The purpose of this study was to evaluate the effects of bilateral upper premolar extraction on mandibular growth. Twenty-six subjects (eight males, 18 females) in maximum pubertal growth with an Angle Class II molar relationship, normal to mild overjet increase, mild or no lower arch length discrepancy but severe upper arch discrepancy and no severe skeletal discrepancy were divided into two groups equal in number and gender, as extraction and control groups. The median chronological age was 11.2 years in the extraction group and 12.6 years in the controls. The subjects were observed for a median period of 1.1 years in the extraction group after bilateral extraction of the upper premolars and 1.2 years in the controls until termination of pubertal growth (DP3u) without any orthodontic treatment. Twenty-nine linear and angular measurements were made on 52 lateral cephalograms and hand-wrist radiographs taken before and after the study period. The increase in SNB measured on the total superimposition was significantly greater in the controls than in the extraction group (P < 0.05). In addition, anterior mandibular (counter-clockwise) rotation was only significant (P < 0.05) in the control group. Thus, it might be suggested that bilateral upper premolar extractions might affect the mandibular rotation tendency. 相似文献

The frequency, location, and function of the vomeronasal organ, also known as the Jacopson organ, in human beings remains poorly understood. In this study, a search for the frequency of the vomeronasal organ was performed by nasal examinations of 346 adult patients and 21 cadaver heads by anterior rhinoscopy and videotaped rigid 30 degrees endoscopy. The vomeronasal organ was identified in 112 patients (32%) and in 8 cadaver heads (38%). The location, shape, type, and relation to sex of the vomeronasal organ were described. Ten specimens were examined histologically and histochemically for neuron-specific enolase (anti-neuron-specific enolase), high-molecular-weight cytokeratin (anti-high-molecular-weight cytokeratin), and low-molecular-weight cytokeratin (anti-low-molecular-weight cytokeratin). Considering its variability in shape and the lack of immunohistochemical characteristics of nerve tissue, the present results are not suited to argue for functional significance of the vomeronasal organ in human beings. 相似文献

The aim of the present study was to evaluate the possible benefit of platelet-rich plasma (PRP) in sinus grafting as compared with recombinant human bone morphogenetic protein-7 (rhBMP-7). For this purpose, we performed a bilateral sinus augmentation with anorganic bovine bone and simultaneous insertion of a titanium screw implant in five miniature pigs. Six hundred microliters of PRP and 15%-vol. autologous bone, which was collected with a trap during preparation of the implant recipient site, were added to the right sinus and 420 microl rhBMP-7 to the left sinus. A polychrome sequential labeling was performed. The animals were sacrificed 6 weeks after surgery. Undecalcified ground sections were evaluated by microradiography, digitized histomorphometry and under fluorescent light. The mean bone-implant contact using rhBMP-7 was 45.8% and 5.7% under PRP (P=0.002). The mean height of newly mineralized bone in the augmented area using rhBMP-7 amounted to 8.3 mm as opposed to 3.6 mm under PRP (P=0.013). Using PRP, the mean area of the newly formed bone was enhanced (51.3%) as compared with rhBMP-7 (33.1%); however, this difference was not statistically significant (P=0.081). In conclusion, under the selected experimental conditions the use of rhBMP-7 led to superior outcomes with regard to the osseointegration of dental implants and the height of new bone as compared with the use of PRP. 相似文献

PURPOSE: This finite element analysis was conducted to determine the magnitude of stress in the supporting bone when implants were arranged in either a straight-line or an offset configuration. In addition, the effects of axial and nonaxial loading and changes in prosthesis height were assessed. MATERIALS AND METHODS: An 8-node hexahedral solid-element 3-dimensional finite element analysis model of the mandible was created using PATRAN software. Three titanium endosseous implants were placed in the model 7 mm apart. The center implant was placed on the line from the centers of the terminal implants (no offset), 1.5 mm lateral to this line (1.5-mm offset), or 3.0 mm lateral to this line (3.0-mm offset). Forces of 200 N were applied to a point corresponding to the center of the middle implant when the implants were in a straight-line configuration. Forces were applied in a straight vertical direction or in 15-degree increments to the vertical to a maximum of 60 degrees. Simulated type IV gold prostheses were made to simulate heights of 6 and 12 mm. RESULTS: The least stress in the supporting bone was found with vertical loading of the no-offset implants with the 6-mm prosthesis (3.12 MPa) followed by the same alignment with the 12-mm prosthesis (3.86 MPa). Changing the angle of force application by 15 degrees resulted in increased stress to the underlying bone, and the creation of an offset did not fully compensate for this increased stress. DISCUSSION: In contrast to previous studies, this study examined 3 elements not previously studied together in a single finite element analysis, using the maximum offset defined by normal anatomic contours of mandibular premolar and molar teeth, thereby describing the relative importance of clinically relevant methods for stress reduction. CONCLUSIONS: Vertical loading of an implant-supported prosthesis produced the lowest stress to the supporting bone. Changes in the angle of force application resulted in greater stress to supporting bone. Reduction in prosthesis height or use of an offset implant location for the middle implant reduced stress, but the reduction did not compensate for the increase found with off-axis loading. 相似文献

