肺癌的纤支镜检查与影像学征象对照观察的临床研究

目的 联合应用影像学(胸部X线和CT)提高纤支镜对肺癌的诊断。方法选择我院纤支镜后组织病理学确诊的肺癌177例，全部病例在做纤支镜前1周内进行胸部X线或CT检查，并将影像学征象与纤支镜下表现及其病理学类型进行对照分析。结果中央型肺癌165例，周围型肺癌12例；纤支镜下主要表现有：外向型生长98例，浸润型生长63例，仅见间接征象5例，未见异常12例；组织病理学类型：鳞癌117例，腺癌29例，小细胞癌24例，其它肿瘤7例；影像学征象表现有：肺部肿块影116例，阻塞性肺炎表现为主27例，以局限性肺不张表现为主19例，片状模糊的阴影12例，两肺多发结节影3例。结论 纤支镜和影像学征象对肺癌的诊断各有特点，两者联合应用，可取长补短，提高肺癌的诊断。     

Synthesis, biodistribution and quantitative structure-activity relationship studies of new 99mTc labeled pseudo-peptide complexes.

Twelve new peptide or pseudo-peptide chelators have been synthesized in the course of our continuing investigation of 99mTc-labelled peptides for application for renal imaging agents. All compounds were characterized on the basis of, IR, 1HNMR, 13CNMR spectroscopy, as well as MS and elemental analysis. All peptides yield stable 99mTc complexes using Sn(II) reactive coupling and exhibit renal uptake. Linear regression analysis between Logarithm of renal uptake value (RU) and the parameters obtained by the ZINDO/1 method was performed. Some equations were obtained which showed that molecular polar and charge have some relationship with their renal uptake.     

小剂量FK778对大鼠移植肾慢性排斥反应的预防作用

目的探讨小剂量FK778对大鼠移植肾慢性排斥反应的预防作用。方法应用显微外科技术制作移植肾慢性排斥反应大鼠模型，将肾移植大鼠分为两组。肾移植后第16周开始治疗组大鼠接受FK778每日5mg／kg体重灌胃，对照组接受赋形剂。移植后每4周行24h尿蛋白含量测定，第24周处死大鼠，对移植肾组织行组织学、免疫组织化学及实时定量RT—PCR检测。结果治疗组大鼠蛋白尿、肾组织病理损害程度，淋巴细胞和单核／巨噬细胞浸润程度和对照组比较显著减轻，肾组织生长因子TGF-β基因的表达也减少。结论小剂量FK778能预防大鼠移植肾慢性排斥反应。移植肾组织T淋巴细胞和单核／巨噬细胞浸润减轻，TGF-β生长因子基因表达的减少，可能是其预防同种移植肾慢性排斥反应机制中的重要环节。     

经鼻垂体瘤68例切除及CT、MRI、神经导航分析

目的：探讨垂体瘤的治疗方法。方法：使用显微镜结合CT、MRI及神经导航系统行经鼻蝶手术。结果：68例病人无一人死亡，其中尿崩为7％左右，且为一过性，复发2例。结论：熟知局部解剖，利用CT、MRI及导航技术，不断提高手术技巧，有助于进一步降低经蝶垂体瘤手术并发症的发生。     

Characteristics of manifest glaucoma at early stages

Intraocular pressures and optic disc hemorrhages were studied in all cases of manifest glaucoma detected in a defined population over a decade. High intraocular pressure was found to be an unreliable signat the detection of a glaucomatous visual field defect and an unusual finding 5–10 yearsbefore. The prevalence of optic disc hemorrhages at the detection of a glaucomatous visual field defect was 33%. All but 9 of 44 persons with manifest glaucoma were found to have had at least one optic disc hemorrhage at one time or another.     

手术示教系统的细节设计

手术示教系统已是一个较为成熟的教学系统,本文将根据现时流行的几种设计方案进行分析,并对设计过程和选材中遇到的一些细节问题提出一些意见.     

