Pathways to HIV risk and vulnerability among lesbian,gay, bisexual,and transgendered methamphetamine users: a multi-cohort gender-based analysis

Background  

Methamphetamine (MA) use continues to be a major public health concern in many urban settings. We sought to assess potential relationships between MA use and individual, social, and structural HIV vulnerabilities among sexual minority (lesbian, gay, bisexual or transgendered) drug users.     

Ethics committees in India: Facing the challenges!

The past few years have seen a tremendous rise in the number of clinical trials conducted in India. This is been attributed to the huge patient population, genetic diversity, and rich technical pool in our country. However, the economical upsurge in the clinical trial industry has also caused concerns pertaining to the efficiency of the Regulatory Agencies and Ethics Committees (EC). The EC plays an important role in the regulation of clinical research at the local level. However, it is seen that many ECs are oblivious to their roles and responsibilities. It is reported that ECs lack standard operating procedures, do not have a proper composition or adequate representation, thus affecting their functions in regulating clinical research. Moreover, ECs seem to function in isolation, as self-sufficient bodies, having no communication with the regulatory agency or other ECs. This brings forth the need for ECs to come together and share their experiences and observations, with the aim of updating themselves and refining their functions. Efforts also need to be focused on capacity building, centralized registration of ECs, and bringing an oversight mechanism in place. The Ethics Committees in India need to work in close association with forums such as the Forum for Ethics Review Committees in India and the Forum for Ethical Review Committees in Asia Pacific, in an effort towards empowering themselves.     

Right-sided colonic angiodysplasia in a young patient-a case report

The histological diagnosis of angiodysplasia is one of the most difficult problems in G.I.T. pathology. These lesions are presumed to be degenerative in nature, commonly found in population over 60 years of age and rarely in young patients. So, diagnosis of angiodysplasia in this age group is even more difficult. We present a case of massive bleeding from colonic angiodysplasia in a 22-year-old female to increase awareness of this uncommon but potentially life threatening cause of lower intestinal bleeding in younger age group.     

Cardiovascular disease and musculoskeletal disorder labels in family practice acted as markers of physical health severity

Objective

Family practitioner diagnostic labels applied in consultation provide a signpost for treatment and management. Yet, it is unknown whether each label reflects the health of the respective patient group.

Study Design and Setting

Consultation records of 7,799 patients aged 50 years and older from six family practices were linked to a cross-sectional baseline health survey. Associations between six mutually exclusive cardiovascular disease and nine mutually exclusive musculoskeletal disorder categories, and physical health severity as measured by the Short Form-12 questionnaire were examined.

Results

There were 2,447 (31.4%) cardiovascular disease and 3,321 (42.6%) musculoskeletal disorder consulters. The mean physical health scores ranged from 38.38 (95% confidence interval [CI]: 37.8-39.0) for hypertension to the poorest score of health 28.98 (95% CI: 27.5-30.5) for consulters with heart failure, whereas in the musculoskeletal disorder group, scores ranged from 44.85 (95% CI: 42.2-47.5) for soft tissue disorder to 28.79 (95% CI: 26.8-30.8) for consulters with inflammatory polyarthropathy (trend P < 0.001). This trend in the association between diagnostic categories and physical health severity within both spectrums remained after adjustment for confounders.

Conclusion

Specific diagnostic labels for selected chronic illness indicate the severity of physical health for the corresponding consulting population.     

