The value of visual field testing in the era of advanced imaging: clinical and psychophysical perspectives

White‐on‐white standard automated perimetry (SAP) is widely used in clinical and research settings for assessment of contrast sensitivity using incremental light stimuli across the visual field. It is one of the main functional measures of the effect of disease upon the visual system. SAP has evolved over the last 40 years to become an indispensable tool for comprehensive assessment of visual function. In modern clinical practice, a range of objective measurements of ocular structure, such as optical coherence tomography, have also become invaluable additions to the arsenal of the ophthalmic examination. Although structure‐function correlation is a highly desirable determinant of an unambiguous clinical picture for a patient, in practice, clinicians are often faced with discordance of structural and functional results, which presents them with a challenge. The construction principles behind the development of SAP are used to discuss the interpretation of visual fields, as well as the problem of structure‐function discordance. Through illustrative clinical examples, we provide useful insights to assist clinicians in combining a range of clinical results obtained from SAP and from advanced imaging techniques into a coherent picture that can help direct clinical management.     

The effect of MMF dose and trough levels on adverse effects in pediatric heart transplant recipients

Limited pharmacokinetic and safety data exist for MMF in pediatric HTR. Previously targeted MPA‐TL are 1.5–3.0 μg/mL. The objective of this study was to assess the outcomes targeting MPA‐TL of 0.8–2.0 μg/mL in pediatric HTR. MPA‐TL were retrospectively collected 2–12 months post‐transplant. Acute rejection, infection, leukopenia, and GI complaints were then correlated with MPA‐TL. A total of 355 MPA‐TL from 22 HTR were included. Median age was 2.5 yr. Primary indication for transplant was dilated cardiomyopathy (64%). Mean MPA‐TL was 1.7 ± 0.9 μg/mL. African American patients received significantly higher doses (702 ± 235 mg/m²) compared with other races (p = 0.035). Leukopenia was less common in patients with SUB MPA vs. others (p = 0.01). MMF was discontinued for GI complaints in one patient and leukopenia in two patients. One SUB patient had acute rejection, and one SUP patient had infection. One‐yr survival was 100%. Targeting a lower range for MPA‐TL was not associated with significant rejection or infection. Despite lower MPA‐TL, MMF was discontinued in 3/22 patients for adverse effects.     

Cornelia de Lange syndrome

Cornelia de Lange syndrome (CdLS) is a rarely seen multisystem developmental disorder syndrome characterized by facial dysmorphia (arched eyebrows, synophrys, depressed nasal bridge, long philtrum, down-turned angles of the mouth), upper-extremity malformations, hirsutism, cardiac defects, growth and cognitive retardation, and gastrointestinal abnormalities. We present here a case of newborn male who presented with the complaints of feed regurgitation, choking and cyanosis. There was a distinct facial dysmorphism with arched and bushy eyebrows, long philtrum, thin upper lip, depressed nasal bridge and hirsutism. The patient was diagnosed as having Cornelia de Lange syndrome on the recognition of distinctive facial features in addition to the pre- and postnatal growth retardation, feeding problems and physical malformations including limb defects.

Costen syndroma or temporo-mandibular joint disorders is not welknow in the ENT service, despite the common presentation with otalgia. We therefore present 60 cases diagnose in our hospital and present clinical and therapeutic aspect.
