Eighteen months after sustaining a stab wound to the left upper chest, a 59-year-old man presented with cyanosis and extertional dyspnea. Arterial desaturation due to a central 22 per cent right-to-left shunt was present. A selective pulmonary arteriogram demonstrated a fistula between the main pulmonary artery and the left atrium. At operation the fistula was closed. A laceration of the pulmonic valve and healed pericarditis were present. Marked symptomatic improvement followed the operation, but a murmur of pulmonic valvular regurgitation persisted. The fistula and laceration of the pulmonic valve were probably traumatic in origin.  相似文献

Myocardial infarction in young vs old male rats: pathophysiologic changes   总被引:3,自引:0,他引:3  
Young (90 days) and old (15 months) male, Sprague-Dawley rats were subjected to an acute and massive myocardial infarct by giving them two injections of a large dose of isoproterenol. The animals were autopsied at sequential time intervals to ascertain the similarities or dissimilarities in the pathophysiologic events which attend acute myocardial infarction and repair in young vs old rats. Although the signs and severity of hypotensive shock appeared to be equal, mortality was higher in the old rats, especially during the acute necrosis phase. The older rats also manifested more severe and persistent congestive heart failure, i.e., hydrothorax. Serum enzymes (CPK, SGOT, SGPT, and LDH), lipids (triglycerides, free fatty acids, and cholesterol), glucose, and BUN levels manifested a dynamic rise and fall concomitant with the induced myocardial necrosis and repair phases with distinct differences in these metabolic changes between young and old rats. Despite initially higher circulating levels of corticosterone in the old vs young rats, the older animals manifested little or no increase in circulating corticosterone levels during the acute stress of myocardial infarction. This apparent lack of adrenocortical responsiveness was accentuated by the concomitant finding of greatly hypertrophied, hemorrhagic, and lipid-depleted adrenal glands in the old rats vs a dynamic increase in circulating corticosterone levels and alterations in the weight of adrenal and thymus glands of the young rats. During the myocardial repair phase, the young rats manifested extensive endocardial fibrosis whereas the old rats displayed little or no endocardial fibrosis but copous and persistent myocardial edema and ground substance in keeping with their higher concentration of cardiac hexosamine. The pathophysiologic course of events which attends myocardial necrosis and repair is quite different in young vs old rats and may be related to the degree of responsiveness of the pituitary-adrenal axis which changes with age.  相似文献

Maximal oxygen uptake (VO2), skeletal muscle blood flow by xenon-133 washout technique and femoral vein arteriovenous oxygen difference and lactate were measured at rest and during maximal bicycle exercise in eight patients with severe congestive heart failure before and after 8 weeks of therapy with captopril. During therapy, skeletal muscle blood flow at rest increased significantly from 1.5 +/- 0.6 to 2.6 +/- 1.0 ml/100 g per min (p less than 0.05), with a concomitant decrease in the femoral arteriovenous oxygen difference from 10.0 +/- 1.7 to 8.3 +/- 1.9 ml/100 ml (p less than 0.05). Maximal VO2 increased significantly from 13.4 +/- 3.0 to 15.5 +/- 4.1 ml/kg per min (p less than 0.05). In four patients, the increase in maximal VO2 averaged 3.7 ml/kg per min (range 2.7 to 4.9), whereas in the remaining four patients, it was less than 1 ml/kg per min. Overall, peak skeletal muscle blood flow attained during exercise did not change significantly during long-term therapy with captopril (19.6 +/- 6.2 versus 27.6 +/- 14.3 ml/100 g per min, p = NS). However, the four patients with a significant increase in maximal VO2 experienced substantial increases in peak skeletal muscle blood flow and the latter changes were linearly correlated with changes in maximal VO2 (r = 0.95, p less than 0.001). Femoral arteriovenous oxygen difference at peak exercise was unchanged (12.6 +/- 2.6 versus 12.6 +/- 2.4 ml/100 ml). Thus, improvement in maximal VO2 produced by long-term therapy with captopril is associated with an increased peripheral vasodilatory response to exercise, and this improvement only occurs when the peak blood flow is augmented.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

The speed and scale of the COVID-19 pandemic has highlighted the limits of current health systems and the potential promise of non-establishment research such as “DIY” research. We consider one example of how DIY research is responding to the pandemic, discuss the challenges faced by DIY research more generally, and suggest that a “trust architecture” should be developed now to contribute to successful future DIY efforts.  相似文献

Objectives

The objective of this paper is to show how children and adolescents, living in the street in Haïti after the 2010 earthquake, use the street as a space to rebuild their identity. Using drawings of children and adolescents encountered in the context of research on resilience and the creative process among child and adolescent victims of the earthquake (ANR-10-HAIT-002 RECREAHVI), we explored the difficulties, the trauma and the creative expression of these children in the aftermath of the earthquake. These young people found themselves in a particularly vulnerable situation, whether socially, psychologically, or in terms of their safety. Their environmental envelopes were uncertain, fluctuating or damaged, so that the psychic envelopes of these street children was also put to the test. This article concerns the analysis of the drawings of street children and adolescents. We focus on the characteristics of the graphic envelopes to understand how these young street-dwellers symbolize the damage to their environmental and psychic envelopes, and how they express their needs for reconstruction.

Method

The study was conducted in Port-au-Prince, Haiti, one year after the earthquake. The methodological approach entailed a series of drawing workshops conducted with 45 children and adolescents aged 10–18 years encountered in three different institutions: a public education center and two shelters. In each establishment, the workshops were held three times a week over a period of three weeks. Participation in the workshops was voluntary and the children and adolescents were instructed to draw freely. Blank sheets and pencils were made available to them. The sessions of 1 h 30 were led by a student in psychology and a facilitator. A total of 680 drawings were collected from these workshops, highlighting various issues and themes. For this article, the analytical work on psychic and environmental envelopes concerned 270 drawings randomly selected in the three institutions. We conducted a classification according to three types of envelope (1) fixed containers/envelopes (houses or other buildings), (2) mobile containers/envelopes (means of transport) and (3) human containers (persons or groups of persons).

Results

Analysis of the drawings across the different graphic envelopes showed fragile psychic envelopes and a pervasiveness of the trauma of the earthquake. The fragilities are also linked with past trauma in these young people. The analysis also showed that external environmental envelopes that were destroyed in the earthquake (house, school, church, club, etc.) were characterized by internal disorganization, discontinuity and insecurity. The drawings show attempts to express, and a psychic organization characterized by mobility, so as to survive, and to face up to the internal and external instability.

Discussion

For these young street-dwellers, the street-environment, as it is assumed and experienced, is a representation of their psychic space and in particular the way they experience their bodies. Their situation was already fraught with trauma, well before the earthquake reactivated the trauma. Indeed, most street children have experienced other trauma or domestic abuse, and have often been either the victims or the instigators of urban violence. We observed that these young people are looking for stable support, despite their distrust of their environment. However, the group, and the symbolization of the house or home in drawings, appear as the predominant elements. We also note the importance of the secondary therapeutic effects of the drawing workshops, especially the cathartic benefits and the scope for elaboration of trauma in the group, with the support of drawing as a space for play and creativity.

Conclusion

The vulnerabilities of these young people deploy in an interweaving of family traumas, earlier experiences of psychic and social precariousness, and the collective trauma caused by the earthquake in 2010. The study shows the need to design counseling and educational facilities to develop their potential and help them build stable relationships with their environment.  相似文献