Synthetic LXR agonist suppresses endogenous cholesterol biosynthesis and efficiently lowers plasma cholesterol

The liver X receptors (LXRs) are key regulators of genes involved in cholesterol homeostasis. Natural ligands and activators of LXRs are oxysterols. Numerous steroidal and non-steroidal synthetic LXR ligands are under development as potential drugs for individuals suffering from lipid disorders. N,N-dimethyl-3β-hydroxycholenamide (DMHCA) is a steroidal ligand of LXRs that exerts anti-atherogenic effects in apolipoprotein E-deficient mice without causing negative side effects such as liver steatosis or hypertriglyceridemia. In this report, we investigated the consequences of DMHCA treatment on cholesterol homeostasis in vivo and in vitro. Despite its hydrophobicity, DMHCA is readily absorbed by C57BL/6 mice and taken up by intestinal cells, the lung, heart and kidneys, but is undetectable in the brain. DMHCA significantly reduces cholesterol absorption and uptake in duodenum and jejunum of the small intestine and in turn leads to a reduction of plasma cholesterol by 24%. The most striking finding of this study is that DMHCA inhibited the enzyme 3β-hydroxysterol-Δ24-reductase resulting in an accumulation of desmosterol in the plasma and in feces. Thus, the reduction of plasma cholesterol was due to a block in the final step of cholesterol biosynthesis. Taken together, DMHCA is an interesting compound with properties distinct from other LXR ligands and might be used to study desmosterol-mediated effects in cells and tissues.     

The biocompatibility and separation performance of antioxidative polysulfone/vitamin E TPGS composite hollow fiber membranes

The extended interaction of blood with certain materials like hemodialysis membranes results in the activation of cellular element as well as inflammatory response. This results in hypersensitive reactions and increased reactive oxygen species, which occurs during or immediately after dialysis. Although polysulfone (Psf) hollow fiber has been commercially used for acute and chronic hemodialysis, its biocompatibility remains a major concern. To overcome this, we have successfully made composite Psf hollow fiber membrane consisting of hydrophilic/hydrophobic micro-domains of Psf and Vitamin E TPGS (TPGS). These were prepared by dry-wet spinning using 5, 10, 15, 20 wt% TPGS as an additive in dope solution. TPGS was successfully entrapped in Psf hollow fiber, as confirmed by ATR-FTIR and TGA. The selective skin was formed at inner side of hollow fibers, as confirmed by SEM study. In vitro biocompatibility and performance of the Psf/TPGS composite membranes were examined, with cytotoxicity, ROS generation, hemolysis, platelet adhesion, contact and complement activation, protein adsorption, ultrafiltration coefficient, solute rejection and urea clearance. We show that antioxidative composite Psf exhibits enhanced biocompatibility, and the membranes show high flux and high urea clearance, about two orders of magnitude better than commercial hemodialysis membranes on a unit area basis.     

Mycobacterium avium-intracellulare brain abscess in HIV-positive patient

Mycobacterial opportunistic infections are a major cause of morbidity and mortality among patients living with HIV (PLHIV) worldwide. Nontuberculous mycobacterial (NTM) infection is one of the leading causes of opportunistic infection in patients with advanced acquired immunodeficiency syndrome i.e., with CD4 count less than 50/cu.mm. Mycobacterium avium complex (MAC) is among the most common opportunistic bacterial infections in those patients with advanced immunodeficiency apart from cryptococcal meningitis, progressive multifocal leukoencephalopathy, etc. Common presentations of mycobacterium avium complex are fever, lymphadenitis and respiratory disease. Immune reconstitution disease is also known to manifest with MAC infections in PLHIV on highly active antiretroviral therapy. Very few cases of central nervous system involvement due to NTM infection have been described. We are reporting a case of advanced acquired immunodeficiency who presented with brain abscess due to Mycobacterium avium intracellulare.     

Melanotic neuroectodermal tumor of infancy: Report of a case associated with high urinary excretion of Vanilmandelic acid

Melanotic neuroectodermal tumor of infancy is a rare congenital neoplasm involving the head and neck region in young patients. A case of melanotic neuroectodermal tumor of infancy is presented. This tumor occurred in right maxillary alveolar ridge in a 4-month-old infant. The present case showed an increased urinary level of vanilmandelic acid, confirming that the tumor is originated from neural crest. Clinical assessment, histologic diagnosis, and laboratory findings supported the diagnosis.
