Purpose

About one-third of individuals with focal epilepsy continue to have seizures despite optimal medical management. These patients are potentially curable with neurosurgery if the epileptogenic zone (EZ) can be identified and resected. Stereo-electroencephalography (SEEG) to record epileptic activity with intracranial depth electrodes may be required to identify the EZ. Each SEEG electrode trajectory, the path between the entry on the skull and the cerebral target, must be planned carefully to avoid trauma to blood vessels and conflicts between electrodes. In current clinical practice trajectories are determined manually, typically taking 2–3 h per patient (15 min per electrode). Manual planning (MP) aims to achieve an implantation plan with good coverage of the putative EZ, an optimal spatial resolution, and 3D distribution of electrodes. Computer-assisted planning tools can reduce planning time by quantifying trajectory suitability.

Methods

We present an automated multiple trajectory planning (MTP) algorithm to compute implantation plans. MTP uses dynamic programming to determine a set of plans. From this set a depth-first search algorithm finds a suitable plan. We compared our MTP algorithm to (a) MP and (b) an automated single trajectory planning (STP) algorithm on 18 patient plans containing 165 electrodes.

Results

MTP changed all 165 trajectories compared to MP. Changes resulted in lower risk (122), increased grey matter sampling (99), shorter length (92), and surgically preferred entry angles (113). MTP changed 42 % (69/165) trajectories compared to STP. Every plan had between 1 to 8 (median 3.5) trajectories changed to resolve electrode conflicts, resulting in surgically preferred plans.

Conclusion

MTP is computationally efficient, determining implantation plans containing 7–12 electrodes within 1 min, compared to 2–3 h for MP.

Objective

To review and analyze periodontal clinical parameters after bariatric surgery.

Background

Periodontitis, a dysbiotic inflammatory disease, has been associated with obesity. The purpose of bariatric surgery is to reduce weight and systemic inflammation. Consequently, it is of interest to systematically review the impact of bariatric surgery on periodontal status.

Methods

Electronic searches were conducted in MEDLINE, EMBASE, Thesis database, and the Cochrane Library databases. Gray literature and the main journals of both specialties were also reviewed. Only cross-sectional and prospective studies focusing on bleeding on probing, pocket depth, and clinical attachment levels were selected.

Results

After a thorough screening of 651 studies, 10 studies were selected by 2 independent reviewers. Four (n?=?250) and 3 studies (n?=?191) were included in the meta-analysis at 6- and 12-month follow-up, respectively. At 6-month follow-up increased periodontal inflammation (P?=?.03) and periodontal destruction were observed. However, 12 months after baseline, the difference between bariatric patients and control was no longer significant.

Conclusion

The present systematic review and meta-analysis suggests that deterioration of periodontal status may be observed in the first 6 months after surgery. Consequently, periodontal screening and management of the patient's request for bariatric surgery should be recommended to avoid further deterioration of periodontal status after bariatric surgery.  相似文献

Infants born prematurely are at increased risk of adverse neurodevelopmental outcome. The measurement of white matter tissue composition and structure can help predict functional performance. Specifically, measurements of myelination and indicators of myelination status in the preterm brain could be predictive of later neurological outcome. Quantitative imaging of myelin could thus serve to develop biomarkers for prognosis or therapeutic intervention; however, accurate estimation of myelin content is difficult. This work combines diffusion MRI and multi‐component T2 relaxation measurements in a group of 37 infants born very preterm and scanned between 27 and 58 weeks equivalent gestational age. Seven infants have longitudinal data at two time points that we analyze in detail. Our aim is to show that measurement of the myelin water fraction is achievable using widely available pulse sequences and state‐of‐the‐art algorithmic modeling of the MR imaging procedure and that a multi‐component fitting routine to multi‐shell diffusion weighted data can show differences in neurite density and local spatial arrangement in grey and white matter. Inference on the myelin water fraction allows us to demonstrate that the change in diffusion properties of the preterm thalamus is not solely due to myelination (that increase in myelin content accounts for about a third of the observed changes) whilst the decrease in the posterior white matter T2 has no significant component that is due to myelin water content. This work applies multi‐modal advanced quantitative neuroimaging to investigate changing tissue properties in the longitudinal setting. Purpose

Prone position (PP) improves oxygenation and outcome of acute respiratory distress syndrome (ARDS) patients with a PaO2/FiO2 ratio <150 mmHg. Regional changes in lung aeration can be assessed by lung ultrasound (LUS). Our aim was to predict the magnitude of oxygenation response after PP using bedside LUS.

Methods

We conducted a prospective multicenter study that included adult patients with severe and moderate ARDS. LUS data were collected at four time points: 1 h before (baseline) and 1 h after turning the patient to PP, 1 h before and 1 h after turning the patient back to the supine position. Regional lung aeration changes and ultrasound reaeration scores were assessed at each time. Overdistension was not assessed.

Results

Fifty-one patients were included. Oxygenation response after PP was not correlated with a specific LUS pattern. The patients with focal and non-focal ARDS showed no difference in global reaeration score. With regard to the entire PP session, the patients with non-focal ARDS had an improved aeration gain in the anterior areas. Oxygenation response was not associated with aeration changes. No difference in PaCO2 change was found according to oxygenation response or lung morphology.

Conclusions

In ARDS patients with a PaO2/FiO2 ratio ≤150 mmHg, bedside LUS cannot predict oxygenation response after the first PP session. At the bedside, LUS enables monitoring of aeration changes during PP.
