Early radiation survey of Iitate village, which was heavily contaminated by the Fukushima Daiichi accident, conducted on 28 and 29 March 2011

Following the news that the radiation level in Iitate Village, located 25-45 km from the Fukushima Daiichi Nuclear Power Plant, was seriously increased, an urgent field survey was carried out on 28 and 29 March 2011. Radiation levels at 130 locations were measured inside a van that traveled throughout the village using a CsI pocket survey meter and an ionization chamber. Soil samples were also taken at five locations and submitted to gamma ray analysis using a Ge detector. A radiation exposure rate of more than 20 μSv h was observed in the southern part of Iitate Village. Volatile radionuclides such as iodine and cesium were found to be the main components of radioactive contamination. A trace amount of plutonium isotopes originating from the accident was also confirmed in several soil samples, the level of which was less than the global fallout. Based on the measured density of radionuclides at the highest contamination location during the present survey, an exposure rate of about 200 μGy h at 1 m above the ground was estimated at the time of the radioactive deposition on March 15. At this location, the cumulative exposure would reach 50 mGy in the middle of May 2011.     

Utility of three-dimensional transthoracic echocardiography in diagnosis of intermediate atrioventricular septal defect: report of an adult case

We present the case of a 54-year-old male with an intermediate atrioventricular septal defect (AVSD). He was referred to our institute for treatment of shortness of breath. Two-dimensional transthoracic echocardiography demonstrated an ostium primum defect of the atrial septum, a common atrioventricular valve with mild regurgitations, and an interventricular communication, which are the features of complete AVSD. However, three-dimensional transthoracic echocardiography (3D-TTE) could clearly visualize the tricuspid pouch (i.e., persistence of the tissue in the endocardial cushion), a feature of partial AVSD. 3D-TTE appears to be a useful modality for diagnosis of intermediate AVSD.     

Survivor of cardiogenic shock following acute myocardial infarction with Churg-Strauss syndrome: first angiographic documention of coronary recanalization of infarct-related arteries: a case report

A 42-year-old man was treated under a diagnosis of Churg-Strauss syndrome with predonisolone pulse therapy. Three days later, he developed cardiogenic shock following acute myocardial infarction. Coronary angiography showed total occlusions in three peripheral coronary vessels. Intraaortic balloon pumping was used to maintain hemodynamics and predonisolone pulse therapy was repeated. However, he developed cardiogenic shock again after the second pulse therapy and needed percutaneous cardiopulmonary support and intraaortic balloon pumping. Accordingly, combination therapy of predonisolone and cyclophosphamide was given. He then recovered. Follow-up angiography showed recanalization of the infarct-related arteries.     

Senile systemic amyloidosis presenting as bilateral carpal tunnel syndrome.

A 74-year-old woman who had developed numbness in both hands was diagnosed as having bilateral carpal and right cubital tunnel syndrome and underwent bilateral carpal and right cubital tunnel release. Transthyretin immunoreactive amyloid deposits were seen on specimens and were also detected in gastric, duodenal and ileal mucosal biopsies. The transthyretin gene analysis showed no mutation. This is a rare case of senile systemic amyloidosis presenting as carpal tunnel syndrome.
