From the very early stages of reduction in lung compliance, the thoracic organs may be forcibly displaced, be subject to attraction or be prevented from fully carrying out their normal movement. By evaluating these changes on a chest radiogram we may assess, at least broadly, the existence of reduced compliance and to some extent its degree. Given that reduction of lung compliance can constitute the initial sign of establishment of a secondary neoplasm or other pathological condition in the lung, the potential of chest radiograms becomes wider both in the diagnostic sphere and in that of pathophysiological research. The more frequent displacement of the trachea to the right is, we believe, due to the greater overall mass of the right lung, so that in homogenous reduction of the lung compliance the total attraction to the right may be greater. The rising of the hemidiaphragms usually starts in their frontal region. This, we consider, is due to the different resistance to the reduction in lung compliance of the adjacent areas of the lung resulting from their different structure. The reduction of compliance in segments or subsegments adjacent to the hemidiaphragms produces local rising of them. When these segments are adjacent to the osseous thoracic wall they cause pleural thickening by activating the suction mechanism. The reduction in lung compliance may cause S-shaped distortion of the bronchovascular bundles of the lower lobes, displacement of the hili, blunting of the angle of the carina, curvature of the main bronchi and of the intrathoracic part of the trachea. All these changes can be seen on a chest radiogram.  相似文献

BACKGROUND/AIMS: Despite the fact that Helicobacter pylori (Hp) is regarded as a major gastroduodenal pathogen, it has recently been suggested to be an important factor for non-gastroenterologic conditions such as diabetes mellitus. Accordingly, it seems that Hp infection may have implications in glycemic control and in fasting plasma glucose concentrations. As overnutrition and obesity are directly related to impaired glucose tolerance, the aim of the present study was to determine whether Hp infection leads to alterations in fasting plasma glucose concentrations of Hp carriers and especially in relation to their body mass index. METHODS: Serum was obtained from 224 young, male navy recruits. An enzyme-linked immunosorbent assay to detect Hp-specific IgG serum antibodies as well as gastroscopy along with biopsy was used to identify the infected individuals. Serum levels of glucose, urea, creatinine and uric acid were also determined. Non-fasting subjects and persons with abnormal oral glucose tolerance curve test were excluded. RESULTS: Among Hp-positive individuals, obese persons presented with a significantly lower mean blood glucose level than non-obese persons. Obese Hp-contaminated participants had significantly lower mean fasting blood glucose concentrations as well as a significantly smaller percentage of participants with abnormal elevated blood glucose levels than obese participants negative to Hp infection. CONCLUSIONS: Our data suggest that obesity in combination with Hp infection may induce an enhanced response to insulin leading to reduced fasting blood glucose levels, among Hp-positive obese persons in comparison to Hp-positive lean persons.  相似文献

We report the results of Ilizarov gradual distraction to regain 6.7 cm of leg length in a severely contracted hip 11 months after a resection arthroplasty. Restoration of leg length allowed revision hip arthroplasty. At 24 months after the revision arthroplasty, the patient is ambulating independently and pain free. The use of Ilizarov gradual distraction restored leg length and facilitated postoperative function.  相似文献

Shipyards are industrial areas where workers are likely exposed to environmental pollutants such as welding fumes, fine organic solvent and dye dust, that render the occupational environment a high risk one. Assessing the risk that workers are exposed to is a high critical factor in improving their working conditions. The present study aims to investigate the potential genetic damage to workers exposed to a harsh environment in a Greek shipyard. It is focused on assessing the percentage of induced micronuclei, as well as on changes in the various cell types of shipyard workers’ oral mucosa epithelium by implementing the buccal micronucleus cytome assay. Exposed workers appeared with statistically significant induced micronuclei as compared to office employees. Statistically, significant cell lesions were detected and are related to workers’ exposure to environmental conditions. The workers’ smoking habit contributed as well to the observed buccal epithelial cell alterations. The observed data signify the high-risk workers are exposed; resulting in the shipyard’s management the need to implement measures improving the working environment conditions and to reevaluate the workers’ personal protective equipment requirements.  相似文献

The present study was designed to determine the efficacy of a novel multicomponent mycotoxin detoxifying agent (MMDA) containing modified zeolite (Clinoptilolite), Bacillus subtilis, B. licheniformis, Saccharomyces cerevisiae cell walls and silymarin against the deleterious effects of Aflatoxin B1 (AFB1) and Ochratoxin A (OTA) in broiler chicks. A total of 160 one-day-old Ross 308® broiler chicks were randomly allocated in four treatment groups, with four replicates, according to the following experimental design for 42 days. Group A received a basal diet; Group B received a basal diet contaminated with AFB1 and OTA at 0.1 mg/kg and 1 mg/kg, respectively; Group C received a basal diet contaminated with AFB1 and OTA and MMDA at 1 g/kg feed, and Group D received a basal diet contaminated with AFB1 and OTA and MMDA at 3 g/kg feed. Results showed that ingested mycotoxins led to significant (p ≤ 0.05) reduction in body weight and feed conversion from 25 days of age, induced histopathological changes, increased the pH of the intestinal content, and altered the biochemical profile of birds with significantly (p ≤ 0.05) increased aspartate aminotransferase (AST) values (p ≤ 0.05). On the other hand, the supplementation of MMDA significantly (p ≤ 0.05) improved the feed conversion ratio (FCR) during the second part of the study, diminished biochemical alterations, reduced pH in jejunal and ileal content, and E. coli counts in the caeca of birds (p ≤ 0.05). It may be concluded that the dietary supplementation of the MMDA partially ameliorated the adverse effects of AFB1 and OTA in broilers and could be an efficient tool in a mycotoxin control program.  相似文献
