Summary: For the quantitative determination of the testicular size or volume, two groups of apparatus are available: on the one hand, model testes, which are extremely valuable in longitudinal and cross-sectional studies during the period of pubescence, and on the other hand, measuring devices, with which the testicular dimensions can be specified. The latter instruments are helpful chiefly during the adolescent years and the diagnosis of problems surrounding fertility. The testicular volume can be calculated subsequently from the specific testicular measurements. The formula, for computing the volume of an ellipsoid, seems to be the most suitable for this purpose. Appliances also have been developed, which allow the largest (longitudinal) dimension (in mm) and the testicular volume (in cm3) to be read off simultaneously. A tonometer can be useful to quantitatively estimate the testicular consistency, which is a parameter of the testicular integrity at the level of tubular function (spermatozoa production).     

Nitratbehandlung

Organic nitrates are used for the treatment of angina pectoris symptoms, acute coronary syndrome and for acute and chronic heart insufficiency. In spite of the proven effectiveness of organic nitrates for acute cases, long-term therapy is limited by to the development of tolerance. Recent animal experiments, as well as clinical data, show that an endothelial dysfunction is also induced, a disquieting point, especially considering the negative prognostic impact of endothelial dysfunction. Due to these side effects, the published guidelines of the American Heart Association and the American College of Physicians do not recommend the use of long-term nitrates, such as mono- and dinitrate, for the treatment of stable angina pectoris. Rather, if good left ventricular function is present the calcium antagonist amlodipine should be used. In cases of appreciably reduced left ventricular function, there is a current discussion, based on the recently published A-HeFT study, as to whether a combination therapy with organic nitrates and the effective arteriole dilator hydralazine is beneficial for patients with severe heart insufficiency.

Überbrückende Antikoagulation

Patients on anticoagulants of the vitamin K antagonist type may sometimes be scheduled for invasive procedures or surgical operations. In order to minimize the risk of thromboembolism caused by the interruption of chronic anticoagulation for the procedure, temporary administration of anticoagulants with shorter half-lives is required (so-called bridging anticoagulation). The present review outlines the spectrum of risks during this period regarding both thromboembolism and major bleeding. Low molecular weight heparins may be considered the medication of choice for bridging anticoagulation, mainly for practical reasons. Since they require no coagulation monitoring or dose adjustment, outpatient treatment is feasible. Such heparins are not labelled for the indication of bridging anticoagulation. However, based on recent studies of large patient cohorts, evidence of their efficacy and safety is significantly more solid than for unfractionated heparin. A simple dosing scheme for low molecular weight heparins is given here and all requirements are discussed for safe guidance through episodes of bridging anticoagulation.

Molecular genetic techniques and their use in preimplantation genetic diagnosis (PGD) have been widely expanded in recent years worldwide. The reliability and accuracy of PGD have been improved by including newer PCR modifications such as nested PCR, multiplex and fluorescence PCR, or FISH techniques after nuclear transfer. International discussion on PGD concentrates on expanding indications for PGD, for example, diagnosis of inherited predisposition for cancer and sexing for several reasons. In Germany, an extensive discussion on the basic introduction of PGD has been conducted in recent years, but not yet concluded. It is still open to question whether PGD will be allowed at all or introduced with some limitations concerning the spectrum of indications.
