Tuition fees for medical school are continuously and riotously increasing. This upsurge is amassing debts on the backs of students. In the class of 2018, 75% finished medical school with an outstanding balance of $196,520, on average—a $5826 increase from 2017. Tuition fees differ in terms of the ownership of the medical school (public vs. private) and according to the medical student residence status (in-state or out-of-state). It is critical that students arrange a long-term budget that shows them where they stand: in surplus or in deficit. Students may classify expenditures into two groups: “fixed” and “variable,” where they can manipulate the variable expenses to fit into their budget. To pay for their tuition, medical students have four possibilities: cash, scholarships and grants, service-obligation scholarships, and loans. Loans are the most common alternatives, and so there are Traditional Repayment Plans and Income-Driven Repayment Plans. This article serves to provide medical students with attainable alternatives for funding their education and for repaying their debts.  相似文献

Acute myeloid leukaemia (AML) with t(6;9)(p23;q34) is a poor-risk entity, commonly associated with FLT3-ITD (internal tandem duplication). Allogeneic stem-cell tranplantation (allo-SCT) is recommended, although studies analysing the outcome of allo-SCT in this setting are lacking. We selected 195 patients with t(6;9) AML, who received a first allo-SCT between 2000 and 2016 from the EBMT (European Society for Blood and Marrow Transplantation) registry. Disease status at time of allo-SCT was the strongest independent prognostic factor, with a two-year leukaemia-free survival and relapse incidence of 57% and 19% in patients in CR1 (first complete remission), 34% and 33% in CR2 (second complete remission), and 24% and 49% in patients not in remission, respectively (P < 0·001). This study, which represents the largest one available in t(6;9) AML, supports the recommendation to submit these patients to allo-SCT in CR1.  相似文献

Canine breast cancer was considered as an ideal model of comparative oncology for the human breast cancer, as there is significant overlap between biological and clinical characteristics of the human and canine breast cancer. We attempt to clarify expression profile of the embryonic stem cell (ES) gene signatures in canine breast cancer. Using microarray datasets (GSE22516 and GSE20718), expression of the three major ES gene signatures (modules or gene‐sets), including Myc, ESC‐like, and PRC modules, was primarily analyzed through Gene‐Set Enrichment Analysis (GSEA) method in tumor and healthy datasets. For confirmation of the primary results, an additional 13 ES gene‐sets which were categorized into four groups including ES expressed (ES exp1 and ES exp2), NOS targets (Nanog targets, Oct4 targets, Sox2 targets, NOS targets, and NOS TFs), Polycomb targets (Suz12 targets, Eed targets, H3K27 bound, and PRC2 targets), and Myc targets (Myc targets1, and Myc targets2) were tested in the tumor and healthy datasets. Our results revealed that there is a valuable overlap between canine and human breast cancer ES gene‐sets expression profile, where Myc and ESC‐like modules were up‐regulated and PRC module was down‐regulated in metastatic canine mammary gland tumors. Further analysis of the secondary gene‐sets indicated overexpression of the ES expressed, NOS targets (Nanog targets, Oct4 targets, Sox2 targets, and NOS targets), and Myc targets and underexpression of the Polycomb targets in metastatic canine breast cancer.  相似文献

Upon demand, 1,3‐diamino‐propane‐triethylborane (DAPTB) is photolyzed in the presence of an acid generator such as a diphenyliodonium salt. Interestingly, triethylborane is released. In the presence of oxygen, ethyl radicals are generated and initiate an acrylate polymerization. This is the first example of a phototriggered production of a triethylborane usable in the photoinitiation of acrylate polymerizations. Abstract: There is concern that the use of topical corticosteroids in patients with eczema herpeticum may facilitate dissemination of herpes simplex virus and worsen disease. Our primary aim therefore was to determine whether topical corticosteroid use in children hospitalized with eczema herpeticum is associated with longer hospital length of stay (LOS). We performed a multicenter retrospective cohort study of 1,331 children ages 2 months to 17 years admitted with a diagnosis of eczema herpeticum between January 1, 2001, and March 31, 2010, to 42 tertiary care children’s hospitals in the Pediatric Health Information System database. Multivariable linear regression models determined the association between receipt of topical corticosteroid therapy on the first day of hospitalization and the main outcome measure: LOS. Receipt of topical corticosteroid therapy on day 1 of hospitalization was not associated with a longer LOS on unadjusted or multivariable analysis (p = 0.75). Receipt of topical calcineurin inhibitors during the hospitalization was also not associated with a longer LOS (p = 0.12). Receipt of systemic corticosteroids was associated with an 18% adjusted longer LOS (95% confidence interval 2%–36%; p = 0.03). Further study is needed to identify which children with eczema herpeticum may benefit from topical corticosteroids, but their use during active infection is not associated with poorer outcomes, although the use of systemic corticosteroids was associated with a longer LOS and should be avoided in patients with eczema herpeticum pending future prospective study.  相似文献

Polysaccharides isolated from mushrooms have recently attracted attention due to its potential immune-stimulatory activity. The aim of this study was to validate the in vitro immune-stimulatory activities of various mushroom extracts. The 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay revealed that Pleurotus eryngii, with the highest β-glucan (18.94%) content, displayed highest viability on macrophage cells of 62.59% at 200?μg/ml concentration. Pleurotus cystidiosus, with 18.16% β-glucan, content showed highest activation of NF-kB (0.7?µg/ml) at a concentration of 100?µg/ml. Termitomyces heimii, with the lowest percentage of β-glucan (0.51%), exhibited highest phagocytosis index of 9.38 at 12.5?µg/ml. The brown strain of Agaricus bisporus with 1.54% of β-glucan stimulates the highest nitric oxide (NO) production of 12.39?µM nitrite oxide at 100?µg/ml. This study revealed that hot water extracts of mushrooms have different β-glucan contents and produced varying immune-stimulatory activities. Among these, Pleurotus spp. demonstrated the highest percentage of β-glucan content and viability of macrophage cells. Pleurotus spp. are deemed immune-stimulatory by increasing phagocytic activity, NO production, and triggered the activation of NF-kB.  相似文献
