Development of femoral artery pseudoaneurysm represents a continuing problem after percutaneous peripheral interventional procedures as well as coronary angioplasty. We report a case of symptomatic, expanding femoral artery pseudoaneurysm in a 60-year-old man who underwent percutaneous transluminal coronary angioplasty and stenting for acute myocardial infarction. A self-expanding Wallgraft Endoprosthesis (Boston Scientific, USA) was delivered under fluoroscopic guidance via contralateral percutaneous femoral approach to the site, resulting in immediate complete exclusion of the pseudoaneurysms. Follow-up color duplex scanning confirmed false aneurysm exclusion 1 year postprocedure. Endovascular treatment of iatrogenic pseudoaneurysm appears to be an attractive alternative to surgical repair in critically ill patients, with a high degree of technical success, low morbidity and short hospital stay.  相似文献

Cyclosporine absorption profiling uses either the area under the concentration curve in the first 4 h post dose, AUC(0–4), or the concentration 2 h post dose (C2) to optimize immunosuppression in adult kidney and liver transplantation. We characterized C2 versus AUC(0–4) relationships over time after transplant and across transplant indications in 56 pediatric transplant patients. There were 36 kidney transplant patients aged 9.7±3.9 years. Nineteen of these patients were studied in the de novo period on day 7 post transplant and 17 in the maintenance phase more than 1 year post transplant. In addition, 20 liver transplant patients aged 8.9±4.2 years were studied in the maintenance phase. All patients had five blood samples collected over the 12-h dose interval that were analyzed by validated assay methods at a central laboratory. Pediatric C2 values were 1,463±658 ng/ml for de novo kidney, 954±322 ng/ml for maintenance kidney, and 619±339 ng/ml for maintenance liver transplant patients. C2 was a strong predictor of AUC(0–4) in all three pediatric groups, with coefficients of determination (r 2) ranging from 0.861 to 0.936. Although data were limited from the de novo period, the C2 versus AUC(0–4) regression was consistent over time after transplant and between transplant indications, with a regression slope of 2.50 in de novo kidney, 2.54 in maintenance kidney, and 2.76 in maintenance liver transplant recipients. These slopes were also comparable to that in adult maintenance kidney transplant patients (2.60). In conclusion, C2 versus AUC(0–4) relationships demonstrated consistency over time (de novo vs. maintenance phase), between transplant indications (kidney vs. liver), and across age groups (pediatric vs. adult patients). Average C2 values achieved with current pediatric cyclosporine dosing practices cluster around the target C2 ranges recommended for adults.  相似文献

Actinobacillus actinomycetemcomitans has an important aetiological role in localized juvenile periodontitis and in progressive periodontitis in adults. A. actinomycetemcomitans is found mainly in periodontal pockets but also in whole saliva, a potential transmission medium. It is sensitive to peroxidase-halide systems, but the differences between periodontitis associated clinical isolates and type strains are unclear. The sensitivities of these 2 strain groups to lactoperoxidase (LP)-iodide (I(-))-hydrogen peroxide (H(2)O(2)) combinations were investigated, and the sensitivities were compared with the susceptibilities to four antibiotics. There was great variation between the sensitivities of different strains, but the 2 strain groups responded similarly. The LP (75 microg)-I(-) (100 nmol)-H(2)O(2) (1000 nmol) combination produced a similar degree of inhibition as 2 microg ampicillin. The LP-I(-) system might be a potential antimicrobial agent against A. actinomycetemcomitans transmission via saliva.  相似文献

With progressively brighter stimuli, the amplitude of the photopic b-wave first increases, briefly saturates and then decreases gradually to reach a plateau, where the amplitude of the b-wave equals that of the a-wave; a phenomenon previously presented as the photopic hill. The unique presentation of this luminance-response function seriously complicates its analysis with curve fitting equations such as that of Naka-Rushton used for scotopic electroretinogram. We report a method of analysis of the photopic hill based on easily identifiable and reproducible features of the ascending and descending limbs of this function. The clinical usefulness of these parameters is illustrated with selected cases of retinal disorders.  相似文献

