Methods of preoperative radiologic localization of insulinoma were compared in 52 patients, 44 of whom had solitary tumors. Examinations performed in these 44 patients were preoperative ultrasonography (US) in 28, angiography in 26, and computed tomography in 23. Prospective sensitivities were 61%, 54%, and 30%, respectively. Imaging sensitivities were lower for the eight patients with multiple insulinomas. In 28 of the 44 patients, intraoperative US was performed without the examiner being aware of the surgical findings. The sensitivity was 84%. Four insulinomas were not palpable but were visualized sonographically. The combined sensitivity of intraoperative US and surgical palpation for detecting solitary insulinomas was 100%. High-frequency intraoperative US is valuable for detecting occult solitary insulinomas and considerably useful for determining the proximity of insulinomas to the pancreatic and bile ducts. 相似文献

Four hundred fifty consecutive patients with dysphagia were evaluated radiologically over a 14-month period; 127 of these (28.2%) were also examined endoscopically. The most common abnormality seen was dysmotility (34%), followed by hiatal hernia, benign stricture, and esophagitis. Correlation with endoscopy was generally good. Radiologic study demonstrated all cases of esophageal malignancy; radiologic/endoscopic correlation was also strong in patients with moderate or severe esophagitis, though the radiologist had some difficulty detecting mild inflammation. Endoscopy failed to demonstrate some benign strictures. Radiologic study was relatively accurate in detecting significant organic disease; most motility disorders were not detected by endoscopy. For these reasons, as well as lower cost, increased convenience, and patient comfort, radiologic assessment is recommended as the primary method of evaluating patients with dysphagia. 相似文献

Forty-five patients undergoing double-contrast barium enema examinations were studied to identify the incidence of transient bacteremia associated with the examination. Blood cultures were obtained immediately prior to the examination and at 5, 10, and 20 minutes after the start of the examination. Blood samples were cultured in both aerobic and anaerobic media, and four of these cultures were positive for organisms that are common skin contaminants. No bacteremia was identified from enteric pathogens. Although sex chromosomes have been the focus of a great deal of scientific scrutiny, most interest has centred on understanding
the evolution and relative importance of X and Z chromosomes. By contrast, the sex-limited W and Y chromosomes have received
far less attention, both because of their generally degenerate nature and the difficulty in studying non-recombining and often
highly heterochromatic genomic regions. However, recent theory and empirical evidence suggest that the W and Y chromosomes
play a far more important role in sex-specific fitness traits than would be expected based on their size alone, and this importance
may explain the persistence of some Y and W chromosomes in the face of powerful degradative forces. In addition to their role
in fertility and fecundity, the sex-limited nature of these genomic regions results in unique evolutionary forces acting on
Y and W chromosomes, implicating them as potentially major contributors to sexual selection and speciation. Recent empirical
studies have borne out these predictions and revealed that some W and Y chromosomes play a vital role in key sex-specific
