Exudative macular degeneration and intravitreal triamcinolone: 18 month follow up

Purpose: To evaluate the safety and efficacy of intravitreal triamcinolone after 18 months of follow up in patients with age-related macular degeneration and subfoveal or juxtafoveal choroidal neovascularization considered unsuitable for laser photocoagulation. Methods: Thirty eyes of 28 patients, referred from general eye clinics as well as the private clinic of one of the authors to a hospital-based retinal out-patient clinic, were treated with an intravitreal injection of triamcinolone (4mg). The primary outcome measure was the proportion of eyes with loss of six or more lines on a Bailey-Lovie Chart. The incidence of adverse events associated with treatment was also observed. Results: Of the 20 eyes with initial visual acuity (VA) of 6/60 or better, the vision was maintained (± 1 Bailey-Lovie lines) in 11 eyes (55%), while six eyes (30%) suffered severe visual loss (six or more lines). The VA improved by five to six lines in three of 10 eyes with initial vision of 3/60 or worse. Three of four eyes receiving a second injection suffered either progressive cataract or elevated intra-ocular pressure (IOP) requiring cataract surgery and/or filtering surgery. One of 26 eyes (3%) receiving a single injection showed progression of cataract and elevation of IOP within 6 weeks of treatment and required anti-glaucoma medication for 6 weeks. Progression of nuclear sclerosis 8–12 months after treatment was observed in six of 26 eyes (23%) receiving a single injection. Conclusions: The results of the present study suggest that a single intravitreal injection of 4 mg triamcinolone is reasonably well tolerated by the human eye. The rate of development of severe visual loss was less than reported for historical controls. Because the results are preliminary and uncontrolled, the treatment should not be used routinely until its benefit to patients is established by a prospective, randomized controlled study.     

The cost of providing restorative dentistry in an alternative delivery mode.

This study is based on the Forsyth Dental School experiment, where it was shown that dental hygienists can be trained to perform restorative dental functions which are now the exclusive responsibility of dentists. The focus of the Forsyth study was to determine the length of training time required and to measure the qualitative and quantitative aspects of services carried out by the hygienists. As a follow-up to that study, this paper is concerned with the economic aspects of the personnel substitution suggested by the Forsyth project. The basic hypothesis of the study is that there would be a considerable gain from this change, and that this gain can be measured by estimating the costs of operation in the new setting anc comparing them to costs in the conventional setting. Combining estimated clinic costs and productivity data provides a measure of the per-surface supply price for an amalgam restoration. This estimate is 44 percent lower than usual fees and 29 percent lower than current welfare fees. The magnitude of this difference implies that the introduction of Forsyth clinics would lead to considerable economics in the elivery of these services.     

Hand problems in pregnancy

The most frequently encountered hand disorders in pregnancy are carpal tunnel syndrome and de Quervain's tenosynovitis. Conservative therapy should be initiated to minimize patient discomfort. Appropriate splinting is the cornerstone of treatment in pregnant women. Surgery is usually not indicated in this patient population.     

Vegetable consumption and lung cancer risk: a population-based case-control study in Hawaii

We conducted a population-based study of diet and lung cancer among the multiethnic population of Hawaii in 1983-1985. We completed interviews for 230 men and 102 women with lung cancer and 597 men and 268 women controls, frequency-matched to the patients by age and sex. A quantitative dietary history assessed the usual intake of foods rich in vitamins A and C and carotenoids. A clear dose-dependent negative association was demonstrated between dietary beta-carotene and lung cancer risk in both sexes. After adjusting for smoking and other covariates, the men in the lowest quartile of beta-carotene intake had an odds ratio of 1.9 (95% confidence interval, 1.1-3.2) compared to those in the highest quartile of intake. The corresponding odds ratio for women was 2.7 (95% confidence interval, 1.2-6.1). No clear association was found for retinol, vitamin C, folic acid, iron, dietary fiber, or fruits. All vegetables, dark green vegetables, cruciferous vegetables, and tomatoes showed stronger inverse associations with risk than beta-carotene. This observation suggests that other constituents of vegetables, such as lutein, lycopene, and indoles, and others, may also protect against lung cancer in humans.     

Capitate-radius arthrodesis: an alternative method of radiocarpal arthrodesis

Arthrodesis of the wrist may be indicated for a variety of conditions and can be achieved by many techniques. We have had experience with radiocarpal arthrodesis by fusion of the capitate to the radius after a modified proximal row carpectomy . The common feature in our 10 patients was a flexion deformity of the wrist. In five of the patients it was the result of spastic posturing. Five of the patients had a variety of other conditions. Primary arthrodesis occurred in all patients and the cosmetic improvement was appreciated by all patients. Functional improvement seemed to be most related to the preoperative condition. The follow-up evaluation averaged 5.4 years with a range from 6 months to 11 years.