PURPOSE: Studies show significant variability in the thickness of laser in situ keratomileusis (LASIK) corneal flaps cut by various microkeratomes. Most studies of corneal flap thickness are based on contact ultrasonic pachymetry measurements taken during the surgical procedure. This study reports a technique to obtain reproducible corneal flap thickness and residual stromal bed thickness measurements using noncontact optical coherence tomography (OCT) following LASIK. METHODS: The corneal flap thicknesses of 26 eyes of 15 patients were measured following LASIK in which the flap was created using the Amadeus microkeratome: 160-microm head, 9.5-mm ring, 4.0-mm/s translation speed, 8000 oscillations/m, and full vacuum. Zeiss Humphrey OCT-2 line scans were performed on postoperative days 1 and 7. The raw data from three scans for each eye and day were exported to Microsoft Excel for processing, averaging, and analysis. RESULTS: The OCT corneal flap thickness and residual stromal bed thickness measurements correlated well with ultrasonic pachymetry measurements performed during surgery (R2 = .92). The OCT technique yielded reproducible results, as the variance for repeated scans was only 2.5% of the variance between eyes. In bilateral cases a single blade was used for both eyes. The mean flap thickness of 15 first eyes was significantly greater than that of the 10 second eyes: 181 +/- 31 microm vs. 143 +/- 41 microm (P < .01). A positive correlation was found between the preoperative pachymetry and corneal flap thickness. CONCLUSIONS: The OCT scan averaging technique is a reproducible, noncontact postoperative method for measuring corneal flap and residual stromal bed thicknesses following LASIK.  相似文献

Hepatocytes carry out many vital biological functions, such as synthetic and catabolic reactions, detoxification and excretion. Due to their ability to restore a tissue-like environment, hollow-fibre bioreactors (HFBs) show great potential among the different systems used to culture hepatocytes. Several designs of HFBs have been proposed in which hepatocytes or hepatocyte-derived cell lines can be cultured in suspensions or on a solid support. Currently the major use of hepatocyte HFBs is as bioartificial livers to sustain patients suffering from acute liver failure, but they can also be used to synthesize cell products and as cellular models for drug metabolism and transport studies. Here, we present an overview of the set-up of hepatocyte HFBs and aim to provide potential users with the basic knowledge necessary to develop their own system. First, general information on HFBs is given, including basic principles, transport phenomena, designs and cell culture conditions. The importance of the tests necessary to assess the performance of the HFBs, i.e. the viability and functionality of hepatocytes, is underlined. Special attention is paid to drug metabolism studies and to adequate analytical methods. Finally, the potential uses of hepatocyte HFBs are described.  相似文献

This study examined the effect of intrathecal (i.t.) injection of the endocannabinoid anandamide in urethane-anesthetized rats. The tip of the i.t. cannula was positioned at the T12–L1 level of the spinal cord. Either anandamide or its metabolically stable analogue methanandamide (25 to 100 nmol) produced dose-dependent decreases in the blood pressure that persisted at least for up to 30 min. The hypotensive responses to 100 nmol anandamide and to 100 nmol methanandamide were –17.7±1.6 mmHg (n=5) and –17.9±2.0 mmHg (n=4), respectively. Hypotensive effects were also obtained with the CB1 cannabinoid receptor agonist WIN 55212-2 (20 nmol; i.t.) as well as with the vanilloid VR1 receptor agonist capsaicin (3 nmol; i.t.). Nicotinic ganglionic blockade with hexamethonium bromide [10 mg/kg; intravenous(i.v.)] abolished the responses to both anandamide and capsaicin. The i.t. administration of the CB1 receptor antagonist, 20 nmol SR 141716A, as well as the VR1 receptor antagonist, 20 nmol capsazepine, prevented almost completely the hypotensive responses to both anandamide and methanandamide. SR 141716A prevented the hypotension caused by WIN 55212-2 but did not modify the response to the vanilloid receptor agonist capsaicin. On the contrary, capsazepine antagonized the hypotension caused by capsaicin but failed to affect the decrease in blood pressure caused by the CB1 cannabinoid receptor agonist WIN 55212-2. These results suggest that anandamide could modulate the blood pressure through the activation of cannabinoid CB1 receptors and vanilloid VR1 receptors localized at the spinal cord.  相似文献