OBJECTIVE: To assess differences in the aesthetic and functional long-term results of one-stage and two-stage surgical and orthodontic treatment in patients with cleft lip, palate, and alveolus. DESIGN: Sixty adult patients who were operated on as children for unilateral cleft lip, palate, and alveolus were examined. In every patient the lip was closed using Tennison's technique. Thirty patients had soft and hard palate closure in two stages and 30 patients in a single stage. Lateral cephalometric and model analyses were conducted at a mean age of 18.4 years. RESULTS: In the model analysis, transverse narrowing was seen in all patients after two-stage operations and in three patients after one-stage operations. The deficit was more severe in the molar region in the two-stage group and nearly similar in the premolar and molar region in the one-stage group. A sagittal deficiency in the anterior maxilla was found in 26 patients after two-stage operations and in 16 patients after one-stage operations. In the lateral cephalometric analysis, the mean sella-nasion-point A angle in the one- and two-stage group was 78.2 degrees and 76.8 degrees, respectively. The ANB angle was normal in both groups. In both groups the inclination of the midface was low. There was a low posterior facial height. Minor scarring was seen in the single-stage group. CONCLUSION: A more severe impairment of growth of the maxilla in the sagittal and frontal plane was observed after two-stage operations on the cleft palate. 相似文献

OBJECTIVE: The aim of the present study was to determine the surface hardness of light- and auto-cured resins for the fabrication of occlusal splints employing Vickers hardness measurements. MATERIALS AND METHOD: In this study we used three auto-polymerized resins (Palapress, Orthocryl, Steady-Resin M) and four light-polymerized resins (Acrylight, Primosplint, Triad Tran- Sheet Colorless and Triad TranSheet Pink). The Vickers hardness measurement was carried out by means of a universal Durimet indenter applying a test load of 50 g for 30 seconds. The light-cured resins were polymerized in a Tagris Power light oven for 10 and 15 minutes each. Three separate test series were carried out (the hardness of plates under optimal conditions and of occlusal splints was measured, and the curing of light-polymerizing materials in layers of varying depth was evaluated). Data underwent statistical analysis via ANOVA and the Scheffé test. RESULTS: The microhardness determined in each case amounted to values between 10.4 HV 0.5 and 39.3 HV 0.5. The Vickers hardness determined for the plates that had been produced under optimal conditions demonstrated that their surface was significantly (p < 0.05) harder than that of cylinders and splints. The hardness values of the light-cured material Triad TranSheet Pink (39.3 HV 0.5) were significantly higher (p < 0.05) than those of all other resins. In all auto-polymerized resins, the surface hardness of the samples we examined (in the form of plates and splints) was significantly lower (p < 0.05) than that of the light-cured materials Triad TranSheet Pink and Colorless. CONCLUSION: The results we have obtained so far concerning surface hardness indicate that, in the fabrication of occlusal splints, light-cured resins may represent an alternative to auto-polymerizing materials. 相似文献

BACKGROUND: Transforming growth factor-beta(1) (TGF-beta(1)) has significant effects on periodontal host response regulation. Limited knowledge on the role of TGF-beta(1) in various periodontal disease types and particularly in advanced periodontitis forms warranted the present study. The aim of the present study was to evaluate the gingival crevicular fluid (GCF) TGF-beta(1) levels in patients with different forms of periodontal disease. METHODS: GCF TGF-beta(1) levels were investigated in 32 chronic periodontitis (CP), 30 generalized aggressive periodontitis (G-AgP), 15 gingivitis patients and 16 periodontally healthy subjects. Periodontal status was evaluated by measuring probing depth, clinical attachment loss, plaque and bleeding on probing. TGF-beta(1) levels were analyzed by enzyme-linked immunosorbent assay. The results were expressed in terms of total amount (pg) and concentration (pg/microl). RESULTS: G-AgP and CP groups had significantly elevated GCF TGF-beta(1) total amount compared to healthy group (p<0.008). Moreover, GCF TGF-beta(1) total amount of G-AgP group was significantly higher than that of gingivitis group (p<0.008). G-AgP and CP groups had similar GCF TGF-beta(1) total amount (p>0.008). Significant correlation was found between GCF TGF-beta(1) total amount and all clinical periodontal parameters (p<0.05). CONCLUSIONS: The results of the present study suggest contribution of TGF-beta(1) to the pathogenesis of advanced chronic and aggressive periodontitis. TGF-beta(1) may thus be one of the components modulating exaggerated host response together with other major mediators of inflammation. 相似文献
