In this study, pulmonary MR angiography (MRA) using a tailored coil at 4 Tesla in conjunction with an intravenous injection of contrast agent is described. Three-dimensional gradient-echo images were obtained during the intravenous injection of 0.05, 0.1, and 0.2 mmol/kg body weight of gadodiamide to investigate the signal enhancement effect of the contrast agent in pulmonary arteries qualitatively and quantitatively. In the qualitative analysis, the subsegmental branches were visualized on every dose. In the quantitative analysis, the average contrast-to-noise ratios (CNRs) of the main pulmonary arteries increased in a dose-dependent manner. However, the CNRs of segmental arteries did not increase as the dose of contrast agent increased, as observed at 1.5 Tesla MRI. These observations demonstrate the feasibility of delineating the pulmonary vasculature using a contrast agent; however, our results also suggest possible high-field-related disabilities that need to be overcome before high-field (> or =4 Tesla) MRI can be used to full advantage.  相似文献

The MR signal intensity change in the pulmonary parenchyma during the cardiac cycle was studied using HASTE sequence in volunteers. In addition, the potential to assess pulmonary perfusion abnormality by subtraction between diastolic and systolic HASTE images was tested in a pig model of pulmonary embolism. Signal intensity decreased in systole while it increased gradually in diastole. In a pig model with pulmonary embolism, subtracted images could identify the perfusion abnormality. Thus, subtraction of diastolic and systolic HASTE images has the potential to detect pulmonary perfusion abnormality. The technique may provide a new simple method for evaluating pulmonary perfusion.  相似文献

Oxygen-enhanced MR imaging is a new technique, and its physiological significance has not yet been fully elucidated. This review article covers (1) the theory of oxygen enhancement and its relationship with respiratory physiology; (2) design for oxygen-enhanced MR imaging sequencing; (3) a basic study of oxygen-enhanced MR imaging in animal models and humans; (4) a clinical study of oxygen-enhanced MR imaging; and (5) a comparison of advantages and disadvantages of this technique with those of hyperpolarized noble gas MR ventilation imaging. Oxygen-enhanced MR imaging provides not only the ventilation-related, but also respiration-related information. Oxygen-enhanced MR imaging has the potential to replace nuclear medicine studies for the identification of regional pulmonary function, and many investigators are now attempting to adapt this technique for routine clinical studies. We believe that further basic studies as well as clinical applications of this new technique will define the real significance of oxygen-enhanced MR imaging for the future of pulmonary functional imaging and its usefulness for diagnostic radiology and pulmonary medicine.  相似文献

OBJECTIVE: The aim of this study was to determine the frequency of tracheomalacia incidentally detected on CT pulmonary angiography in patients with suspected pulmonary embolism. MATERIALS AND METHODS: CT records of 163 consecutive patients imaged with CT pulmonary angiography for suspected pulmonary embolism were retrospectively reviewed at our institution. The patients underwent CT pulmonary angiography with three different types of CT scanners. All images were visually assessed by two thoracic radiologists for excessive collapse of the trachea and the main bronchi. The cross-sectional area of the lumen at the site of maximal collapse of the airway was measured, and the percentage of luminal narrowing was calculated by comparing it with the cross-sectional area of the airway lumen at an adjacent area without collapse. We defined tracheomalacia as a 50% or greater decrease in tracheal lumen. RESULTS: Sixteen (10%) of 163 patients showed evidence of tracheomalacia (seven men, nine women; age range, 41-95 years; mean age, 72 years). The most common presenting symptom was shortness of breath, which was observed in 13 (81%) of 16 patients. Known causes of tracheomalacia were found in 15 (94%) of 16 patients, prior intubation was confirmed in 12 patients, and history of asthma or chronic obstructive pulmonary disease was observed in five patients. CONCLUSION: Tracheomalacia is a relatively common incidental finding on CT pulmonary angiography studies. The central airways, as well as pulmonary vasculature, should be reviewed carefully for clinical interpretation in patients with suspected pulmonary embolism. Paired expiratory-inspiratory CT is recommended if patients present with known causes of tracheomalacia such as prior intubation, chronic obstructive pulmonary disease, or asthma.  相似文献

