Cytomegalovirus (CMV) infections are frequent in patients with severe immunodeficiency. We report a case of isolated digestive localization in a young woman initially non-immunocompromised. Initially, she was admitted in the intensive-care unit for a severe post-operative shock secondary to a respiratory distress syndrome. She then developed severe enterocolitis which was initially unexplained. Outcome was favorable but digestive perforations required multiple surgical digestive resections. The histological diagnosis was confirmed by immunofluorescence staining and the antigenemia and specific antibodies kinetics. We emphasize the various characteristics of this pathology and point out the risk of missing this unusual cause of digestive perforation and severe bleeding. 相似文献

The effect of training on hamstring flexibility has been widely assessed through the measurement of the maximal range of motion or passive torque. However, these global measures do not provide direct information on the passive muscle mechanical properties of individual muscle. This characterization is crucial to better understand the effect of interventions as selective adaptations may occur among synergist muscles. Taking advantage of shear wave elastography, we aimed to determine whether elite sport athletes exhibit different passive shear modulus of hamstring heads compared to controls. Passive shear modulus was measured on semitendinosus (ST), semimembranosus (SM), and biceps femoris (BF) using shear wave elastography with the knee flexed at 60° and 90°, and 90° of hip flexion. A total of 97 elite athletes from various sports including running sprint, figure skating, fencing, field hockey, taekwondo, basketball, and soccer and 12 controls were evaluated. The shear modulus measured at 60° of knee flexion was lower in SM for figure skating (P < .001; d = 1.8), taekwondo (P < .001; d = 2.1), fencing (P = .024; d = 1.0), and soccer (P = .011; d = 0.9) compared to controls, while no difference was found for athletic sprinters, field hockey, and basketball players. Shear modulus of the BF and ST muscle was not significantly different between controls and elite athletes, regardless of the sport specialization (all P values = 1). Among patients with multiple sclerosis (MS), the impairment of exercise tolerance is closely related to disability. Maximal oxygen uptake (VO2max) is the gold standard to assess exercise tolerance in healthy subjects (HS). Among patients with MS, the accuracy of VO2max measurement is often impaired because the patients are unable to reach the maximal exercise intensity due to interdependent factors linked to the disease (such as pathological fatigue, pain, lack of exercise habit, and lack of mobility). This study assesses the accuracy of simplified indices for assessing exercise tolerance, which are more suitable in patients with MS. They are simple in the way they are either measurable during submaximal exercise (oxygen uptake efficiency slopes (OUES), physical working capacity at 75% of maximal heart rate (PWC75%), oxygen consumption at a respiratory exchange ratio of 1 (VO2@RER1)) or not based on gas exchange analysis (peak work rate (PWR)-based predictive equation and PWC75%). All indices were significantly lower in the MS group compared to the HS group (P < .001). OUES appeared highly correlated (r > .70, P < .001) with VO2peak, in both groups, without difference between groups. PWR-based prediction of VO2peak showed a standard error of the estimate of 315 mL min−1 in HS and 176 mL min−1 in MS. PWC75% did not correlate to VO2peak in neither group. These findings suggest an impairment of exercise tolerance functions in mildly disabled persons with MS, independently from other factors. Submaximal indices involving gas exchange analysis or peakWR-based estimation of VO2peak are usable to accurately assess exercise tolerance. 相似文献
