximal stimulation of the phagocytic process at 10−11 M. The two neuropeptides induced no change of intracellular cyclic AMP in murine macrophages. Moreover, adherence and chemotaxis decreased significantly in the presence of EGTA (1 mM), a chelator of extracellular Ca2+, or ryanodine (0.5 mM), a blocker of a Ca2+-gated channel from the endoplasmic reticulum, in both controls and samples with the addition of neurotensin or neuromedin N. These results suggest that there is no relation between the cAMP messenger system and the phagocytic process stimulation in murine peritoneal macrophages by neurotensin or neuromedin N. In addition, the results observed with EGTA and ryanodine could indicate that these two neuropeptides produce their effects through an increase of intracellular Ca2+ concentration.  相似文献