RATIONALE AND OBJECTIVES: The purpose of this study was to demonstrate the construction of voxelwise ventilation-perfusion (V/Q) ratio maps in a porcine model by nonrigidly aligning the respective ventilation and perfusion images using a multimodality registration algorithm. MATERIALS AND METHODS: The first-pass contrast agent technique for a blood flow map and 3He used for ventilation imaging were performed using a normal porcine model. The registered 3He-ventilation image was then aligned to the blood flow map using a multimodality registration algorithm. The voxelwise V/Q ratios were calculated by dividing the registered 3He-ventilation image by the blood flow map. The V/Q ratios were also semi-logarithmically scatter-plotted against the number of voxels. RESULTS: From perfusion magnetic resonance images, a voxel-by-voxel blood flow map was produced. Registered 3He ventilation image was successfully obtained as well as V/Q ratio map. Plots of the V/Q ratios obtained by this registration approach were similar to the logarithmic normal distribution. CONCLUSION: Registration of MR perfusion and ventilation images can potentially enable quantitative evaluation of regional pulmonary function and thus yield deeper insight into the physiology and pathophysiology of the lung.  相似文献

OBJECTIVE: We surveyed the practices and policies of the radiology departments of the Society of Thoracic Radiology members regarding the use of CT pulmonary angiography in pregnant patients suspected of having pulmonary embolism. MATERIALS AND METHODS: Surveys were mailed electronically to the 403 members of the Society of Thoracic Radiology (403 addresses). Respondents were asked to send one response from each institution or department. Information gathered included use of CT angiography in relation to ventilation-perfusion imaging in pregnant patients, written policies, informed consent procedures, and modifications of standard protocols for dose reduction. RESULTS: Fifty-seven members responded; 43 (75%) reported that they perform CT angiography in pregnant patients suspected of having pulmonary embolism. Of the 43 respondents who perform CT angiography in pregnant patients, 23 (53%) generally perform CT angiography as the initial study rather than ventilation-perfusion scanning, 26 (60%) require informed consent from the patient, seven (16%) have a written policy concerning CT angiography in pregnant patients, and 17 (40%) modify standard imaging protocols for pregnant patients. The most common modification for dose reduction is decreasing the scanning area along the z-axis. CONCLUSION: Most respondents perform CT angiography in pregnant patients suspected of having pulmonary embolism, but their policies and practices vary considerably.  相似文献

Pulmonary contrast enhanced magnetic resonance angiography (CE-MRA) is useful for the primary diagnosis of pulmonary embolism (PE). Many sites have chosen not to use CE-MRA as a first line of diagnostic tool for PE because of the speed and higher efficacy of computerized tomographic angiography (CTA). In this review, we discuss the strengths and weaknesses of CE-MRA and the appropriate imaging scenarios for the primary diagnosis of PE derived from our unique multi-institutional experience in this area. The optimal patient for this test has a low to intermediate suspicion for PE based on clinical decision rules. Patients in extremis are not candidates for this test. Younger women (< 35 years of age) and patients with iodinated contrast allergies are best served by using this modality We discuss the history of the use of this test, recent technical innovations, artifacts, direct and indirect findings for PE, ancillary findings, and the effectiveness (patient outcomes) of CE-MRA for the exclusion of PE. Current outcomes data shows that CE-MRA and NM V/Q scans are effective alternative tests to CTA for the primary diagnosis of PE.  相似文献

Cystic papillary carcinoma of the thyroid gland: a new sonographic sign   总被引:19,自引:0,他引:19  
Cystic thyroid lesions are generally considered to be benign and managed by repeated aspiration and/or biopsy. We report characteristic sonographic signs in eight cases of cystic papillary carcinomas of the thyroid gland. In all eight cases, ultrasonography (US) revealed mostly cystic lesions with solid excrescences protruding into the cyst. These nodules contained multiple punctate echogenic foci suggesting calcification. In three of these cases the initial fine needle aspiration biopsy was negative. The 'calcified nodule in cyst' sign was found to be very specific for cystic thyroid carcinoma in a review of the ultrasound findings in 115 patients with nodular thyroid lesions. Careful evaluation and/or surgery should be recommended in patients who have such characteristic sonographic findings even if the fine needle aspiration result is negative.  相似文献

We report high resolution sonographic (7.5 MHz) findings in four cases of undifferentiated carcinoma of the thyroid gland. Sonographic findings in these four cases included poorly marginated, hypoechoic masses associated with calcifications, and invasion of adjacent cervical structures. A knowledge of the sonographic features of undifferentiated carcinoma of the thyroid gland is of clinical importance, since the tumour has a grave prognosis, quite different from the relatively favourable prognosis of well differentiated thyroid carcinoma.  相似文献