多潘立酮在增强结肠运动中的作用

Objective To observe the prokinetic effect of domperidone on colon and compare with that of mosapride and cisapride.Methods ① In viva experiment:forty rats were divided into control,domperidone,mosapride,and cisapride groups with 10 in each group.Stran gauges were planted both in proximal and distal colons and colonic motor activities were recoded in conscious rats.② In vitro experiment:the prokinetics effects of dopamine or dopamine combined with domperidone on the contraction of isolated rat colon strips were recorded by tone-transducers in the thermostatic muscle bath.Results In viva experiments:① in the interdigestive period of resting,activities of rhythimic phasic contraction were recoded in colon at conscious rats,and ② it had been showed that dornperidone significantly enhanced the contraction of colon in dose-dependent manner.Compared with control group,the mean contraction amplitude of proximal colon and distal colon increased by 84.61% ± 7.26% and 76.37 % ± 8.47% in domeridone group,respectively,which was higher than those in mosapride group (50.32%±8.16% and 45.13%±7.16%,respectively),but as same as those in cisapride group (92.55% ± 8.37% and 81.27% ± 9.95%,respectively).In vitro experiments:① perfusion of dopamin (40 mg/ml) could significantly decrease the contraction of isolated colon strip by 91.56% ± 10.24% in comparison with Krebs-Ringer solution,and ① domperidone could block the relaxant effect of dopamin on the isolated colon strip.Conclusions Domperidone can significantly enhance the colonic motor activities via antidopaminergic action.The effects of domperidone are similar as eisapride and greater than mosapride.     

应用生理性成骨力值概念治疗骨折与骨不连——兼论MO现象与有效固定

治疗骨折与术后合并的骨不连,至今仍是骨创伤领域的难题之一。研究分析当前的学术思想,探索更接近骨愈合规律的理念与技术,是必然的学术趋势。1当前骨创伤理论与技术1.1AO从20世纪50年代起,AO组织提出解剖复位与骨块间坚强固定与加压的概念。50年间,由于伴随生物材料学的发展和内固定形式的进步,业已成为治疗骨折与骨不连的主要理论与方法,并取得了瞩目的成就,但同时也发现了存在并需继续探讨的若干问题。(1)偏心类的固定模式,如加压钢板,其疗效超越一般非加压性钢板。但同时也发现:①内固定期间,骨质萎缩,骨痂纹理凌乱或难觅骨痂;②钢板…     

c-myc mRNA在成釉细胞瘤中的表达

目的:研究成釉细胞瘤(AB)和牙源性角化囊肿(OKC)中c-mycmRNA的表达,探讨c-myc在AB和OKC中的发生、发展及其生物学意义。方法:使用原位杂交法检测54例AB、16例OKC和7例口腔正常黏膜(NOM)组织中c-mycmRNA的表达,并将AB按原发、复发、恶变分组,结果使用χ2检验进行统计分析。结果:AB、OKC及NOM组织中c-mycmRNA的阳性表达率分别为81.5%(44/54)、75.0%(12/16)和14.3%(1/7),3组比较有显著性差异(χ2=15.488,P<0.05)。原发组AB中c-mycmRNA的阳性表达率为71.0%,复发组为94.7%,恶变组为100.0%,伴随原发、复发、恶变,差异有显著性(χ2=16.912,P﹤0.05)。结论:c-myc表达在AB的发生、发展中有重要作用;c-mycmRNA的表达与AB的临床生物学行为有关,伴随其生物学行为变化,c-mycmRNA表达增强;提示c-myc有可能成为评价预后的有效指标。     

18例青年人肺癌的CT表现与临床分析

目的：对青年人肺癌18例的CT表现与临床分析。方法：收集我院近5年肿瘤科、呼吸内科及门诊就诊的肺癌325例,经CT检查结合临床表现进行诊断、治疗和分析。结果：18例从起病到确诊时间为2 w至半年不等,平均2个月;其中,中央型肺癌10例,周围型肺癌8例。结论：近年来青年人肺癌发生率有上升趋势、应对青年人群定期体检,争取早诊断早治疗以提高青年人肺癌治疗效果。
