Intraductal shock-wave lithotripsy in complicated common bile duct stones   总被引:1,自引:0,他引:1  
Summary Intracorporeal shockwave lithotripsy was performed in 36 patients with problematic common bile duct stones. All of the patients had undergone unsuccessful mechanical lithotripsy prior to this procedure. In 29 patients (80.6%), the stones were fragmented under cholangioscopic control and subsequently extracted with a Dormia basket. In seven patients, the procedure failed due to stone impaction or failure to intubate the common bile duct with a nasobiliary tube. No complications were observed. Cholangioscopically guided intracorporeal shockwave lithotripsy is a highly effective and safe procedure for the conservative treatment of complicated common bile duct stones.Abbreviations ESWL extracorporeal shock-wave lithotripsy - ISWL intracorporeal shock-wave lithotripsy Dedicated to Prof. Dr. med. The baseline EEG sleep patterns of 10 young depressed patients, 20 patients with anorexia nervosa, 10 patients with bulimia nervosa, and 10 healthy subjects were found to be indistinguishable, except for an increased REM density in the depressed patients. In eating disorder patients, a concomitant major depressive episode had no influence on EEG sleep. The results of the cholinergic REM sleep induction test revealed a significantly faster induction of REM sleep in the depressed patients when compared with the eating disorder patients and the control subjects. This indicates a subthreshold hypersensitivity of the REM sleep triggering cholinergic transmitter system in depressives, but not in eating disorder patients.  相似文献

Insomnia is the most frequent sleep disorder and a public health concern that increased during the Covid 19 pandemic. Fully restrictive lockdowns during Covid are interesting periods to examine the impact of environmental and behavioural changes on the emergence of insomnia symptoms. In this cross-sectional study we aimed to (1) determine the main factors associated with insomnia symptoms during a Covid-19 fully restrictive lockdown examining the associated daily life alterations and (2) create a predictive model of insomnia symptoms. We used the data drawn from the “Covid-RythmE” study that reached volunteers from the general French population through an online survey during the last 2 weeks of the 2 month full lockdown. Associations with insomnia symptoms were tested and significant associations were entered in a Backward Stepwise Logistic Regression (BSLR) to assess the best combination to classify individuals with or without insomnia symptoms. From the 1624 participants, 50.64% suffered from mild to severe insomnia symptoms as assessed by the ISI. The best combination for explaining insomnia symptoms with 74.26% of accuracy included: age (OR = 1.15), females (OR = 1.26), smaller home sizes (OR = 0.77), environmental noises (OR = 1.59), anxiety symptoms (OR = 1.24), depressive symptoms (OR = 1.15), regularity of sleep–wake schedules (OR = 1.25), exposure to screen during the morning (OR = 1.13), and LED light during the evening (OR = 1.17). Thus, lifestyle schedule and exposure to natural synchronizers such as light, are primordial in considering in insomnia physiopathology, prevention and treatment, as well as the associated mental health status.  相似文献

目的为临床肺移植积累经验。方法分析总结29例犬左肺移植的资料。其中15例的供体肺用Euro-Collings液在4℃下保存6h,14例用UW液保存22~25h,作同种异体移植,并阻断受体的右肺动脉,以测定移植左肺的功能。结果所有的移植左肺均能胜任气体交换,循环稳定。结击本左肺移植方法可靠,可供临床应用时参考。  相似文献

This paper reports on the incidence and level of man-made electrical impulse noise capable of causing interference to low-power, ASK and FSK radio biotelemetry links. Measurements were made at two acute hospitals, one in a residential area of Belfast, Northern Ireland and the other in an industrial part of Berlin, Germany. Monitoring was effected in the 450 MHz band using a communications receiver with its AGC line disabled and AM-detector AM output fed to a PC-based logger. The RF bandwidth was 20 kHz and a rotatable folded-dipole receiving antenna was used. Seven-day impulse counts were recorded at each location, for separate RF input thresholds of -100, -110 and -120 dBm and a common trigger window of 10 micros-10 ms. Histograms depicting the average pulse count per minute are presented; values range from a maximum of 55x10(3)/min for -120 dBm sensitivity in Berlin, to a minimum of 1.2/min for -100 dBm sensitivity in Belfast, both with vertical polarisation.  相似文献

Nitric oxide (NO) influences electrophysiological and morphological parameters of the mammalian cochlea. Recently, the isoform of the NO-producing enzyme neuronal NO synthase (nNOS) has been demonstrated in spiral ganglion cells and olivocochlear neurons. The cochlea is also innervated by fibers stemming from the trigeminal ganglion (TG) and superior cervical ganglion (SCG). Whether these ganglion cells contain nNOS is not known yet. We therefore identified TG and SCG cells upon injection of Fluoro-Gold (FG) into the cochlea and retrograde neuronal transport of FG in rat and guinea pig. These ganglion cells were investigated for neuronal NOS immunohistochemically. Perikarya labeled by FG were found in the ipsilateral TG and SCG. In both species investigated, a considerable number of FG-labeled TG cells were also nNOS-immunoreactive whereas SCG cells were not. These data, demonstrating the existence of nNOS-containing TG cells that project to the cochlea, provide evidence that these neurons are further sources of nitric oxide in the cochlea.  相似文献
