The authors report 100 cases of knee septic arthritis seen during ten years among 2298 patients admitted in the rheumatology department of Brazzaville university teaching hospital. Bone and joint infection is the first hospitalisation cause in this service. Knee is the first septic localization with 42% of cases coming before hip 25%, sacro-iliac 18% and shoulder 7%. Patients are 56 males and 44 females aged from 5 to 79 years old, average 35 years. Most frequently germs are Staphylococcus aureus in half cases, Mycobacterium tuberculosis in 30% of cases and gram-negative bacillus. Contributing factors are AIDS: 12 cases, diabetes mellitus: 7 cases, alcohol-tobacco intoxications: 4 cases, ickle-cell disease: 3 cases, gout and renal failure in 2 cases each. Late diagnosis and consultation, inadapted antibiotic drugs are responsible for important articular damages compromising further functional prognosis. 相似文献

Neural stem cells (NSCs) are self-renewable, multipotential cells capable of differentiating into the three major neural cell types, but the mechanisms which regulate their development are not fully understood. Both basic fibroblast growth factor (bFGF) and epidermal growth factor (EGF) promote the proliferation of NSCs. However, studies on the role of FGFs in the differentiation of EGF-expanded NSCs are still incomplete. We have studied the expression of distinct FGF receptors (FGFRs) in the progeny of EGF-expanded NSCs isolated from E15 rat striatum. In situ hybridization analysis and immunocytochemistry showed a developmentally related expression pattern and a cell lineage-specific distribution of these receptors. FGFR1 and FGFR2 were identified in many early precursors and in the oligodendrocyte lineage. The latter receptor was also present in a subpopulation of astrocytes. FGFR3 was detected in a restricted population of early precursors, in oligodendroglial progenitors, and in neurons and protoplasmic astrocytes of late-term cultures. Basic FGF treatment of the progeny of NSCs increased the proliferative rate of precursors and the number of oligodendrocytes generated, whereas the number of differentiating neurons was significantly reduced. Together these data provide evidence that FGFs modulate the development of EGF-expanded NSCs, and that this is at least partly determined by a cell lineage-specific expression of multiple FGFRs. 相似文献

Despite all the precautions during surgery, it may be assert that obliviousness of a compress or of a mesh remains still possible. In order to define this accident a non-medical term, textiloma, is used, only in hospital environment. Radiology and medical imaging represent the main tool for the diagnosis of textilomas. Plain film of the abdomen and/or ecography may be sufficient for revealing the textiloma. Just in a few cases a computed tomography may be required. In order to demonstrate all these, using two cases, we'll emphasize the main radio-semiological aspect of textilomas, and thus intending to establish an examination algorithm. 相似文献

We studied the effect of transections at the superior cerebellar peduncle during the evolution of amygdaloid kindling. Dentato- and interposito-thalamic pathways, including the ascending fastigial fibers, were transected in 10 rats at the contralateral side of the stimulated amygdala, and in other 8 at the ipsilateral side. A group of 18 rats was used as control. Contralateral lesion significantly slowed amygdala kindling, while ipsilateral lesion decreased kindled seizure duration. Furthermore, when kindled seizures were reached by 6 control rats, transection of the ipsilateral superior cerebellar peduncle led to reduction of subsequent seizures. These specific effects produced by transection of the superior cerebellar peduncle suggest that the cerebellum could exert a tonic effect over the participating circuitry used by the kindling process. 相似文献

Fibroblast growth factor (FGF) signaling has been implicated in the pathogenesis of pulmonary fibrosis. Mice lacking FGF2 have increased mortality and impaired epithelial recovery after bleomycin exposure, supporting a protective or reparative function following lung injury. To determine whether FGF2 overexpression reduces bleomycin‐induced injury, we developed an inducible genetic system to express FGF2 in type II pneumocytes. Double‐transgenic (DTG) mice with doxycycline‐inducible overexpression of human FGF2 (SPC‐rtTA;TRE‐hFGF2) or single‐transgenic controls were administered intratracheal bleomycin and fed doxycycline chow, starting at either day 0 or day 7. In addition, wild‐type mice received intratracheal or intravenous recombinant FGF2, starting at the time of bleomycin treatment. Compared to controls, doxycycline‐induced DTG mice had decreased pulmonary fibrosis 21 days after bleomycin, as assessed by gene expression and histology. This beneficial effect was seen when FGF2 overexpression was induced at day 0 or day 7 after bleomycin. FGF2 overexpression did not alter epithelial gene expression, bronchoalveolar lavage cellularity or total protein. In vitro studies using primary mouse and human lung fibroblasts showed that FGF2 strongly inhibited baseline and TGFβ1‐induced expression of alpha smooth muscle actin (αSMA), collagen, and connective tissue growth factor. While FGF2 did not suppress phosphorylation of Smad2 or Smad‐dependent gene expression, FGF2 inhibited TGFβ1‐induced stress fiber formation and serum response factor‐dependent gene expression. FGF2 inhibition of stress fiber formation and αSMA requires FGF receptor 1 (FGFR1) and downstream MEK/ERK, but not AKT signaling. In summary, overexpression of FGF2 protects against bleomycin‐induced pulmonary fibrosis in vivo and reverses TGFβ1‐induced collagen and αSMA expression and stress fiber formation in lung fibroblasts in vitro, without affecting either inflammation or epithelial gene expression. Our results suggest that in the lung, FGF2 is antifibrotic in part through decreased collagen expression and fibroblast to myofibroblast differentiation. Although the central nervous system (CNS) is considered to be an immunoprivileged site, it is susceptible to a host of autoimmune
as well as neuroinflammatory disorders owing to recruitment of immune cells across the blood–brain barrier into perivascular
and parenchymal spaces. Dendritic cells (DCs), which are involved in both primary and secondary immune responses, are the
most potent immune cells in terms of antigen uptake and processing as well as presentation to T cells. In light of the emerging
importance of DC traficking into the CNS, these cells represent good candidates for targeted immunotherapy against various
neuroinflammatory diseases. This review focuses on potential physiological events and receptor interactions between DCs and
the microvascular endothelial cells of the brain as they transmigrate into the CNS during degeneration and injury. A clear
understanding of the underlying mechanisms involved in DC migration may advance the development of new therapies that manipulate
these mechanistic properties via pharmacologic intervention. Furthermore, therapeutic validation should be in concurrence
with the molecular imaging techniques that can detect migration of these cells in vivo. Since the use of noninvasive methods
to image migration of DCs into CNS has barely been explored, we highlighted potential molecular imaging techniques to achieve
this goal. Overall, information provided will bring this important leukocyte population to the forefront as key players in

Abstract A 45-year-old man with a past history of aortic valve insufficiency presented with recently increasing chest discomfort. Coronary angiography revealed significant stenosis at the mid-portion of the left anterior descending artery and origin of the first obtuse marginal branch. The right coronary artery (RCA) was faintly seen to be free from significant stenosis. (Selective RCA angiography had not been possible.) At surgery while dissecting between the ascending aorta and the main pulmonary artery, the RCA ostium was incidentally found to be located approximately 10 cm above the aortic annulus and 5 cm above the sinotubular junction, between the medial curve of the ascending aorta and the main pulmonary artery. After coronary artery bypass grafts, we repaired the aortic valve and the RCA remained intact. 相似文献
